About the role:
No one should leave hospital with nowhere safe to go.
As Homeless Pathway Coordinator, you’ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You’ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision.
Day to day, you’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.
You’ll work closely with the Homeless Pathway Lead, hospital teams, Newham Council and SHP’s wider Health Team, bringing professionals together and helping everyone stay focused on the person behind the case. You’ll also build links with community services, identify gaps in existing pathways and share learning that helps the hospital respond more effectively to people experiencing homelessness.
This is a role where strong relationships can change the course of someone’s recovery. You’ll join an experienced and supportive team, with opportunities to deepen your knowledge of inclusion health, homelessness and psychologically informed practice through training, reflective practice and shared learning.
About you:
About us:
We’re London’s leading homelessness charity – and we get things done.
In a city where hundreds are forced into homelessness every day, our work has never been more needed or more challenging. And we’re not shying away. We’re rolling up our sleeves to make change and helping over 10,000 Londoners every year. We prevent homelessness, provide safe places to live and give people the opportunity to rebuild their lives and transform their futures. And we never give up.
We’re here for Londoners wherever they are on their journey. We start with trust, building relationships that help people feel safe, supported, and ready to move forward. Every day, we put people first in everything we do, challenging injustice and barriers that keep people from the safety, stability and opportunity they deserve. We stand alongside people as they rebuild and shape a future that feels their own.
Joining Single Homeless Project means joining a team that’s bold, compassionate and determined to do better for the people we support and for each other. You’ll work alongside colleagues with lived experience, in a space that’s trans-inclusive, disability-friendly, and actively striving to be anti-oppressive and equitable.
We’re not perfect, but we’re real. We listen. We learn. And we push forward, together. Because this isn’t just a job. It’s a chance to lead with empathy, spark change, and help build a London where no one is left behind.

Salary: Starting salary £32,604 rising to £36,796, plus London weighting of £4,000
Hours: Full time (32 hours per week)
Contract type: This is a fixed term role for 12 months

High-quality data will enable us to develop stronger relationships with our supporters, retention and maximise lifetime value working in an agile way.
What does the role involve?
Why is this position on a fixed term contract?
Having this CRM Officer role in post for a year will support the ongoing business as usual data processes, whilst increasing capacity in the team to support efforts to increase the amount of automation of processes and integration between systems. Safeguarding is Everyone’s business – Mental Health Foundation is committed to safeguarding and promoting the wellbeing of all its beneficiaries, those who surround them, its staff, volunteers, and anyone else who comes into contact with its services and expects all trustees, staff, and volunteers to share this commitment. The successful applicant will be subject to appropriate vetting procedures (proof of eligibility to work in the UK, proof of residency and satisfactory employment screening, including a Disclosure check and two most recent references) along with 3-year renewals of Disclosure checks. We are unable to provide sponsorship for this post, you must be able to demonstrate your eligibility to work in the UK.
How to apply
If you think your skills match and you’d like to be part of a dynamic and growing organisation, please complete and submit your application below. Please ensure you attach an up-to-date CV and statement of suitability answering all points of the person specification. Applications will close at 9pm on Wednesday 2nd September and we are unable to accept late applications. Interviews are planned for w/c 14th September.
We believe our people should represent the communities, organisations and individuals we work with. Diversity and inclusion is a strategic priority for us as an employer and mental health charity, and we are proud to be signatories of the Business in the Community Race at Work Charter and the Disability Confident Committed Scheme. Applications from under-represented sections of the community are actively encouraged.
If you have a disability, require any additional support or have any questions regarding the role, please contact us. We make reasonable adjustments throughout the recruitment process and during employment. Disabled candidates who meet all the essential person specification criteria will be offered an interview. Therefore, please do ensure you tick the relevant box on the application form and clearly indicate in your application/covering letter if you consider yourself to meet the requirements of the Equality Act 2010 / Disability Discrimination Act 1995.
We are currently operating mostly digital recruitment (including interviews via video conferencing). We operate a hybrid working model of a minimum of 2 days per week in the office and the rest working from home.
We look forward to hearing from you!
