We are looking for someone with an inspiring and creative personality to work as part of a team within the Independent Living Medication Support Service.
You will work with clients diagnosed with a mental health condition and who are living within the borough of Lambeth in their own homes.
The role of the Specialist Support Worker is to enable our customers to develop greater independence and fulfilling lives. As part of your role, you will be required to oversee Medication compliance, provide practical support to those with general needs issues.
Create robust Support and Risk plans, updating as required, and working with a patch list of customers on an agreed basis.
Enable customers to improve their overall wellbeing, through maintaining good physical and mental health, motivating customers to par-take in social activities, and assisting customers to identify goals to improve their quality of life.
Ensure customers have applied for the correct benefits; that customers are managing their rent and utilities, whilst maintaining their home to a safe standard.
Applicants require a good standard of English and Maths, and established IT skills in the workplace, to support the multiple IT systems we use within the organisation. We are looking for staff members who are motivated and keen to positively engage with our customers.
Working pattern of 12 hrs shifts rolling:
W1: 3 day week
W2: 3 day week
W3: 3 day week
W4: 4 day week

Gateway Mental Health is a 24 hour male only high and medium support accommodation service. Gateway is home for 28 customers from Lambeth and Southwark.
Support workers are vital members of the team as they provide support to customers helping them to develop the life-skills they require to move on to less supported accommodations or even independent living.
Support workers will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices, and achieve their goals
Support Workers will have a caseload of customers, for which they are responsible, and are expected to support all customers while on shift, with any immediate support needs and appointments.
Shifts are - 8-4pm 2pm-10pm Monday to Friday and double or 8am-3pm/3pm-10pm Saturday and Sunday. This includes bank holidays
What you'll do:
* Building supportive, trusting relationships with customers and creating a positive atmosphere.
* Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals.
* Create support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customer to progress in their recovery
* Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'.
* Provide support and access to relevant services to enable customers to continue their recovery and empower customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making;
* Undertake support work in partnership with external stakeholders to compliment their interventions;
* Developing productive relationships with partner organisations to improve service outcomes.
* Demonstrate strong commitment to the functioning of the staff team and to support colleagues in providing a high level of service and safety to customers;
* Involving customers in the design, development and delivery of the service.
* Adhering to lone working protocols and maintaining high levels of awareness in relation to the health and safety of yourself and colleagues.
* Ensuring Look Ahead Health and Safety policies and local protocols are adhered to at all times e.g. fire risk assessments, fire alarm checks etc.
* Being responsible for the handover of key information between shifts.

The successful applicant will have experience working closely with a range of services to support individuals to address and/or recover from their dependence on drugs and or alcohol. Or have expertise in supporting customers with complex mental health needs, including dual diagnosis, forensic histories, and personality disorders.
Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the post. Specialist Support Workers provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. Support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support. Working with individuals with complex needs, who may display challenging, chaotic behaviours, regularly test boundaries and have experienced multiple traumas as a result of abuse, neglect and substance misuse and addictions it is integral that the post holder is motivated to support individuals to progress. They should continually hold aspirations for individuals despite multiple setbacks, and display emotional resilience, able to process challenging incidents and remain motivated to deliver excellent support.
You will have excellent knowledge of harm reduction techniques and various treatment approaches for addiction and be confident in the use of these approaches. These should include but are not limited to medication treatment, behavioural counselling and relapse prevention. You will also provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. Provision of care and support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support. Post holders will deliver trauma informed and asset-based support to a caseload of up to 4 complex service users with substance misuse support needs. - The caseload is subject to change depending on the service needs. Shifts are - Early 8-4pm late 2pm-10pm Monday to Friday and double or 8am-3pm/3pm-10pm Saturday and Sunday. With some mid shifts. Working days also includes bank holidays.

We work with a trauma-informed, survivor led framework with intersectional feminism at the core.
The Tower Hamlets Domestic Abuse Service provides emergency accommodation to women and children fleeing domestic and sexual violence. As a refuge support worker you will provide emotional and practical support, such as legal options, housing, benefits and signposting.
You will risk assess and develop safety and support plans with survivors at the centre.
The role is 16 hours per week
Due to the Client and the nature of this role, this position is for Women only.

What you'll do:
* Manage customer referrals, completing Dash Risk Assessments and obtaining and maintaining all relevant paperwork
* Building supportive, trusting relationships with customers and creating a positive atmosphere.
* Oversee On Track implementation from an operational perspective during the initial upload of cases to ensure that any issues are highlighted to management.
* Conducting regular Keyword sessions with each survivor.
* Working proactively with other members of the team to handle the service caseload and administrative responsibilities

You will work within a multi-disciplinary team, providing support to customers with a range of mental health needs - emotional, social, physical or domestic -, who have had contact with the Criminal Justice System.

What you'll do:
1. Keyworking, Support Planning & Recovery
* Hold a keyworking caseload (typically 4-6 customers), meeting each customer regularly for structured keywork sessions focused on mental health, risk, daily living skills and recovery goals.
* Complete and regularly review support plans, risk management plans and crisis plans with customers and the wider team, ensuring they are meaningful, co-produced and linked to clear, achievable outcomes.
* Use a psychologically- and trauma-informed approach to explore behaviour, triggers and patterns, and support customers to understand their own risks and responsibility.
* Ensure customer paperwork (e.g. support plans, risk assessments, reviews and keywork notes) is completed to a high standard and within required timescales, recorded on relevant systems and shared appropriately with the team.

About you:
Experience & Knowledge
* NVQ Level 2/3 or equivalent
* Experience of working with people with significant mental health needs, ideally in forensic or criminal justice settings (secure services, probation, prison, step-down or similar).
* Understanding of recovery-oriented practice and trauma-informed approaches in mental health.
* Awareness of risk assessment and risk management processes in forensic mental health (e.g. working with index offences, relapse indicators, safeguarding, conditions of discharge or licence).
* Experience of keyworking or case management, including multi-agency liaison and report writing.
* Comfortable working in a structured environment with policies, procedures and clear documentation standards, while remaining flexible and responsive to individual need.

Campbell Rd Young Peoples service provides support and housing for 37 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of around 7-10 young people. You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.
What you'll do:
- Building supportive, trusting relationships with customers
- Working proactively with other members of the team to handle the service caseload and administrative responsibilities
- Supporting key customers to set personalised goals in the form of a Support Plan
- Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
- Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
- Proactively manage risk and safety both in and outside of their physical living environment
- Providing support with daily living activities, including practical assistance where skills are not yet developed, to ensure that customers enjoy a high quality of accommodation
