Job description

For over 100 years The Creighton Centre has been working in Hammersmith & Fulham caring for older, vulnerable people and people living with disabilities. We tackle problems caused by social isolation and disadvantage by developing and providing programmes that offer people practical solutions and emotional support.

Join us to lead Keep Active, our programme helping older people regain strength, confidence, and independence after illness or a fall.

You’ll recruit, train, and support volunteers who work with clients referred by health professionals, providing 9-week goal-focused support to keep them active and connected. This hands-on role involves home visits, building strong relationships, and seeing the real difference you make in older people’s lives.

Key Responsibilities

  • Coordinate referrals and link clients with trained volunteers.
  • Support clients and volunteers throughout the 9-week programme.
  • Recruit, train and supervise volunteers, ensuring they feel valued and supported.
  • Work closely with NHS Community Independence and Rehab services.
  • Keep accurate records, gather feedback and share impact stories.
  • Promote inclusion and accessibility for all participants.

✨ What we’re looking for

  • Experience supporting or coordinating volunteers (desirable).
  • Understanding of the challenges older people may face living alone.
  • Great communicator – friendly, empathetic and organised.
  • Confident with IT and record-keeping.
  • Able to work independently and as part of a small team.
  • Willing to travel within Hammersmith & Fulham for home visits.

✨ Why join us?

  • Be part of a trusted, community-rooted charity.
  • Directly improve the wellbeing of older people.
  • Work in a friendly, supportive team.
  • Gain experience in health, volunteering and community development.
