About the role:
No one should leave hospital with nowhere safe to go.
As Homeless Pathway Coordinator, you’ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You’ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision.
Day to day, you’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.
You’ll work closely with the Homeless Pathway Lead, hospital teams, Newham Council and SHP’s wider Health Team, bringing professionals together and helping everyone stay focused on the person behind the case. You’ll also build links with community services, identify gaps in existing pathways and share learning that helps the hospital respond more effectively to people experiencing homelessness.
This is a role where strong relationships can change the course of someone’s recovery. You’ll join an experienced and supportive team, with opportunities to deepen your knowledge of inclusion health, homelessness and psychologically informed practice through training, reflective practice and shared learning.

In a city where hundreds are forced into homelessness every day, our work has never been more needed or more challenging. And we’re not shying away. We’re rolling up our sleeves to make change and helping over 10,000 Londoners every year. We prevent homelessness, provide safe places to live and give people the opportunity to rebuild their lives and transform their futures. And we never give up.
We’re here for Londoners wherever they are on their journey. We start with trust, building relationships that help people feel safe, supported, and ready to move forward. Every day, we put people first in everything we do, challenging injustice and barriers that keep people from the safety, stability and opportunity they deserve. About the role:
Real change rarely happens in a straight line. At our Ashley Road project, you’ll work alongside people who have experienced homelessness, trauma and multiple disadvantage, offering the consistency, practical support and belief that can help them begin moving forward.
As a Project Worker, you’ll hold a caseload and build purposeful, trusting relationships with clients. You’ll complete assessments, develop person-centred support and safety plans, and help people identify goals that feel meaningful and achievable to them. This could include supporting someone to improve their health, address substance use, manage their accommodation, strengthen their daily living skills or reconnect with services and opportunities in their community.
You’ll be there when situations become challenging, responding calmly to risk, safeguarding concerns and periods of crisis while continuing to recognise each person’s strengths and potential. Working closely with colleagues and external partners, you’ll advocate for clients, coordinate joined-up support and help remove the barriers that have previously prevented them from accessing the help they need.
This is a hands-on and varied role where no two days will look the same. Alongside direct support, you’ll contribute to the safe and effective running of the accommodation, maintain clear and accurate records and help create an environment where clients are treated with dignity, respect and hope. WHO WE ARE
We are Zarach, the children’s bed charity, and we’re on a mission to end child bed poverty, giving children the opportunity to engage at school and break the cycle of poverty.
In our nearly eight years as a registered charity, we’ve captured the hearts and attention of many individuals, community groups, businesses and funders who want to support us on our mission. We’ve also been given an incredible media platform from which we have raised awareness of child bed poverty at local, regional and national levels. This support and awareness has seen us deliver over 17,000 bed bundles to children to date!!
WHO WE ARE LOOKING FOR
We are looking to recruit a Family Engagement Officer to join our dedicated team. As a Family Engagement Officer, you will be at the very heart of our frontline service. You will be responsible for receiving and processing incoming referrals, conducting home visits, working closely with local schools, and supporting other local volunteers just like yourselves.
As our ideal candidate you will need to be within commuting distance of our warehouse in Leeds and cover the wider Yorkshire region. Because of this, you must have a full driving license and access to your own vehicle, and be fully willing to travel across the Yorkshire area as our services develop.
You will be someone who has good experience of the broad range of day-to-day case management, working with external partners, volunteer support, and administrative activities that working in a dynamic charity brings, and someone who enjoys the busyness and variety of this.
Alongside your family support and engagement-related skills and experience, we’ll be looking to see that you have a genuine care for our mission and, as such, are self-motivated to do a great job and be a values-led member of our team.
Ideally, you’ll have experience of working with vulnerable families and managing volunteers in the charity sector, due to some of the specialties that come with this, such as safeguarding and partnership working. But charity experience is not essential if you can demonstrate an appetite and track record for continual learning and development across your working life to date.
WHAT WE OFFER
Starting at £26,000 with progression up to £30,000 p/a (FTE)
28 Days, plus Bank Holidays.
Healthcare Cash Plan
Quarterly Staff Recognition Award
A great working environment! One of our colleagues described us as- “Zarach is a wonderful place to work with the mission at the core of all that we do”
To decide if you’d like to bring your skills and experience to bear in the pursuit of our mission, please have a look at the job description, our website and the coverage of us across social media.
Please note that we may close this vacancy early if we receive a high volume of applications. Therefore, we encourage you to submit your CV and cover letter as soon as possible.
