Purpose of the role: 

The Kinship Navigator provides intensive, time-limited support to kinship carers through the Kinship Connected programme, a structured six-month intervention designed to help kinship families stabilise placements and access the support they need.

Working directly with kinship carers in West Dunbartonshire, you will build trusting relationships while completing structured assessments, goal setting and reviews to help families strengthen their support networks and navigate services such as children’s social care, education, health and community support.

This is a community-facing role, working from Y Sort It’s offices and directly in kinship carers’ homes and community spaces. You will work alongside Y Sort It’s Intandem mentors to ensure that both carers and the children they care for receive coordinated, complementary support.

Accurate recording of your work is essential to enable Kinship and Inspiring Scotland to understand what is working, demonstrate impact and learn from this pioneering pilot.

Direct support

You will provide intensive one-to-one support to kinship carers for up to six months, working within the delivery approach set out in the Kinship Connected Service Manual. You will also establish and facilitate a monthly support group for kinship carers in West Dunbartonshire.

This may include:

• Providing emotional and practical support to kinship carers.
• Completing structured needs assessments, SMART goal setting and regular reviews with kinship carers.
• Advocating for kinship carers in meetings with professionals where appropriate.
• Establishing and facilitating a monthly support group for kinship carers in the local area.
• Mapping local services and building relationships with organisations that can provide specialist support, training or activities for kinship families in West Dunbartonshire.
• Liaising with schools, local authorities and other professionals to coordinate support around families.
• Supporting kinship carers with challenges relating to the child(ren) in their kinship care.
• Signposting to relevant services, support organisations and Kinship training opportunities.
• Referring children in kinship care to Y Sort It’s Intandem mentoring programme where appropriate.
• Coordinating celebration and family events (including in Kinship Care Week).
• Supporting applications for grants for essential items or family breaks.

Partnership working with Y Sort It and Inspiring Scotland

• Work closely and collaboratively with Y Sort It colleagues on a day-to-day basis, building a strong co-location relationship that benefits kinship families.
• Develop and maintain clear cross-referral pathways between Kinship Connected and Y Sort It’s Intandem mentoring programme.
• Contribute to joint learning and shared reflection about how the whole-family model is working and what outcomes are being achieved for both carers and children.
• Represent Kinship professionally in all interactions with Y Sort It, Inspiring Scotland and other local partners.
• Participate in any joint reporting, learning or evaluation activities required by Inspiring Scotland.

Collaboration and wider partnership working

• Build constructive relationships with West Dunbartonshire Council and other statutory and voluntary sector partners to support awareness of the programme and effective referral pathways.
• Work closely with colleagues across Kinship, including Peer Support, Training and Communications, to ensure kinship carers in West Dunbartonshire can access the full range of Kinship’s support.
• Contribute to cross-team learning, sharing emerging insights about kinship carers’ experiences and the local landscape in Scotland to inform Kinship’s broader organisational learning.
• Represent Kinship at local meetings, networks or events to strengthen collaboration and raise awareness of kinship care in Scotland.

Safeguarding and risk management

Kinship has a robust safeguarding structure. You will be supported by a Designated Safeguarding Lead (DSL) and Designated Deputy Safeguarding Leads (DDSL). In line with this policy, and mindful of the Scottish legislative and practice context, you will:

• Recognise, report, record, respond and refer safeguarding risks via our safeguarding process with the support of the safeguarding team.
• Follow and understand Kinship’s organisational safeguarding policies, as well as relevant Scottish guidance and legislation.
• Complete risk assessments for events in line with Kinship’s policy.
• Follow the Kinship Health and Safety policy.
• Use the StaySafe lone worker app.

Monitoring, evaluation and data quality

Accurate and consistent data recording is a core requirement of this role. As Kinship Connected in West Dunbartonshire is a pilot programme, high-quality records and documentation are essential to demonstrate impact to Inspiring Scotland and to generate the learning that will inform future development of Kinship’s work in Scotland.

• Maintain accurate, timely records of all activity, assessments, support plans, contacts and outcomes on Kinship’s CRM system (Salesforce) in line with organisational policy and programme protocols.
• Complete kinship carer needs assessments, SMART goal setting, reviews and outcome recording in accordance with the Kinship Navigator model.
• Contribute to monitoring and reporting requirements for Inspiring Scotland, ensuring activity and outcomes are documented consistently.

About the role

Jasmine Mother and Child Family Residential Centre is Trevi’s specialist residential service supporting women during pregnancy and mothers living with their children while addressing substance use, trauma, mental health needs and wider family pressures.

As Registered Manager, you will lead the day-to-day delivery of this seven-bed residential family service, ensuring a safe, nurturing and trauma-responsive environment for women, children and staff.

Working alongside the Head of Service, you will lead and develop a multidisciplinary team, maintain high standards of care and ensure the service meets Ofsted requirements and all relevant regulatory and safeguarding standards.

Role Context

Recovery in Community is a successful partnership between Manchester Mind and Gaddum. We support people to manage and live with their mental health in their community.  Staff are employed by both organisations but work together collaboratively and as a team.

Role Purpose

You will be building trusting and kind relationships with the understanding that they enable people to be listened to and heard and that this makes a difference.

You will work with people prior to discharge from mental health hospital and then support the person get the care they need so they can recover in the community. We want to enable people to feel connected within their communities – to services, friends and family.

This role will work with a small but intensive caseload (max 15 cases) to focus on:

· Get people home: when people are sent to an out of area placement (OAPs) advocate to get them back to Greater Manchester so they can recover in their community.

· Discharge: Supporting people when they’re ready to be discharged from inpatient mental health services to get back to the community and recover.

· Prevent re-admissions: Work with people, post-discharge, to ensure they continue to get the support they need to make informed choices, stay well and ultimately thrive.

Main responsibilities:

· To provide support to people that are ready to be discharged from inpatient psychiatric care to return to the community with the services and resources they need to recover.

· To work alongside people post-discharge, in their community setting to ensure they get what they need to stay well.

Role Context

Recovery in Community is a successful partnership between Manchester Mind and Gaddum. We support people to manage and live with their mental health in their community.  Staff are employed by both organisations but work together collaboratively and as a team.

As the Recovery in Community Worker (Referral and Assessment) you will be supporting people in who have been referred into Greater Manchester Mental Health (GMMH) services but may not meet the threshold for secondary or specialist mental health services. The aim of this work is to ensure that people referred receive the right support at the right time.

You will work closely with the Referral and Assessment team at Greater Manchester Mental Health Trust (GMMH) to take referrals and ensure people get access to the mental health and social support they need.

Role Purpose

You will be building trusting and kind relationships with the understanding that they enable people to be listened to and heard and that this makes a difference. You will be part of a team providing effective person-centred support to people who come through the referral and assessment hub – many of whom often have tried many times to get the help they need.  We want to enable people to feel connected within their communities – to services, friends and family.

You will also be building relationships with NHS and statutory services. The structure of the service is brief intervention (working with people for a short period of time) with a focus on supporting people to connect to community services and resources effectively.

You will be well supported in this role. The service takes a team-based approach, and you will be joining a team of five and you will be supported directly by a Team Coordinator as well as being part of the wider Recovery in Community Service.

Main Duties and Responsibilities

· Take referrals from the GMMH (Greater Manchester Mental Health) Referral and Assessment Hub and build trusting relationships with people so that they receive the support they need to recover and stay well.

· Build relationships with other key professionals to ensure individuals achieve their goals (e.g. GPs, social care, community services etc).

· Support the development of a supportive and productive referral route into and out of the service, managing exit from the service safely.

· Support the development of this new service, responding professionally to changes as we improve and evolve our working practice collaboratively.

· Provide a trauma-informed service to people: listening, offering safety, choice and empowerment to the people you support so they can effectively engage with services and make sustainable links in with their communities.

· Identify and deliver appropriate interventions.

· Identify when people need referrals to secondary services or specialist mental health services and make appropriate referrals.

· Independently travel to visit people in a range of settings across in Manchester and across Greater Manchester, adhering to lone working and risk management procedures.

· To integrate effectively with colleagues at Manchester Mind, taking a “one team approach” to deliver a seamless service, understanding the different roles in the Recovery in Community team and recognising each other's strengths.

· Maintain accurate and up-to-date case notes and records using agreed case management systems, following standard operating procedures.

· Support people to engage in our service through coproduction, engagement events and feedback forms.

· Contribute to service reporting, monitoring and evaluation as required.

· Identify additional needs that people have and highlight them to Manchester Mind with a view to sharing them with Greater Manchester Mental Health Trust and commissioners.

All staff are expected to work within all Gaddum policies and procedures. This role is subject to an enhanced DBS check.

Family Support & Mediation Worker

We are seeking a Family Support & Mediation Worker to help young people and families experiencing conflict rebuild relationships and prevent homelessness.

Position: Family Support & Mediation Worker
Salary: £27,943 FTE, £11,177.20 actual salary
Location: North Tyneside, with travel across the North East
Hours: Part-time, 15 hours per week
Contract: Permanent

About the Role

This is a rewarding opportunity to work directly with young people aged 11 to 25 and their families, providing practical support and mediation during periods of conflict, crisis or relationship breakdown.

You will manage a small caseload, helping families improve communication, resolve difficulties and build stronger relationships. The aim is to intervene early, reduce family breakdown and help prevent young people from becoming homeless.

Working across the North East, you will build strong relationships with young people, families, schools and other professionals.

Key responsibilities include:

· Providing family support and mediation to young people and their families

· Managing a small caseload and developing individual support plans

· Completing risk and needs assessments

· Helping families improve communication and work through conflict

· Supporting young people to remain safely within their family or support network wherever appropriate

· Working closely with schools, local authorities, homelessness services and other agencies

· Developing referral pathways and building relationships with partner organisations

· Responding appropriately to safeguarding concerns

· Maintaining accurate case records and monitoring outcomes

This is a mobile role requiring independent travel across the North East.

About You

You will have experience supporting young people, families or individuals experiencing challenging circumstances and will be confident building relationships with people who may be experiencing conflict or crisis.

You will also have:

· Experience managing a caseload or providing one-to-one support

· Strong communication and relationship-building skills

· Experience of risk assessment, support planning or safeguarding

· The ability to remain impartial and calm in difficult situations

· Confidence working independently and with external agencies

· Good organisation and case-recording skills

· A strengths-based, solution-focused approach

You do not need to already hold a mediation qualification. Full training will be provided, including support to complete the Level 4 Interpersonal Mediation Practitioner's Certificate during your probation period.

Experience of family mediation, youth work, family support or homelessness prevention would be particularly welcome.

This is an essential car user post. The applicant must hold a current UK driver’s licence, with no more than 6 points, have access to a car and be able and willing to drive UK wide as required.

This senior role requires flexibility in working hours and location.  It is primarily offered on a hybrid basis with a minimum of 2 days in our Amersham, Buckinghamshire, office each week. We’re looking for a compassionate and motivated Community Connector to join our GP Integrated Mental Health Service and Mental Health Integrated Community Service in Surrey Heath.

This is an exciting opportunity to work at the heart of local communities, supporting people with their mental health and wellbeing by helping them access the right support at the right time. Working alongside GPs, mental health practitioners, partner agencies and community organisations, you will provide timely assessments, brief psychosocial interventions and practical support that helps people build on their strengths, overcome barriers and connect with services, activities and networks that promote independence and recovery.

Key responsibilities:

To work jointly with the Mental Health Practitioner in conducting assessments, including risk assessment, and to work with patients to support them in identifying their socially determined needs and goals, provide self-management tools and facilitate the development of personal support plans

• To provide a range of motivational and structured psychosocial interventions
• To promote independence through an enabling asset-based approach that draws on individuals’ strengths, preferences and ‘natural’ support networks
• To establish effective working relationships with a range of agencies to facilitate a ‘joined up’ approach to support plans
• To liaise with, develop and maintain good relations with GPs, practice managers and other health practitioners across mental health and physical health pathways and wider support networks
• To provide continuity throughout recovery; engaging the patient with key services such as Safe Havens, accessing CMHRS, and providing support for those engaged with and leaving CMHRS, and linking into local wellbeing services and activities
• To co-ordinate and support patients to access a range of community services such as wellbeing services, housing providers, family and carer support services, debt and employer advisors where appropriate
• To assist with community resource mapping exercises and maintain a database of community resources; map where there are gaps in provision across the PCN and wider geography and work with other VCS organisations to develop resources where most needed
• To promote and support (where necessary) advised follow up actions from physical health checks for people with serious mental illness
• To support patients to engage with local peer support workers and volunteering services
• To support and supervise any peer support workers linked to PCN
• To work with the service to identify opportunities to expand provision particularly in local communities where there is a lack of service provision identified
• To broker and establish new partnerships between public and voluntary sector agencies to enhance service delivery and access to services
• To develop and sustain professional relationships with service users, partner agencies and appropriate external agencies
• To complete locally agreed quality outcome measures, with patients and to undertake regular reviews
