As an Annual Health Practitioner, you will provide support to local health services with their SMI (Severe Mental Illness) clinics. You will be key in shaping this role as the service develops as well as providing support to the core Access Community Mental Health team. The Annual Health Check Practitioner will also provide 1:1 support to encourage and enable people to attend their annual health checks.

What You'll Do: -

Collaboration and Efficiency: Work hand-in-hand with existing Mental Health Provision for people and their families, as well as other local agencies/organisations to effectively support people transitioning from services.

Innovation and Empowerment: Channel your passion into providing evidence-based interventions, becoming key in shaping this role as the service develops as well as providing support to the core Access Community Mental Health team.

The motivation behind the LifeSkills programme is to inspire millions of young people and equip them with the key skills to move forward into the 21st century workplace. The programme focuses on teaching skills such as CV writing, interview skills, networking, problem solving, creativity, resilience, communication and managing online reputation. This combined Programme Manager, Facilitator role will be instrumental in raising awareness and developing skills in schools across Wales.

This post requires daily travel within South Wales.

The role will involve:

• A thorough understanding of the ready-made online resources to facilitate LifeSkills workshops, online and in person
• Creation and management of a deployment plan to achieve agreed targets for students and engagement of Barclays staff
• Maintaining systems for timely and accurate monitoring, evaluation and reporting of the effectiveness of the LifeSkills programme in line with contract requirements
• Sharing best practice and collaborating within and across BITC’s programme managers/facilitators in Scotland and Northern Ireland

The successful candidate will be:  

• Managing the delivery of programmes to meet contractual obligations
• Working with business, schools and/or community organisations to create mutually beneficial and sustainable relationships
• Delivering highly engaging workshops which respond to learner needs and remain true to workshop objectives

The role requires travel across the region for programme delivery and participant interaction.

Salary:
The salary band for this role reflects the flexibility we offer with the contractual location. Candidates will be based at home and have personal transport to travel to schools across Wales. This role is 28 hours per week until 31 July 2025 with a full-time equivalent salary of £27,945 to £32,000 per annum (this will be prorated for part-time hours).

REQUIRED SKILLS

Cranstoun is an independent charity and leading provider of specialist treatment, harm reduction and recovery services to those affected by drug and alcohol use. This is an exciting opportunity to become a part of our high performing psychosocial drug and alcohol service in HMP Birmingham.

We provide a range of services for those affected by drugs and alcohol. Our skilled and compassionate people work closely with services users to change and save lives. We combine proven expertise in treatment and recovery with innovative approaches and actively involve those we help in improving the design of the services we provide.

You will be based in our psychosocial team working with adult males (over 18 years old). You will be responsible for delivering a range of elements of our substance misuse work including case management, assessments, care planning, the delivery of one-to-one interventions to include harm reduction and motivational work, group work, and engagement support as well as other associated duties. You need to be committed supporting clients to make positive changes in order to prepare them for release into the community, and you will arrange continuity of care. You will work in collaboration with the clinical team and integrated working is a key element of this role. You will work within the prison setting and contribute towards the wider drug strategy.

Having experience of supporting people with complex health needs including substance misuse, you will bring knowledge and commitment to deliver drug and alcohol treatment. You will have a non-judgemental attitude and a desire to support the lives of our service users to be healthier and safer by delivering a high-quality service.

Experience of working with drug and alcohol misuse and individuals is essential. You will be a competent communicator, with the ability to work independently and be comfortable and enthusiastic, both with the highlighted client group and working in partnership with other service providers. Previous experience or knowledge of working with clients on opiate replacement treatment, and/or involved in the criminal justice system will be advantageous.

The role of Community Connector is fundamental to the developing of these innovative new teams and mental health services based within networks of GP Practices (Primary Care Networks). Indeed, whoever takes on this particular challenge will be instrumental in supporting the ongoing development and mobilisation of integrated primary care mental health services in the future. Along the way, you’ll work jointly with a Mental Health Practitioner to conduct assessments. We’ll also rely on you to help patients identify their socially determined needs and goals, provide self-management tools, and develop personal support plans. Delivering a range of motivational and structured psycho-social interventions, promoting independence through an enabling asset based approach and developing effective relationships with a range of agencies to facilitate a ‘joined up’ approach - these are just some aspects of this vital role.

As well as proven relevant experience, you’ll need an appropriate diploma or degree i.e., in counselling, psychology, social work, probation, mental health, Health and Social Care (level 3), NVQ’s level 3+ and/or community experience of drug, alcohol, mental health work. You’ll also need the ability to work co-operatively and flexible as part of a multidisciplinary team from a service hub in one of three locations and be willing to travel to and from a number of different locations on a daily basis. We're seeking a skilled and dedicated individual to join our team as a Young Person’s Worker. In this role, you will play a vital role in providing support to at-risk young people who are experiencing homelessness.

The Junction is an out of hours “calming café” service which is open 365 days of the year to support people experiencing pre and/or mental health crisis. The aim of the project is to alleviate the need for people within the Swindon locality to access A&E and/or acute mental health services.

As a Wellbeing Practitioner, you will be providing a preventative, person centred approach to people accessing the service who may be experiencing a mental health crisis.

The service is open from 4PM-11PM and there are positions available for part time hours and full time hours and  working patterns will include some weekends.

We are looking for a full time Peer Engagement Practitioner.

This is an innovative new service, integrating the expertise, skills and knowledge of the network in Barnet with NHS Mental Health services. The purpose of the role is to provide peer led psycho-social support to clients, working within specialist Mental Health teams and the VCS delivery partners. This role will also serve to enhance the work of the MH teams, and other practitioners within the service, to ensure it remains clients focused, recovery informed, and embedded with experience led practice.

Job Title:                      Foundation/Domestic Abuse Prevention Practitioner

Responsible to:             Foundation Manager

Working Hours:             Full time 37.5 hours per week

Salary:                            £27,000 – £32,000 per annum

About the role:

The Foundation/Domestic Abuse Prevention Practitioner (FDAP) will play a key role in working directly with individuals using harmful/abusive behaviour within their intimate relationships. The role will include suitability and risk assessments, delivery of interventions, facilitation of group programmes, risk identification, information sharing and preparation of reports for courts and other agencies.

The FDAP Practitioner will be co-located into Priority Crime Teams situated in Newport Police Station to assist with the identification, screening, and engagement of individuals using harmful behaviour in their relationships, working closely with police to provide a timely and streamlined process for earlier identification, referral, and engagement/disruption of high harm perpetrators of domestic abuse.

For this role is it essential you have excellent understanding of domestic abuse, including the impact on victims and their children. Experience of working with individuals experiencing domestic abuse/ and or causing harm. Experience of group work delivery is desirable; however, we will consider applications from candidates who demonstrate an aptitude for the role.

You will need to be able to build and maintain effective working relationships with statutory and voluntary agencies to deliver the best possible outcomes within a multi-agency setting. As well as being tenacious in support of your clients and adept at problem solving.

IT literacy is a must and candidates must evidence both the ability to use bespoke packages and excellence in their recording practice.
