We are seeking Senior Community Support Workers (Positive Behaviour Support)to join a new short break residential home in Wythenshawe. You will provide first line management to a team of Community Support Worker (Positive Behaviour Support), developing their capability through demonstrating the right values and behaviours, mentoring staff, and supporting their ongoing development.

You will work alongside other practitioners within the residential home and community, to upskill and increase the resilience of children and their families. The residential home benefits from excellent public transport links and convenient access to community resources. Successful candidates will receive extensive training and on-the-job coaching from a well-qualified clinical team, Registered Manager, and Senior management team. Additionally, staff employed within the service will have the opportunity to receive formal Positive Behaviour Support (PBS) training and coaching from qualified behavioural specialists.

Working Pattern – 37.5 hours per week, worked flexibly over five days. Weekend working will be required.

Salary – £25,546 (£13.10ph) rising to £27,590 (£14.15) from April 2024 based on incremental pay progression.

25% pay enhancement for all weekend hours worked.

Location – Wythenshawe, Manchester.

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. The support takes the form of weekly or bi-weekly visits to the client’s home, as well as contact by phone and email and liaising with social care and health professionals as directed by the client’s wishes or needs.

This role requires the worker to work independently and manage their own caseload. With other members of the Dementia Services team, the Dementia Support Worker will also help with transport, group activities, trips, events, training and local forums.

We are seeking a Part Time Dementia Support Worker (up to 21 hours per week)

We’re Causeway, the UK-wide charity who believes in the power of lasting change. Since 2005, we have expanded across multiple regions and now provide accommodation and outreach support to hundreds of survivors of modern slavery every year. In 2018, we collaborated with South Yorkshire Police in an initiative called Plan B. This helped to launch the LifeNavigate department. Using, a trauma-informed approach, we are fully committed to breaking the cycle of crime, alongside other organisations such as South Yorkshire Police and Merseyside Police. Our vision is to ensure a thriving future for marginalised and vulnerable people, and it would not be possible without our dedicated team of staff and volunteers.

We currently have an exciting opportunity for a Male ReFocus Advocate within our LifeNavigate Team based in Merseyside. The aim of this project is to support and signpost individuals who meet relevant criteria and show a motivation to change after being identified as being at potential risk of committing low level Domestic Abuse. A key element of this project is that it is survivor focused and looks to address abusive behaviours at their source, at an early intervention phase, via support and education. This means working with both those who have carried out such acts and those who have been made subject to them. The successful candidate is expected to support a caseload of individuals in order to jointly assess and manage any initial risk and to ensure all safeguarding protocols have been followed. The role is not that of and IDSVA, although some of the skills will overlap, the is to provide on-going support to individuals in areas
of need in which they feel disempowered and hence build confidence and skills. This process will happen in partnership with the organisation DMAT who will run ‘See the signs’ course which will look to educate and equip individuals to feel safe and empowered in this area. You will be working in an agile and responsive team environment and be part of the team providing support and pathways to those who have experienced domestic abuse. When capacity dictates, there will also be a requirement to work with people displaying potentially abusive or coercive behaviours (perpetrators), offering education and support to promote long term change. Due to the nature of working with victims of gender based abuse it’s important we have the flexibility to offer victims the opportunity to be supported by someone of the same gender, because of this we are currently recruiting for a male applicant.

Responsibilities:
• Providing support for individuals who are subject to low level Domestic Abuse (primarily male victims).
• The full time Advocate will oversee up to 16-20 clients of varying levels of support need at any one time.
• To work effectively as a team, including giving responsive feedback and handover information to Line Manager, and colleagues where there is a shared caseload, especially in regard to partnership with DMAT.
• To independently meet deadlines, including accurately recording and logging details of support, documenting relevant service user information and completing other administrative paperwork in adherence to the terms of the contract.
• To risk assess using the DASH model and take appropriate steps to safeguard service users.
• To create and follow individual support plans and ensuring all service users safety and well-being is paramount at all times.
• To arrange appointments for individual service users, and liaise with organisations and individuals regarding service users in a professional manner.
• Signpost service users to appropriate support.
• Supporting service users face to face (where suitable) in an appropriate environment, including using remote sessions where needed.
• To facilitate engagement of service users on the ‘See the Signs’ course, including journey planning and move on strategies, within the required time frame.
• To regularly attend supervision meetings with Line Manager. Adhering to all internal procedures regarding Causeway and any relevant legislation especially with regard to the Domestic Abuse Act (2021).
• When required, to work with perpetrators of low level domestic abuse, facilitating their engagement with the ‘See the Signs’ course, ongoing risk assessment, and provision of holistic support to promote lasting change.
• Promoting Causeway in adherence to the terms of the contract – this may include attending events on behalf of Causeway as a whole.

About the role

We are looking for a dynamic, creative, innovative team player to join our Young People and Families Service in Scotland. This role will be based in Scotland, ideally close to Glasgow but will support service delivery across Scotland and the UK.

You will act as the bridge between young people, their families and professionals – especially in healthcare settings where you will play a unique role working collaboratively with multidisciplinary teams to meet a range of needs. You will support young people during the pivotal transition from paediatric to adult services and life, making sure it is a time of empowerment and growth.

About you

• Experience of working with the third sector, preferably within the fields of youth/community development work and/or health and social care – with a passion for improving the lives of young people.
• Understanding the importance of self-management and experience of delivering interventions to people living with long term conditions. Experience of taking a person-centred approach when providing support.

Community Connectors will support patients to access a wide range of community services and resources including social care, housing, family, debt and employment counselling that support maintaining good health and wellbeing.

Are you looking for a challenging and rewarding role where you are able to support young people by providing a safe environment for them to feel safe and comfortable?

You will be working as part of the team in Horsham looking after young people aged 16 -17 in a warm and homely environment. Young people come to us from a variety of backgrounds and have various and often complex needs. The team is made up of passionate and caring individuals, who work together to provide holistic support to the young people in our care, 24 hours a day, 7 days a week. Our specialist supported accommodation is a safe and secure environment where young people are supported to acquire the necessary skills in preparation for living independently and can safely explore their increased freedom and choice and develop the responsibilities associated with adulthood.

Your primary role will be to keep our service and young people, safe. You will help to contribute to a great working environment, with a calm, yet assertive manner, being able to handle potentially difficult situations. In addition, you will complete health and safety checks, ensuring the service is clean and tidy and reporting any repairs. You will be lone working as part of the role, so you will need to be comfortable with decision making and following procedures.

You will work a shift of 8pm-7am on a four night on, four night off rota. Although these are long shifts, you will get four days off after, which can provide a great work life balance.

Ideally you will have experience working with young people, those considered vulnerable or with multiple and complex needs along with being able to demonstrate a real passion for wanting to work with young people. Job Title: Community Nurse

Team/Directorate: Community Team

Job purpose

The post holder will be responsible for providing holistic care and support to children and young people (CYP) who have life-limiting conditions and their families as a key member of the community team. This care is provided in the family’s own home, in the community, and support into the hospices and acute settings.

The post holder will be the first point of contact for families following acceptance to the service and the nurse is responsible for undertaking a holistic assessment of the child and family’s needs. The post holder will provide high quality palliative care to CYPs under the care of Shooting Star Children’s Hospices, using their skills and expertise in a diverse range of situations, autonomously and as a member of the community team. The post holder will have excellent communication skills, be a sound educator, and work proficiently.

They will work closely with the SSCH in house team, family support team and symptom management services to ensure a seamless and comprehensive service to CYP and their families.

SSCH care is provided 24 hours a day, 365 days a year, the post holder will be required to work flexibly to meet the needs of planned and emergency care, including on calls overnight and weekends in addition to contracted hours.

Key tasks and Responsibilities

• Providing professional clinical leadership in the provision of palliative care across care settings for CYP under the care of SSCH.

• To work in partnership with the SSCH care teams and wider MDT, demonstrating excellent communication skills to ensure high quality seamless care.

• To ensure that holistic care is family centred, research based, and meets with all statutory requirements and adheres to clinical governance initiatives and best practice.

• Provide support to CYPs and their families where required through community visits to hospital, home or hospice alongside relevant external professionals from statutory services.

• Maintain an awareness of the multi-cultural nature of the SSCH catchment area, adapting care culturally as required.

• To ensure all documentation is maintained in accordance with SSCH policies, guidelines and standards.

• To act in accordance with the SSCH and statutory Safeguarding Children Policy and Procedure.

Clinical

• To be actively involved with the on-going development of the SSCH care service, in particular the community team, contributing to development through research, audit and quality improvement.

• Provide clinical support and continuity of care to CYPs with palliative needs.

• Work alongside specialist symptom management teams to assess symptoms and provide clinical feedback to form part of decision making under the lead of the specialist services.

• Ability to follow an agreed symptom management plan and liaise with teams to offer high level care in the home as needed.

• Working in partnership with all members of the MDT, encouraging and promoting effective team working from all disciplines within SSCH.

• Attend professional meetings as a representative for SSCH and provide proactive input to ensure high level care for CYPs and families.

• Coordinating transition to adult services when appropriate in conjunction with the transition coordinator.

• Ensure written communication and correspondence is used to communicate with external professionals and services.

• Form part of the community on call service in conjunction with statutory services to endeavour to offer a responsive 24/7 service in end of life care.

• To work to the SSCH medicine policy and adhere to the NMC guidelines at all times when giving medication.

The Margins Project, Union Chapel’s Frontline charity, work with people who are homeless or who are at risk of homelessness and helps address pertinent social justice issues these communities face.

The charity runs three integrated programmes:

• a community drop-in programme providing access to nutritious hot meals, and limited access to showers & laundry services.

• advice and engagement programme providing one-to-one as well as group advice service on benefits and housing, and onward referrals to pertinent services.

• support employment programme offers a paid catering training to 10 people who have had experience of homelessness, substance misuse, mental ill-health, and or imprisonment.

Our range of services are designed to help guests (service users) at varying stages, from having no roof over their head to getting back into employment.

This role offers an opportunity for an experienced adviser to help shape and evolve Margins services. Experiences on the ground at the drop-in programme inform the charity’s strategic direction. The role will involve expanding our partner network to link into pertinent statutory and other charitable organisations that provide services that will help our guests with their housing, substance misuse, recidivism, and mental health challenges.

We work closely with people on an individual case work basis to get guests off the streets, into hostels or temporary accommodation where possible, and work with private and social landlords on a range of issues. We support and represent the people we work with on the phone and in person, including at the council, the doctors, the job centre, and benefits tribunals.

Our drop-ins run on Mondays and Wednesdays 11:00am to 1:00pm where attendance is currently from 65 to 75 guests. The advice and engagement worker works more intensively with smaller numbers of up to 20. The demand for more follow up work and advocacy support has been increasing over the last two years and this in-house advice worker is critical for this demand.

Working collaboratively with the Drop-in Community worker (currently vacant), the head chef, and a small group of dedicated volunteers, you will be responsible for the support of a caseload of guests, some with complex needs, and will undertake key work responsibilities including registering the guests, identifying their housing, benefit and personal needs and conducting all necessary follow ups including occasional advocacy support, where deemed necessary.

Job Title: ADVICE AND ENGAGEMENT WORKER

Purpose of Job:

• To advise and assist guests (service users) with benefit claims and related issues, e.g., Housing Benefit, DLA, PIP, Income Support, ESA, Universal Credit, and other benefits. In particular, assisting them to claim Housing Benefit and UC to reduce risk of rent arrears.

• To develop significant contact and liaison with comparable housing and benefits advice organisations.

• To encourage guests engage with Margins activities, and where practicable, with the wider Union Chapel activities.

• To develop, facilitate and deliver group advice workshops/training programmes and open discussions forums to engage guests who historically do not engage with Union Chapel.

• To Refer Margins guests to relevant internal and external services.

• In liaison with the head of Margins and the chef to refer potential candidates to Margins’ Supported Employment Programme.

The Advice and Engagement Worker will have an active role in identifying and engaging guests as well as referring potential candidates to the Supported Employment Programme, a programme anticipated to expand beyond its current services in Margins catering.

Responsible For:

• Working with caseload of guests

• With the support of drop-in services community worker (currently vacant) to ensure the smooth running of the drop-in services

• Identifying opportunities to improve services both at a project and organisational level.

• Assist the Head of Margins in ensuring that the Margins Project remains GDPR compliant.

• Working in partnership with statutory and other charities to deliver co-ordinated support to guests.

Key Duties:

• To encourage and assist guests achieve the maximum income from benefits to which they are entitled to, and to refer to relevant support services such as mental health, social services, community support.

• To advise guests on basic budgeting and where applicable to refer to Tenancy Support Officers or outside agencies for more complex help.

• To liaise closely with local authorities, social, and private landlords to try to achieve the best outcome for our guests.

• To identify if the guest is accessioning the right benefit that will support their long-term wellbeing.

• To support newly arrived refugees with their housing and benefit advice needs.

• To actively assist guests with formal appeals against Local Authority, DWP and HMRC decisions regarding benefit entitlement. Representing at Appeal Tribunals.

• To signpost guests to resources that can assist them in finding work.

• When needed, signposting guests to specialist providers in mental health or drug and alcohol support services.

• To Support guests on paying their rent, including providing advice on benefits and debt management.

• To Address breaches of occupancy agreements and develop strategies that would prevent and avoid eviction.

• To assist guests with backdated claims for Benefit by supporting their applications through to completion.

• To ensure all guests’ files are up-to-date and that data is captured on our management system.

• To provide written reports and statistics as and when requested.

• To attend supervision, training, and meetings as and when required.

• To provide advocacy support to guests as and when required.

PERSON SPECIFICATION

Essential:

• Good knowledge of the benefits system, including Universal Credit.

• Experience of working with homeless and marginalised people and understanding of the issues that face them.
