The role 

We are looking for Housing Support Workers who can provide intensive housing management & support to service users in our supported housing projects. 

You will work proactively with service users who have been homeless and have complex needs to sustain their tenancies and move on into independent living.

You will also work empathetically to empower services users, actively engaging them with the community and helping them to support each other, to develop resilience, to live independently, and to be an asset to the community. 

The right fit 

• You will have prior experience with service users who have been homeless and/or have complex needs including, Mental health, substance misuse, domestic abuse & chaotic lives etc. 

• You will also have strong ICT skills. 

• You will be competent in facilitating support sessions and devising support plans to achieve successful outcomes. These will be person centred in their approach, identifying individual needs and strengths of each Service User.

We are currently recruiting support workers who will be able to work on a shift pattern that will include a rolling rota with a combination of days, evenings, sleep in nights and weekends including public holidays. The role can be part time or full time. Support Workers will have to be flexible re shifts. We are currently recruiting support workers who will work on a rota in our supported accommodation for young people aged 16-19 who are primarily refugees and asylum seekers and in the care of the local authority. These young people are preparing to live independently and we offer practical and emotional support to them.

The role requires you to record support to prepare reports, for the commissioning authority using IT as required. Familiarity with Googles docs and sheets is required.

The role of the support worker is to help young people to develop their independent living skills. This can involve being out and about with the young people – going shopping, attending interviews and appointments, support to attend college, helping them to know the area and its resources and, in the home, helping them to cook, clean and develop self-care skills. The evening / night shift is focussed more on tasks that can be completed in the placements or in groups such as cooking, cleaning, support with homework, film nights, recreational activities. Some creativity is encouraged to engage and inspire the young people.

 Alongside this you will have some responsibility for light household tasks to keep the house tidy and organised as well as undertaking health and safety checks. The time in the morning will be spent ensuring that the young people who need to be up and out are ready to attend college.

Some experience of working with young people, night working and lone working will be a big advantage.

Job Purpose

This role sits within our new Men’s Minds project, based in Hounslow. The aim of the service is to improve men’s mental health outcomes by developing a mental health network supported and directed by the voices of its members.

This will be achieved through three service strands: 1) Peer support network, 2) Psychoeducation workshops, addressing common needs of men’s mental health 3) Outreach support to raise awareness of men’s mental health and promote this service.

The Men’s Minds Project Coordinator will work closely with the members in shaping the service and ensuring voices of men in Hounslow are heard and acted upon. This service will be staffed with 1 part-time (25hrs per week) Project Coordinator and a pool of peer led volunteers. The Project Coordinator will be responsible for overseeing the volunteers.

The Role

The role of the Men’s Minds Project Coordinator is to arrange and coordinate a range of psychoeducation, and peer-led workshops which will be delivered weekly. With the addition of attending various community and stakeholder events to deliver the outreach strand of the service.

About the Service 
Our Nightlight Service is proud to provide urgent support, crisis intervention and emotional support to people experiencing a mental health crisis in Hertfordshire as an alternative to statutory pathways.

We provide support to adults across Hertfordshire 7 days a week, 365 days a year through our helpline, crisis centres, crisis cafes and a weekend Crisis House. We aim to provide a safe space for people in crisis to talk and feel supported, be listened to without judgement, access relevant support with problem solving and facilitate access to external services when necessary.

The objectives of Hertfordshire Mind Network’s (HMN) Nightlight Mental Health Crisis Services are:

• To improve the mental wellbeing of people experiencing mental health crisis in Hertfordshire.
• To increase early access to help for people experiencing mental health crisis in Hertfordshire, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To provide an alternative pathway to people in distress and crisis to support better outcomes and reduce the demand on statutory crisis services and emergency services.

About the Role 
Mental Healthcare Support Workers are key members of staff delivering the Nightlight Mental Health Crisis Service, taking part in a rota covering 7 days per week, ensuring the delivery of quality crisis support services and instilling Hertfordshire Mind Network’s values throughout the crisis cafe and helpline provision. Working closely with other staff (bank staff, volunteers), coordinators, team leaders and Crisis manager to ensure services are of the highest quality and support the continued growth and development of the service.

Key Responsibilities

• Providing emotional support and developing, alongside clients, flexible and realistic crisis support/person-centred plans within agreed guidelines with support from leadership team (including assessments of need, risk, crisis support plans and review forms), often where risks are significant.
• Liaising with statutory agencies to ensure appropriate intervention is obtained for individuals.
• Drawing upon own lived experience as appropriate to inform the delivery of person-centred support.
• Providing immediate advice and information to clients to support with crisis presentations.
• To support individuals both through telephone support (helpline) and face to face support (crisis café).
• Liaising with Nightlight management and external services e.g. police, ambulance service, Crisis Resolution Home Treatment Team, Single Point of Access, Safeguarding Out of Hours Service, Mental Health Liaison Teams, Adult Community Mental Health Services etc .as needed in terms of accepting referrals and proactively recognising the indicators of deteriorating mental health and facilitating appropriate support.
• Completing appropriate signposting/onward referrals to both internal and external services in accordance with identified needs and risks.
• Supporting clients with making drinks and beverages as needed when visiting the crisis café.
• Assisting with housing keeping/cleaning tasks as necessary and reporting any maintenance problems.
• Contributing to the co-development of the service by supporting service user, family and carer involvement by obtaining feedback and supporting where necessary chances to be involved in wider service reviews.

This role is an opportunity to work for Mind BLMK as part of the Milton Keynes NHS mental health service for someone with experience working in health, social care or information and advice, in direct contact with service users.

Working alongside colleagues from Central Northwest London Foundation Trust (CNWL) this role will provide referral management to and from Secondary Services. This will include signposting patients to statutory services including talking therapies and wider community based & voluntary sector support.

The Community Navigator will work as part of the team to deliver on the aims and objectives of the Mental Health Hub whilst representing the Voluntary and Community sector with the NHS.

Service Delivery

• Work within the NHS Mental Health SPA (Single Point of Access) Team to review and process referrals received from GP Practices and other sources including those to be subsequently triaged and assessed by mental health practitioners.
• Actively call patients with a view to assisting them through the process and making their onward appointments into community or secondary care service providers.
• Support the hub to signpost patients and direct them to appropriate services including secondary care, the local Voluntary and Community sector services and activities that support the patients’ wider determinates of their mental health and wellbeing.
• Answer calls, ensure sufficient phone cover, respond to messages/voicemails, provide administrative support to the Mental Health SPA for all services and deal with queries in a sensitive and timely way liaising with clinical or team lead as is appropriate.
• Monitor service inboxes during core office hours, responding to any queries professionally providing helpful information.
• Monitor and action tasks within SystmOne and maintain records of all activities carried out, i.e. outbound calls, calls received, archiving any received correspondence.
• Build good working relationships with the voluntary and community sector, relevant services and activities to ensure signposting pathways are kept up to date as well navigating and developing the setup of new signposting pathways through networking.
• Provide clinical and non-clinical information in appropriate chronological templates to other services.
• Carry out health and safety responsibilities as directed by Managers in line with Mind BLMK’s H&S policies, procedures, and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).
• To bring VCSE experience, knowledge and culture into an NHS team.

Role Requirements

 As part of the site services team the role holder will be expected to liaise with managers and staff across the Trust. The role holder is expected to work closely with the other members of the facilities team and assist in covering in the laundry, in the deep clean team or other works as and when required.

You will be a natural and enthusiastic housekeeper working not only as part of a team but also individually. You will be health and safety aware, take ownership of your own work, and be understanding of the children’s needs here at The Children’s Trust. Good organisational skills, flexibility and being friendly and caring in nature are a must for this role. Please note that the role requires a significant degree of manual handling and working with houses services and laundry equipment.

You will be working with children and families to provide trauma informed support with the aim of reducing trauma symptoms and rebuild relationships.

You will receive support and training and a competitive starting salary of £20,000 for a 28 hour week. Reduced hours may be possible by agreement. This is a two-year fixed term post with the possibility of becoming permanent after two years.

We are looking for someone who will work with professionalism, empathy and kindness, have knowledge of working with children and families who have experienced trauma due to abuse and have a trauma informed, child focused approach. Experience of working with complex family situations where there are safeguarding concerns with the ability to assess risk is required.

You will be based at our office in Redhill and you must be able to reliably commute to the office.
