About the role

This position involves leading and contributing to identified nutrition focused cancer and nutrition projects within Macmillan engaging with internal and external stakeholders.  There will be a particular focus on the following areas: 

• Review and development of Macmillan nutrition resources from an equality, diversity and inclusion perspective, working on focused pieces of work with communities to co-produce nutrition information through focus groups/attending relevant community events and discussing the importance of nutrition. 
• Leading a nutrition focused partnership project(s) in identified areas with poorer quality health outcomes.
• Development of scoping work to understand the opportunities for creating a single point of access for nutrition and cancer resources. 

About you
The successful candidate will demonstrate the following skills and experience:

• track record of management and clinical leadership
• ability to bring insight, ideas, experience and thought leadership to the role to inform nutrition information for different groups and communities
• strong interest in community engagement and empowering people with cancer along with supporting those with poorer outcomes
• good working knowledge of the current healthcare and social care system and current issues involved in supporting and engaging communities as well as contemporary knowledge of nutrition and dietetics.

The Mulberry Centre’s vision is to be known and respected by everyone living or working within reach of our services who may become affected by cancer.All the services at The Mulberry Centre are geared towards supporting the physical and emotional needs of someone following a cancer diagnosis or at end of life, including their carers, families and friends who also might have to stop working to look after them. It can be difficult to know where to start and what you are entitled to, but we are here to help them every step of the way. To assist us in this endeavour, we are looking for an experienced full-time Welfare and Benefits Information Lead.  You will assist clients in claiming welfare benefits, grants, and any financial aid that may be available to them. This includes informing them of any financial help available, supporting them with applications and following up on claim as necessary.  Where anyone has been given an end of life diagnosis you will be required to start the application on their behalf. In addition, you will also conduct financial wellbeing assessments in the community and schedule one-on-one application completion consultations with individuals who request your assistance.

This vital role will suit an enthusiastic and dynamic person who has excellent communication skills, being able to deal with complex matters effectively both verbally and in writing.  You will need to understand the issues involved in interviewing people within a time defined period. The ideal applicant will have emotional resilience to deal with people who have experienced cancer or bereavement. An empathetic and compassionate approach is important.  The Welfare Benefits Information Lead will be at the forefront of our client-based services and need to be self-motivated and have a positive and flexible attitude.

The Hepatitis C Trust develops projects nationally where peers use their lived experience of injecting drug use and hepatitis C, to provide education and training, increase hepatitis C awareness, and access to testing and treatment for people living with hepatitis C. 

Working with the South West Peninsula ODN, The Hepatitis C Trust are now expanding this commitment through the introduction of the HCC (Hepatocellular Carcinoma) surveillance and early liver disease detection programme.

Are you looking for a new and exciting challenge? Do you have experience of working within health services and with volunteers? Have you been affected by liver disease or supported those at higher risk of developing liver disease? 

We are looking for a self-motivated individual who can work on their own, engage with stakeholders and have a desire to make a difference in promoting liver screening in community settings, and by increasing access to specialist care services.

Working in partnership with local hospital and community outreach clinics, the post holder will ensure that patients are supported through initial liver assessment and ongoing surveillance as required. The post will coordinate delivery of educational and awareness workshops and promote the delivery of liver screening in community settings 

Working under the guidance of the South West Peninsula Peer Programme Manager, the post-holder will work alongside other identified partners to help develop a community outreach liver surveillance program and carry out HCC early detection activity and interventions across Plymouth.

We require a good standard of education, willingness to travel and welcome creativity and innovation in all our work. We are a patient-led organisation – you will be working in an environment where the patient/service user/client is placed at the centre of all that you do. This post will require extensive travel across the region and the post holder is required to hold a clean driving licence and have their own vehicle for work related travel. 

Through the partnership of Macmillan Cancer Support, Self Help UK and the Nottinghamshire Integrated Care System, the Macmillan Beyond Dagnosis service delivers support for people affected by cancer, as well as facilitated access to voluntary and community sector cancer specific support and wellbeing interventions across Nottingham and Nottinghamshire. Support and interventions will be identified using electronic health needs assessments (eHNA’s) and personalised care and support planning. This will provide a structured and clear ongoing support plan for those living with and beyond cancer which are integrated with primary, secondary and community clinical care pathways.

The aim of the service is to provide an improved experience for patients beyond their interactions with clinical services by contributing to the overall holistic approach to supporting people living with and beyond cancer. This will integrate the ‘beyond medicine’ approaches offered by the Voluntary and Community Sector (VCS) into existing and developing cancer pathways.

The Macmillan Beyond Diagnosis Service also includes the NUH Cancer Prehabilitation service, where we provide the Beyond Diagnosis model through the Social Prescriber Link Worker role.

This post is an exciting opportunity to work with Nottingham City’s PCN 8 (Clifton and Meadows).. Using the Macmillan Electronic Holistic Needs Assessment to support Cancer Care Reviews will ensure patients have access to the right support for all their non-clinical needs. The post will also work on the preventative agenda, supporting uptake of cancer screening across the PCNs and supporting the practices within the PCNs to deliver their improvement plan around early diagnosis of cancer. (in line with NHS England and NHS Improvement -  QOF Quality Improvement domain 20/21 – Early diagnosis of cancer).

 You will work in partnership with GP surgeries to give non-medical support that maximises patients’ wellbeing through their Cancer journey. The primary role is to do Cancer Care Reviews, with access to each surgery’s IT systems to track patient lists, document, and communicate with practice staff. This role will involve liaising with services from within the NHS structure as well as external services, so you will need to be skilled in navigating intricate processes and organisational structures from both the inside and the outside. You will also help with improving screening uptake and community engagement, building relationships with local services and groups to further enhance the support.
