Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. We support customers to build confidence in order to empower them to use community resources; we promote healthy lifestyles and wellbeing and offer customers choice and control over the activities they would like to participate in.
The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.

What you'll do:
Build supportive, trusting relationships with customers and creating a positive atmosphere
Undertake key-working responsibilities for a caseload assigned by the Contract Manager
Provide customer medication support and ensuring that medication procedures are followed
Conduct regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
Ensure ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
Provide support with daily living activities, as well as practical assistance to support customers to independent living e.g. budgeting, cooking, community activities
Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions.
Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available
Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
Develop and maintain links with all key agencies and service providers in the local community
Empower customers to ensure they receive the service and benefits they are entitled to

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. Windsor Road service offers 11 beds to people experiencing mental ill health. We are a CQC registered residential care home. We offer individual one to one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days. We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. We also work closely with other agencies and our residents wider care teams, so good communication skills are required. And of course, all of this is recorded on our information management systems, so good record keeping and documentation is also essential. Our service consists of 8 24/7 Recovery Workers, a Service Manager and Registered Manager.

About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. Oaklands & St Kilda is a pair of 6-unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living.
