The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.
South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.
The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.
You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. We are currently recruiting support workers who will be able to work on a shift pattern that will include a rolling rota with a combination of days, evenings, sleep in nights and weekends including public holidays. The role can be part time or full time. Support Workers will have to be flexible re shifts. We are currently recruiting support workers who will work on a rota in our supported accommodation for young people aged 16-19 who are primarily refugees and asylum seekers and in the care of the local authority. These young people are preparing to live independently and we offer practical and emotional support to them.
The role requires you to record support to prepare reports, for the commissioning authority using IT as required. Familiarity with Googles docs and sheets is required.
The role of the support worker is to help young people to develop their independent living skills. This can involve being out and about with the young people – going shopping, attending interviews and appointments, support to attend college, helping them to know the area and its resources and, in the home, helping them to cook, clean and develop self-care skills. The evening / night shift is focussed more on tasks that can be completed in the placements or in groups such as cooking, cleaning, support with homework, film nights, recreational activities. Some creativity is encouraged to engage and inspire the young people.
Alongside this you will have some responsibility for light household tasks to keep the house tidy and organised as well as undertaking health and safety checks. The time in the morning will be spent ensuring that the young people who need to be up and out are ready to attend college.
Some experience of working with young people, night working and lone working will be a big advantage.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.
We are looking for a Lead Worker to join the Hillingdon Streat Outreach Team.
You will:
- Lead outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
- Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this
- Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation
- Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
- Represent the team at multi-agency meetings
- Accurately record all work completed within given timeframes
To succeed as the Lead Worker, you will have:
- A strong commitment to providing a high standard of service
- Experience working with people who have rough slept or vulnerable adults and supporting them to access services
- Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently.
- Good communication skills and confidence in public speaking when representing Thames Reach externally
- Experience in forming strong working relationships with partner agencies, being able to challenge them when required
- Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone
- A clean driving license (desirable)

About this role…
We are looking for a Lead Worker to join the Lambeth Temporary Accommodation Support Service. Our team are commissioned to work with, and support individuals placed in temporary accommodation by the London Borough of Lambeth.
You will:
- Assess the housing and other needs of people living in Temporary Accommodation in Lambeth
- Complete referrals for immediate and long-term housing options, and refer people to organisations that can help with other support needs
- Work creatively with the local authority, community-based services, staff and service users to ensure that support is personalised, effective and measurable
- Contribute to evaluation and measuring impact.

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. We support customers to build confidence in order to empower them to use community resources; we promote healthy lifestyles and wellbeing and offer customers choice and control over the activities they would like to participate in.
The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.
