Hours: 36 hours per week (full time) worked within a 7-day rota (7days a week/365 days a year) The current service operates a daytime service, 9.00am – 6.00pm and evening service 3.00pm – 11.00pm

(This will include weekend work, approximately one in three weekends)

Location: Main place of work will be Fairfield House, 10 Altyre Road, East Croydon.  Some of the team will be based at Croydon Health and Wellbeing Space at the Whitgift Centre, Croydon (and other locations that may be used in the future) and the Outreach Worker will be predominantly working from the Community Hubs around the Borough.

Mind in Croydon is a charity concerned with the welfare of people with mental health problems living in the London Borough of Croydon and the surrounding areas. Mind in Croydon delivers Mental Health Transformation Program services alongside Croydon BME Forum, these include Croydon Health and Wellbeing Space and Mental Health Personal Independence Coordinators. Other services include Recovery Space, Advocacy, Active Minds, Carers Support, Peer Support In-Reach, Social Networking, Counselling, and an Information Service.

This exciting role will manage the provision of a non-clinical Recovery Space (crisis café) funded by South London Partnership Commissioning. This project operates in the day and evenings, people are referred by our Trusted Assessors in various Teams such as Croydon University Hospital, Psychiatric Liaison, Assessment and Liaison, Croydon College, Home Treatment Team and others.

The aim of the service is to divert people, who would be better supported elsewhere, away from Accident and Emergency (A&E) and towards services that can support with their immediate crisis within the Borough of Croydon. The Recovery Space Service Manager leads this service which is non-clinical but a warm and welcoming setting to support and hold individuals during the day and evening. The Recovery Space service referrals will not be accepted after 10pm for attendance at night.

The role will work closely at senior operational level with external partners, clinical, commissioning, and voluntary sector organisations to plan, develop and promote the highest quality service. They will manage a team of nine staff, three are Crisis Workers, operating a day service, one outreach crisis worker also operating during the day and five Recovery Space Workers in the evening.  The role will need to produce reports, develop, and implement policies and procedures in line with best practice and to ensure the service is making a difference to people’s lives.  The role will work with other service managers including those managers within the Transformation services to ensure a One Team approach to provide an effective and efficient service for clients.

Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.

Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

This role consists of consists of day time working patterns including early shifts from 8am and late shifts from 2pm. Shifts are 8 hours long and 5 days a week.

This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.

The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

IBIS House MH Step Down & Crisis

Working in partnership with the London Borough of Newham and East London Foundation Trust, IBIS House supports people who are medically fit for discharge from mental health inpatient stays but have other barriers to discharge. These may include housing, living skills, accessing benefits or the community, or other interventions and will be on a person-centred basis. The main aim of the service is to provide short-term support to customers to link with longer-term services to reduce escalations in need, and reduce inpatient readmission and reduce pressure on inpatient beds.

Shifts will be on a rota pattern and include morning shifts (08:00-16:00) and afternoon shifts (14:00-22:00).

Includes evenings, weekends and bank holidays.

About the Service

The Refuge Service provide emergency accommodation to women and children fleeing domestic abuse and sexual violence. Refuge Case workers provide emotional and practical support this can include housing support, legal options, reporting to the police, help around child contact, benefits, financial advice and accessing therapeutic support services.

About the Role

You will provide advice and support to women who have experienced domestic abuse in a refuge setting working across two sites.  You will carry out risk and needs assessments, develop support plans and carry out key work with women survivors. You should have a minimum of two years’ experience of working with this or a similar client group. An in depth understanding of the needs of women and children from a diverse background is essential.

About You

The ideal candidate will have in-depth knowledge of domestic abuse and its impact on women and their children, as well as experience of providing emotional and practical support to victims of domestic and sexual violence.

You will have experience of managing risk and following case management procedures to meet the needs of the client group, and the ability to work with women to help them to understand their needs, risk assess, develop safety plan, and support them in linking with other agencies.  

You will have a working knowledge of the available legal remedies, housing, and welfare benefits, as well as experience of providing workshops.

In addition to case management, you will be required to undertake housing management responsibilities including health and safety and maintenance of the refuges.  You will have excellent administrative and time management skills.

Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across london on a spot purchase basis.

Specialist Support workers will manage a caseload of customers to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

Consists of working 5 days a week across all days, 8 hours shifts starting at either 8am or 2pm.

Playing a vital role within our expanding youth support team at Future Skills, you will work to support and empower our young people to re-engage and return to education and employment. Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. Teresa House is a fast stream service where customers are supported up to two years while Hamlets Way is a slow stream service where customers can expect support for a minimum of two to a maximum of 4 years.
The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. As a support worker you will play an integral role in developing effective support and risk management plans. This role supports the delivery of METRO’s groups for LGBTQ+ young people, alongside a Lead Youth Worker. The successful applicant will work with the Youth Group Lead to deliver sessions, promote the group and complete administrative tasks associated with the groups. The role sits in METROs vibrant Youth Domain Team, where we are all motivated by creating safer spaces for all LGBTQ+ and questioning young people to thrive.

Programme

METRO youth groups provide safe, social spaces for LGBTQ+ young people to meet and receive support from youth workers and their peers.

The groups provide a structured programme of activities which is co-designed with young people. The programme addresses key areas such as mental health and wellbeing, sexual health, drugs and alcohol awareness, as well as rights for young LGBTQ+ people. The programme supports with employment, study, life skills, and healthy living.

The groups also provide opportunities to connect and participate in cross-group events such as half-term events, Prides, Youth Summits, and other opportunities. To support this staff is required to occasionally change their working pattern to other evenings and weekend work. Employees are offered sessional work as and when there is a need in the domain.

Role

This role will primarily be posted at METRO’s weekly “Snap” LGBTQ+ group in Bromley for 16–24-year-olds. The group is delivered in person. The role will occasionally support cross-group events and outings as per agreement. METRO Youth workers are often offered extra sessional work to provide cover in other locations across south London. Staff travel expenses outside of regular place of work is covered by METRO.
