Holder House is set in the leafy neighbourhood of Abingdon and provides care and support to 14 people from 18+ living with, and recovering from, enduring mental health issues. The Team Manager reports to and will be supported by the Registered Manager. 

As well as ensuring the quality of service and support provided to people who use our services, you will line manage front line staff and organise any building maintenance requirements. Whether you’re carrying out an induction, undertaking an investigation, putting team development plans in place or providing support to our business development team, one thing’s for sure – you won't be short of challenge and variety. We’re currently working towards being awarded ‘Outstanding’ at our next CQC inspection and you will play an integral part in helping us to achieve this.

Substantial experience gained managing a caseload of individuals with complex needs is a given, as well as being outcome driven and able to lead by example using your firm but inclusive management style. You’re open and encouraging too and have lots of tact and diplomacy. What's more, you're great at managing conflicting priorities on time, have a flexible attitude to working hours and are willing to participate in on call arrangements. And, if you have experience of carrying out investigations, writing reports/recommendations or managing budgets, even better, although it's not essential as we offer an extensive induction and training programme for managers.

The service operates two sleep-ins per night (paid at £78.30 per 10-hour sleep-in) and an on-call rota (paid at £18.00) to cover a sleep-in/day shift in the event of absence – applicants should be flexible and available to cover sleep-ins and fill in on the on-call rota where needed.

Mid Kent Mind runs a wide range of services, including but not limited to, mental health training, wellbeing courses, one-to-one sessions, and social groups. Community Navigator (Mental Health)

This role is an opportunity to work for Mind BLMK as part of the Milton Keynes NHS mental health service for someone with experience working in health, social care or information and advice, in direct contact with service users.

Working alongside colleagues from Central Northwest London Foundation Trust (CNWL) this role will provide referral management to and from Secondary Services. This will include signposting patients to statutory services including talking therapies and wider community based & voluntary sector support.

The Community Navigator will work as part of the team to deliver on the aims and objectives of the Mental Health Hub whilst representing the Voluntary and Community sector with the NHS.

Service Delivery

• Work within the NHS Mental Health SPA (Single Point of Access) Team to review and process referrals received from GP Practices and other sources including those to be subsequently triaged and assessed by mental health practitioners.
• Actively call patients with a view to assisting them through the process and making their onward appointments into community or secondary care service providers.
• Support the hub to signpost patients and direct them to appropriate services including secondary care, the local Voluntary and Community sector services and activities that support the patients’ wider determinates of their mental health and wellbeing.
• Answer calls, ensure sufficient phone cover, respond to messages/voicemails, provide administrative support to the Mental Health SPA for all services and deal with queries in a sensitive and timely way liaising with clinical or team lead as is appropriate.
• Monitor service inboxes during core office hours, responding to any queries professionally providing helpful information.
• Monitor and action tasks within SystmOne and maintain records of all activities carried out, i.e. outbound calls, calls received, archiving any received correspondence.
• Build good working relationships with the voluntary and community sector, relevant services and activities to ensure signposting pathways are kept up to date as well navigating and developing the setup of new signposting pathways through networking.
• Provide clinical and non-clinical information in appropriate chronological templates to other services.
• Carry out health and safety responsibilities as directed by Managers in line with Mind BLMK’s H&S policies, procedures, and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).
• To bring VCSE experience, knowledge and culture into an NHS team.

Your challenge? To do everything you can to help the individuals we work with to source appropriate work experience placements with local organisations/employers. Day to day, you’ll offer support, careers advice and guidance, helping people to set their own recovery and employment focused goals and provide training in personal and skills development such as CV preparation, interview techniques etc. You’ll also be responsible for establishing and maintaining positive links with partner organisations, local employers and Mental Health teams and have the initiative to develop and promote the service. Aquarius work with families and young people across a range of home and community settings supporting parents to build insight into the impact of substance misuse whilst developing safety and risk management strategies to minimise risk to the wider family.

Your challenge? To support and deliver a new family model within Birmingham working closely with substance misuse treatment providers, and Childrens Health and Social Care. You will deliver specialist interventions as part of our intensive family support programme exploring risk factors such as substance use, parental mental health and reducing parental conflict. This will be delivered through 1:1 support, 5 step focused sessions, education groups, family support groups; and accredited parenting programmes including M-PACT. Applicants must be committed to attending training for parent training programmes, as well as contributing to the delivery of hidden harm and FASD training to professionals across Birmingham.

Aquarius is a charity that helps people and communities overcome the harm caused by alcohol, drugs and gambling. Right now, we’re looking for a self-confident and energetic individual with a good understanding of gambling, substance misuse and health related issues, to provide a vital support service for individuals with gambling related needs.

Working flexibly across the community and within multi-agency partnership settings on an outreach basis, we’ll rely on you to engage people in a variety of interventions, using a person-centred approach. A core part of the role will involve addressing holistic support needs that could include employment related issues, family work interventions, issues of exploitation, physical and emotional health, domestic abuse and support to engage in rehabilitation. Whatever it takes, in fact, to empower them to reduce risk, increase resilience and achieve positive outcomes.

To succeed, you’ll need significant experience of engaging effectively with clients in an addictions or social care setting. You will hold a caseload of complex cases requiring a case co-ordination approach. Requiring you to complete all recording and case management administration for your caseload accurately and promptly,

• Providing screening, co-produced assessment, psychosocial interventions, integrated and outcome focussed care planning, after-care support, and onward transition to other services as required
• You’ll also need a flexible approach, excellent record keeping and report writing skills and a willingness to work flexibly across the community and within our multi-agency partnership settings, on an outreach basis.
• Due to the geographical area of the role, a driving licence and access to a car is desirable

Right now, we’re looking for someone like you to join our NGAGE Young People’s Service in Northampton. NGAGE is an early intervention drug and alcohol service for young people aged 10-18 and we provide information, education, advice and support to young people in relation to drug and alcohol use.

Your challenge? To provide advice, assessments, brief and structured interventions and group programmes. As a Practitioner, you will be the initial contact for referrals into the service, conducting assessments via telephone, face to face and offering relevant and appropriate advice, harm minimisation, family interventions risk assessment and management, actioning onward referrals to partner agencies. You will also be required to manage a caseload.

An energetic and confident self-starter, you have a qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). Alternatively, we’ll consider candidates with experience of working in the substance misuse field with a commitment to complete NVQ Level 3 Health and Social Care. As well as good knowledge of alcohol/drug and health related issues, you're used to liaising with voluntary and statutory agencies and health professionals and comfortable engaging with clients in a variety of service delivery settings. You will also have the ability to collaborate and work professionally with service users, families and external agencies to provide a service which maximises outcomes for service users. What’s more, you have a flexible approach, a commitment to the principles of confidentiality and excellent record keeping and Microsoft Office.
