About this role…
Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
We are looking for Support Workers to join the East Surrey Outreach Service team.
You will:
• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping across four districts.
• Working closely with Lead Workers and Assistant Support Workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this.
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation.
• Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
• Accurately record all work completed within given timeframes.
• Have a UK Driving Licence.
To succeed as the Support Worker you will have:
• A strong commitment to providing a high standard of service.

We are seeking to recruit a highly skilled Key Worker for our Key Worker Service in East Berkshire. The role will be diverse and service user centred and would suit someone with significant experience in supporting young people and young adults with Autism and Learning Disabilities and their families.
The day to day work will involve a flexible approach to coordination and direct support for the young person and their family, as well as providing responsive communication at times of increased need. This is an exciting role which allows for the Key Worker to support families to navigate the system whilst advocating for them and ensuring that their voice is heard.
As the service is new there will be opportunities for collaboration and co-production and our vision is to provide young person and family centered support, whilst influencing system change to achieve better outcomes for more children, young people and adults under the age of 25.
The service will be provided across the entire East Berkshire footprint covering: Slough, Royal Borough of Windsor and Maidenhead, Bracknell Forest.Although the team will be “virtual”, there will be frequent travel within and across the East Berkshire area, therefore being a car driver with access to a car is essential.

About the role:
As a Resettlement Worker and Domestic Abuse Specialist, you will work in person and remotely with clients who have support needs, in particular, those who are survivors of domestic abuse. In conjunction with other members of the Partnership, you will support our clients to engage with services and find accommodation solutions. Main duties:

Hillside Clubhouse is recruiting for a mental health employment specialist to join our dynamic Camden team supporting people with mental health conditions to gain and retain employment. Hillside Clubhouse is a co-produced mental health employment support charity working across Camden and Islington. People can access our services via our Clubhouse, where they can share their skills and expertise with others and support the running of the charity. Alternatively, they can receive support out in the community from one of our employment specialists based within primary and secondary care.
The Role:
The role is within our IPS Service and will be based within a Camden NHS secondary care team. As an IPS Employment Specialist, you’ll play an important role in supporting people with lived mental health experience on their recovery journey by supporting them back into paid employment. This is an opportunity to offer ongoing support, to motivate and provide people with hope, while developing the tools and skills required to recover, ultimately transforming their lives.
A variety of skills are needed in this post including but not limited to - a strong ability to develop empathy and rapport with clients; a determined approach to seeking out job opportunities; a person centric-approach to working with people; strong communications skills to motivate clients; strong administrative capabilities and an attention to detail engage with prospective employers via business development and face-to-face meetings, and to be able to challenge some perceptions around mental health.

Overall aim:

Primary Duties:
- Manage a caseload of up to twenty clients and work within and apply the 8 principles of IPS in all aspects of role.

This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.
The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.
You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.
Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. The service facilitates peer support and delivery of monthly workshops and drop- ins. The service will provide first-class, person-centred information, advice and guidance to people with learning disabilities through both one-to-one and group support.

Support will be flexibly based operating out of a central Bromley office but delivered at various venues across the borough. About the Service
Our refuges provide emergency accommodation to women and children fleeing domestic abuse and sexual violence, last year nearly 1000 women and children found a safe place to stay in one of our refuges. Refuge Case workers provide emotional and practical support this can include housing support, legal options, reporting to the police, benefits, financial matters and accessing therapeutic support services.
About the Role
You will be a keyworker for a caseload of (on average 6-10 women) living in or moving on from the refuge. You will be with them from the start of their journey to the end, welcoming and helping them settle when they arrive to when they are ready to move on to their next home. This includes carrying out risk and needs assessments and developing support plans. Your work with service users will ensure their practical needs as well as emotional needs are taken care of. You also provide a vital role in keeping the service a safe and secure environment for the women and children who now call the refuge home. It is a face paced, target driven role.
About You
Key to this role is experience of working/volunteering with anyone who has experienced any form of social disadvantage. A knowledge of domestic abuse and its impact on women and their children is important as well as an ability to build supportive and empowering relationships. You must be open and willing to learn new skills which will build on your working practice with service users to co-develop and deliver support planning, risk assessment & linking in with other agencies where needed.
Knowledge and experience of risk assessing, case management and legal remedies, housing and welfare benefits is ideal however not required as we are confident that we can train around these.
