Social Prescribing Link Worker

We have a rewarding opportunity for a Social Prescribing Link Worker to provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.

Position: Social Prescribing Link Worker

Location: Banstead/Hybrid (combination of GP surgeries, community and family centres, home visits, working from home/ office)

Hours: 37 hours per week

Salary: £30,300 per annum

Contract: Fixed term 12 months (subject to funding)

The Role

As a Social Prescribing Link Worker, you will take referrals from a wide range of agencies, working with GP practices within primary care networks, pharmacies, multi-disciplinary teams, hospital discharge teams, allied health professionals, fire service, police, job centres, social care services, housing associations, and voluntary, community and social enterprise (VCSE) organisations (list not exhaustive).

Taking a holistic approach, you will develop trusting relationships by giving people time and focus on ‘what matters to me’. The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the link worker role – e.g. when there is a mental health need requiring a qualified practitioner.

About You

You will have a diploma or degree in appropriate subject e.g. psychology, counselling, social work, mental health, Health and Social Care (Level 3) or NVQ Level 3+, along with the below knowledge/experience:

  • Working directly in a community development context, adult health and social care, learning support, or public health/health improvement
  • Supporting people with their health and wellbeing
  • Partnership/collaborative working and of building relationships across a variety of organisations
  • Knowledge of the wider detriments of health, including social, economic and environmental factors and their impact on individuals, communities, families and carers

Please note: A driving licence is desirable, however candidates without a driving licence will be invited to demonstrate how they could fulfil the requirements of the role without one.

About the Organisation

The charity is a mental health and emotional wellbeing charity, supporting Surrey residents since 1994 in Banstead, Elmbridge, Epsom & Ewell and Mole Valley.

Offering a variety of face-to-face and online services, available to adults (aged 16+) living in Surrey who would like to restore and/or maintain their mental and emotional wellbeing (no diagnosis needed). The services are free of charge, with no waiting list and no time limit.
