About this exciting opportunity
As Young Adults Caseworker, you will focus on promoting the project and engaging with your clients in line with the project’s targets and providing a client-led support service and advocacy that responds to each individual’s needs and achieves project outcomes. You will also deliver a holistic support service, working solo or with colleagues as the situation dictates, that will include practical help such as mentoring, social and housing support, accompanying to appointments, ETE options, benefits work, debt advice, liaising with utilities, DIY work and cleaning.
You will support clients to develop individual action plans and regularly review these plans with your clients to assess progress, plus produce risk management plans based on assessments. We will also count on you to develop and maintain relationships with partner agencies, including community-based services such as Police, YOT, Probation, children’s services and local authorities, as well as ensuring you close cases efficiently and positively, identifying a referral route for young people that can be used for ongoing support.
What we are looking for
- Experience in working as part of a multi-agency team, working together to achieve positive outcomes for young adults and establishing links to further the aims of a project.
- Experience of using support plans to enable people to successfully access support services.
- Proven record of engaging successfully with ‘challenging’ young adults.
- Experience of assessing risk and implementing safety procedures when thinking about young adults involved in or at risk of violence or exploitation.
- A working knowledge of relevant services for young adults in and around Gwent.
- Excellent interpersonal, relationship-building and communication skills, verbal and written.
- A flexible, collaborative and professional approach to your work.

About this key role
Working as part of a larger Trainer and Assessor team, our successful candidate will work with allocated learners to produce realistic assessment plans, assessing learners using a range of assessment methods appropriate for the vocational qualification assessment criteria, and provide vital one-to-one support to learners as appropriate, ensuring you monitor their progress towards timely completion and certification.

About the role
This is a maternity cover post and is 28 hours per week with Hybrid-variable working arrangements. You will work from home, the Derriford Hospital in Plymouth and Bristol Children’s Hospital. The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.
South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.
The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.
You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

With your social work experience with children and families, you will support birth relatives whose children have a plan for adoption or have been placed for adoption. Working closely with colleagues in Buckinghamshire Council who will refer the relatives to PACT, you will provide support through individual sessions, support groups and drop-in sessions.
This role will include supporting birth relatives to meet with adoptive parents and to help birth relatives understand the importance for children, to keep connected to their relatives and significant people from their past.
Ideally, you will have previous experience of working with birth relatives whose children have been placed for adoption and you will have a good understanding of the adoption process. You will work with relatives from a range of diverse backgrounds, lived experiences and those who may be neuro diverse and you will need to demonstrate understanding, respectfulness and adaptability to your social work practice.

This will form part of the new transformation of mental health services in the Royal Borough of Greenwich under the NHS Long Term Plan. The service will bring together the expertise of local primary, secondary and voluntary sector mental health care providers with the aim of providing flexible, holistic, and integrated services for adults with mental health problems.
Unpaid carers in our communities are supporting relatives, friends and neighbours, often with limited or no support, to live the best lives they can. The support of people with dementia, learning or physical disabilities, mental health difficulties or substance misuse problems by unpaid carers has been estimated to save the government close to the equivalent expenditure for the whole NHS.
We are looking for a Mental Health Carers Advisor who possesses an understanding of the challenges of unpaid caring and whose experience of working within mental health services includes support, prevention, recovery, and self-management. Applicants should possess effective skills in communication and have experience of completing assessments, managing a diverse caseload and working in a busy work environment. An ability to work independently but collaboratively, to be proactive and to show initiative is essential for this role.
The role will involve:
- Management of a diverse client caseload
- Providing recovery-orientated individual and group-based support to carers of adults with mental health problems
- Collaborative working with all partners involved in Greenwich Mental Health Hub work
- Undertaking holistic needs and risk assessments
