Are you passionate about supporting vulnerable adults during critical moments in their care journey? As a Hospital Care Navigator, you’ll be a vital part of ensuring patients leave the hospital safely and confidently, with the right support in place.
In this rewarding role, you’ll be the single point of contact for adult patients preparing for discharge from the hospital. You’ll work closely with the Adult Social Care team, health professionals, hospital practitioners, and community services to:
- Identify needs and barriers to safe discharge
- Achieve safe and timely patient discharge
- Coordinate outpatient appointments and follow-up services
- Provide clear, compassionate advice and support to patients and their families
- Ensure patients understand their discharge plan every step of the way.
You’ll meet with patients, their families, friends, or carers to assess ongoing needs, ensuring a safe plan is in place for their return home. This could include ensuring that medication is arranged from the hospital pharmacy, someone is at home to greet them, food and heating are available, and the condition of the home is safe to return to. You may be required to liaise with Adult Social Care and other home providers to coordinate home support. You’ll follow up on calls or visits to ensure the patient is managing well at home. By providing the proper support, patients can be discharged sooner, and the risk of readmission is significantly reduced.
We’re seeking someone who:
- Has experience working with vulnerable adults in social care, healthcare, or the community/voluntary sector
- Communicates with empathy and confidence across different teams at different levels, and with patients
- Is highly organised, with excellent time management skills
- Has awareness and respect for cultural differences
- (Bonus) Experience working in a hospital or clinical setting.
The role is based at our Southwark office, and you will be required to travel to hospitals and across the City of London, as well as to Tower Hamlets, Camden, Hackney, and Southwark.

About the Role
We’re looking for a motivated and organised Volunteering and Community Programme Intern to join our small, supportive Services Team. This role is central to coordinating and developing Ataxia UK’s volunteering network and community initiatives, helping strengthen the support available to people affected by ataxia.
You’ll play a key role in supporting volunteers, organising community events, and assisting with the delivery of services that help people affected by ataxia live the best possible quality of life. Overall Role Purpose
The Communications Specialist – South East helps people with sight loss to live the life they choose by driving the awareness and saliency of the Guide Dogs brand nationally and in their region by leading the development and delivery of central, regional and local integrated communications activity.
The role communicates the strategy and work of Guide Dogs, engaging key audiences both externally and internally (including service users, volunteers, staff, and supporters) to strengthen the brand and further awareness of the charity
Key Responsibilities
· Brand – Drive the awareness and saliency of the Guide Dogs brand nationally and in the region by bringing to life the brand positioning through all customer experience touchpoints.
· Champion the brand identity in the region, leading the implementation across all touchpoints. Lead the regional activation of all aspects of the brand campaign, working closely with the central brand, comms and digital teams.
· Communications & Digital – Lead the development, delivery and review of central, regional and local integrated communications & digital activity in the region to include print and broadcast media, social media, publications, advertising, events, direct marketing and internal communications as appropriate, ensuring objectives are set and measured. Create and edit key regional publications, as and when required.
· Case studies – Source and create local case studies, and work with Content Team to maintain the Chorus case study library with content to support local and national campaigns
· Media relations - Build strong and on-going relationships with national and regional journalists through proactive and reactive media relations, including dealing with issues and crises that are a risk to the organisation’s reputation.
· Work with all areas of the charity to story spot and develop compelling service user case studies that will engage journalists and turn into national news stories
· Generate region-based stories and articles for corporate publications

· Volunteer management - Develop and maintain a network of volunteer media champions across all regions.

We want a Welfare Benefits Caseworker to join our small but dynamic Advice team. Unfold is an established local charity, supporting young people and families in Westminster and Kensington and Chelsea to get where they want to be, identifying and achieving their goals through mentoring and peer support.
For over 35 years, we have been supporting people who face social injustice, from families seeking asylum and young people excluded from school to women rebuilding confidence after hardship. About the Role
We’re looking for a Volunteer & Communications Coordinator who believes in the power of people and storytelling.
This is a role for someone who’s passionate about community and inclusion, and who can bring both heart and strategy to the way we recruit, train, and celebrate our volunteers.
You’ll lead the development of our volunteer and communications strategy, ensuring our work is inclusive, rooted in social justice, and reflective of the communities we serve. From recruiting and training new mentors to telling powerful stories that shift narratives, this is a role where people skills meet purpose.
You’ll:
- Build meaningful partnerships with community organisations, universities, and employers to grow a diverse volunteer base.
- Oversee each step of the volunteer journey, recruitment, training, and recognition, ensuring every volunteer feels supported and valued.
- Shape Unfold’s digital presence and social media, amplifying Global Majority voices and sharing stories that inspire others to get involved.
- Work collaboratively across programmes to strengthen processes, maintain accurate data, and embed rights-based practice.
About You
You’ll bring:
- Excellent written and verbal communication skills and a confident, engaging style.
- Strong organisational ability - able to manage multiple priorities.
- Good data management skills and attention to detail.
- Understanding of the volunteer sector and enthusiasm for supporting families and young people to thrive.
- Experience in marketing or external communications, including managing social media and creating materials in Canva or similar tools.

Beacon is an online membership and activity management system for u3as. It provides a simple interface for managing members, groups and finances. Beacon currently provides over 650 individual u3as with over 300,000 members with a secure, efficient and effective means of managing their day-to-day business functions.
SiteWorks offers a website presence solution for u3as, Networks and Regions. A volunteer-led team provide WordPress websites on a managed platform. SiteWorks provides over 700 u3as with a website presence.
A central aspect of this role will be working closely with volunteers; the service manager will play a key role in overseeing and supporting the ongoing operation of both these systems. Historically theses services have largely been managed by volunteer teams, whose commitment and expertise have been vital to the success of both systems. Mind in Croydon is seeking to recruit a full time Independent Mental Health Advocate (IMHA) to join our small, dedicated team of Independent Mental Health Advocates working to provide advocacy services to adults subjected to detention under the Mental Health Act in the London Borough of Croydon to ensure they understand, express and exercise their rights, and to enable them to access essential services. The advocacy team also provides Community Mental Health Advocacy for people who are informal patients in psychiatric hospital or living in the community and provides weekly advocacy service at two InMind Healthcare Group hospitals in Croydon.
This role is suitable for a fully qualified Independent Mental Health Advocate (IMHA) with a proven ability to support people under the Mental Health Act. We are not considering trainee positions at this time.

We are seeking a part-time Money Skills Coordinator to help us deliver our ambitious plans to equip more individuals with money skills. The primary purpose of this role is to manage Crosslight’s money skills courses that are attended by the local community as well as our debt advice clients. These are delivered online or in-person in a range of different locations across our network.
The role holder will take ownership of the planning and running of our money skills workshops, including setting them up on our systems. They will also act as a ‘champion’ for the courses, promoting them internally and externally, including through community engagement and outreach in different locations. And they will have the opportunity to deliver some of our workshops, as well as supporting and equipping our volunteers to do so to a high standard.
For the right candidate, this role is an exciting opportunity to play a part in supporting a wide range of people, through excellent organisational, interpersonal, technical and presentation skills.
