Set up in 2014 by the Ambitious about Autism Schools Trust, the school has continued to grow and now has a vibrant learning community of nearly 100 pupils.

We have an exciting new role as a Senior PBS (Positive Behaviour Support) Practitioner within the Integrated Services team that comprises of a dynamic team of Occupational Therapists, PBS Practitioners and Speech and Language Therapists, working in our education settings.

As a Senior PBS Practitioner, you will be passionate about improving the quality of life of autistic children and young people working with the transdisciplinary team to improve outcomes for children and young people. You will achieve this through providing high quality PBS advice and support across the tiers of support (universal, targeted and specialist). You'll lead on PBS essentials training across the whole school, whilst line managing 2 PBS Practitioners. You will conduct risk assessments of challenging behaviour and devise and implement PBS plans, providing coaching and support to class teams to optimise fidelity.

• Has or is working towards a level 5 PBS qualification (or relevant alternative)
• Is skilful in the use of functional assessment and intervention
• Manages a complex caseload an prioritises and delegates effectively
• Has advanced skills in building and maintaining positive relationships with children and young people, professionals and parents/carers.

About the role

As an Engagement and Assessment Worker, you will be the first point of contact for someone who comes to Crisis for help to prevent or end their homelessness. You will:

• welcome them to the service on site and at partner agencies,
• listen carefully and non-judgementally to their story,
• complete sensitively thorough Crisis and Mainstay assessments,
• Agree realistic options and the support needed to assist them to prevent their homelessness or a pathway out of homelessness
• Help them formulate a holistic plan and help them take the initial steps, where necessary.
• Hold a caseload of members who require short term intervention to end their homelessness.

About you

To be successful in this role you will have:

• Experience of working successfully with disadvantaged/or socially excluded groups and individuals, utilising a psychologically informed approach.
• Ability to deliver excellent customer service, ensuring accessible, welcoming and safe ‘front of house’ environment in a busy service.
• Experience of assessing presenting needs, and an understanding of strengths-based assessments. Empathy and understanding of the barriers to accessing services.
• Ability to manage successful relationships with external agencies and relevant service providers and successfully advocate on members behalf.
• Excellent verbal and written communication skills used to ensure accurate record keeping on a variety of Case Management Systems/Databases.
• Excellent organisation and prioritisation skills, with the ability to successfully manage a busy and diverse workload.
• The ability to use your own initiative to contribute to the effective running of the service and to work flexibly as part of a team.

Cranstoun is a social justice and harm reduction charity empowering people to empower change. We offer a wide range of services across England within drug and alcohol, domestic abuse, children and young people’s services, criminal justice and housing.

An exciting opportunity has arisen to join the children and young people’s team for passionate and enthusiastic individuals to be part of shaping and delivering a new early support youth hub pilot within Merry Hill Shopping Centre, Dudley. The hub will support people aged 13+ and those who feel they still require a young people’s approach up to 25. It brings together the specialisms of Cranstoun, complimented by wider partnership delivery and lead areas of responsibility.

We have three positions, each to have a lead theme area, including relationships, youth engagement and emotional health and wellbeing. You will be the first point of contact within the youth hub, providing initial information, advice and guidance and facilitating connections to other support or specialist services. Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. Teresa House is a fast stream service where customers are supported up to two years while Hamlets Way is a slow stream service where customers can expect support for a minimum of two to a maximum of 4 years.
The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. As a support worker you will play an integral role in developing effective support and risk management plans. You will also support customers with aspects of their day-to-day living including but not excluded to; social and physical activities, medication, benefit entitlements, maintaining living standards, exploring employment, education and training opportunities.

• Training young people in living skills such as washing, cleaning, cooking and safety, food shopping and budgeting; proactively and intentionally in line with the Baca independent living preparation plan.

This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth. 

Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project. 

The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.

They will preferably have experience of working with both adults, children and young people. Carers’ Hub plays a vital role in Lambeth, supporting the many unpaid carers across the borough. Our work stems from the fact that carers often go about their roles despite the toll it might take on their own wellbeing, often having to make sacrifices that hamper their own ability to lead a normal life. The Role

The purpose of the Roma Outreach Worker role is to deliver an effective service to single people who are currently rough sleeping. This post will actively support Roma clients, providing a continued and consistent point of contact whilst assisting them in navigating the various change pathways available to them.

- Able to speak Romani or Romanian fluently or to a high level.
- Full and clean UK driving licence.

- Knowledge of GDPR and data management.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.

As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.

You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.

There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

Work proactively with Slough Hostel team to handle the service caseload and support an effective team approach to meeting each customer's identified needs.
Motivate the team and champion a positive local culture within the service.

This includes working flexibly and being proactively in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns.
Deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. This may include practical tasks, such a facilitating group actives
Support customers to undertake domestic tasks, including practical assistance to ensure a high quality living environment that promotes customers' dignity, respect and allows customers to feel safe and welcome in their own home.

Work with customers to promote social inclusion to promote independent living, encouraging customers to maximise their skills and choices.
Involve customers in the design, development and delivery of the service through co-production.

Proactive development of links with local statutory and voluntary organizations to provide a range of engagement opportunities for customers in the community.
Competent use of relevant systems, company policies and frameworks.
Adhere to all the policies and procedures of Look Ahead Care & Support plus those specific to the project including any statutory requirements.
