Liverpool SHS is a mental health, 12 hour supported housing and community based service, working to a recovery model. We provide a range of support services for people with mental ill health to access in their local community, including outreach support, tenancy support, peer support and supported housing. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards.

We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in. Responsible for helping and inspiring the people who use our services to maintain their tenancy and independence in the community, we’ll rely on you to work with them to come up with a personal support plan that will see them achieve their goals and aspirations. No relevant experience is required as full training will be provided, although candidates with an understanding of mental health issues and/or lived experience would be welcomed. Thames Reach’s Supported Housing is comprised of Accommodation, Housing First, Navigator and Move on services supporting people to make real and lasting change in their lives. People may present with complex mental health, offending histories, drug and alcohol needs.

Housing First services: Using the established principles of Housing First we work with people to access accommodation first, explore their aspirations, building opportunities for them to spend their time meaningfully and  overcome barriers around unmet mental or physical health needs.

We are looking for a Support Worker to join the Greenwich Housing First team.

You will:

• Build and maintain positive, trusting relationships with individuals that have a history of rough sleeping or insecure housing.
• Support individuals to move into their own, independent accommodation, and to make it a home. You will work closely with landlords and housing providers to help individuals to understand and sustain their tenancies
• Work in partnership with key agencies, supporting individuals to access support services, community resources, and to feel a part of their local community
• Empowering individuals to take an active role in developing their own personalized support plans, and motivating them to achieve their goals and aspirations

Windsor Road service offers 11 beds to people experiencing mental ill health.  We are a CQC registered residential care home. We offer individual one to one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days.  We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. We also work closely with other agencies and our residents wider care teams, so good communication skills are required. And of course, all of this is recorded on our information management systems, so good record keeping and documentation is also essential. Our service consists of 8 24/7 Recovery Workers, a Service Manager and Registered Manager.

Responsible for helping and inspiring the people who use our services to achieve independence in the community, we’ll rely on you to work with them to come up with a person-centred support plan that will see them achieve their goals and aspirations, as part of your key worker role. Care home, mental health support or social care experience is preferred. You will be required to administer medication and complete a medication qualification and the care certificate. Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.

We are looking for an  Assistant Support Workers  to join the Street Outreach Rough Sleepers Team

Thames Reach welcomes applications from people with transferable skills.

You will:

• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Work alongside senior colleagues to identify the fastest and most appropriate route away from the streets for the people that we are working with 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Maintain and support relationships with partner agencies 
• Accurately record all work completed within given timeframes 

To succeed as the Assistant Support Worker, you will have:

• A strong commitment to providing a high standard of service 
• An understanding of homelessness which can be based on your professional work, voluntary experience or personal experience 
• Good written and verbal communication skills

About this role…

Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. Staffed 24 hours to ensure this quick turnaround, they are a key service to ensure people are off the streets and in stable accommodation.

We are looking for a Night Support Worker to join both the brand new Snow Hill Court team and the City Inn Assessment Service team.

We are looking for a Lead Worker  to join the Hillingdon Streat Outreach Team.

You will:

• Lead outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation. 
• Represent the team at multi-agency meetings 
• Accurately record all work completed within given timeframes 

To succeed as the Lead Worker, you will have:

• A strong commitment to providing a high standard of service 
• Experience working with people who have rough slept or vulnerable adults and supporting them to access services 
• Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently. 
• Good communication skills and confidence in public speaking when representing Thames Reach externally 
• Experience in forming strong working relationships with partner agencies, being able to challenge them when required 
• Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone 
• A clean driving license (desirable)

Lincolnshire Crisis Service have two houses in the county, one in Boston and another in Lincoln, we provide a range of support for people with mental ill health and both work to a recovery model. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards. This role does require you to work varied hours as the service is 24/7 and may include some sleep in shifts, these are paid in addition.

About this role…

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

Thames Reach welcomes applications from people with transferable skills.

You will:

• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping across four districts (Mole Valley district council, Tandridge district council, Epsom and Ewell Borough Council, Reigate and Banstead borough council).

• Support our clients with practical tasks that will enable them to move off of the streets or sustain accommodation. 

• Directly support clients, identifying activities that will assist in their journey towards recovery and away from life on the streets. 

• Maintain and support links with community programs and resources.

• Ensure that all paperwork is completed and work is recorded and monitored. 

To succeed as the Assistant Support Worker you will have:

• A strong commitment to providing a high standard of service. 

• Have an interest in homelessness and a desire to support us to have a positive impact on the lives of homeless people.

• Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently. 

• The ability to learn and an enthusiasm for training and acquiring new skills.

• Confident in building strong working relationships with partner agencies, being able to challenge them when required. 

• Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone.
