We are now looking for a committed individual to join our Gig Buddy team. The role holder will help to deliver a broad range of social activities which increase fun, friends, confidence and independence for our members with a learning disability.
The post calls for an individual with excellent administration, communication and social skills. We need a resilient individual who thrives on regular face-to-face interaction with beneficiaries. Our Clinical Assessment & Intervention Team (CAIT)
Led by a Board-Certified Behaviour Analyst and UKBA(Cert), we have highly specialised team of clinicians, including rehabilitation workers for the visually impaired and behaviour consultants. We assess functional vision and test for cerebral visual impairments, which can often be an overlooked establishing operation for behavioural challenges. As well as direct assessment and intervention to work to increase people’s opportunities in life, we train our support teams to meet the people we supports’ unique needs. Our team members are committed to joint working with families, support teams and our NHS and local authority colleagues to achieve the best outcomes for the people we support.
We work as partners with the residential or supported living teams to build plans that are accepted, operationally achievable, generalisable, and sustainable.
Your experience & personal characteristics
As you will be working with people in their own homes, residential and group supported living services, you must be confident engaging with diverse groups of people, family members and multi-agency professionals. You need to be passionate about equality and equity, being able to see the potential for improvement and knowing how to guide the person we support and their team to achieve the best. You will already have experience working directly with people who have learning disabilities, autism, mental health conditions, multi-sensory impairments, and people who may present behaviours that challenge. You will be fluent in the use of applied behaviour analysis to devise ways that are simple but effective to measure baseline and on-going development. You will be able to support our teams by putting plans into action through your direct modelling.
Your qualifications
This position requires a minimum requirement of a Bachelor’s degree in Applied Behaviour Analysis or a related field (such as Positive Behaviour Support) with at least two years of applicable experience; or Master’s degree in Applied Behaviour Analysis, with one year of professional experience.

Reporting to the Service Delivery Coach in the East of England, you will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.

About the Youth Crisis Service
Our Young Person’s Crisis Service offers a safe, welcoming, and non-judgmental environment outside of normal school hours for those aged 14 – 17 who feel in mental distress or crisis.
About the Role
The Youth Mental Health Worker will support young people aged 14-18 to deescalate crisis and improve their mental health and wellbeing, through focussed 1:1s and structured crisis interventions. The role will involve assessing needs and offering one-to-one support to service users to enable them to begin the process of improving their mental health and well-being and to provide clarity on services and pathways available in the area. The role will be part of a larger Young Person's Crisis team made up of Youth Mental Health Workers, Managers and volunteers.
The successful candidate will have experience of working with young people (under 18) with mental health issues, experience of assessing level of need and experience working in a team or small service.
Service Delivery
- Deliver crisis focussed 1:1s and ensure outcomes which supports young people experiencing a mental health crisis in line with Mind BLMK’s agreed crisis service and requirements as well as contract requirements.
- Assessing need and suitability for the service in line with the access to services process, and undertaking 1:1s.
- Contribute to maintaining an effective volunteer workforce for the service in line with Mind BLMK’s HR policies, procedures and guidance (workload planning and support).
- Carry out health and safety and cash handling in line with Mind BLMK’s policies, procedures and guidance.
- Carry out health and safety responsibilities as directed by the Team Leader in line with Mind BLMK’s H&S policies, procedures and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).
- Carry out responsibilities as directed by the Team Leader for the collection, updating, monitoring and reporting of service data in line with Mind BLMK’s contract and systems requirements and procedures.

Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. 132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.
The initial and co-produced assessment will incorporate models of co-production, observation, and person-centred planning. This is with a view that it is solution focused as to provide a framework for progression towards independent living.
Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.
Create support/ action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.
Liaise with partner agencies regarding making referrals where an unmet need has been identified via the multi-agency assessment.
Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.
Undertake key-working responsibilities for a caseload assigned by the Team Leader/Manager
Support customers to undertake all domestic tasks as past of the assessment of daily living skills, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.

Empower customers to ensure they receive the service and benefits they are entitled to, including assessment and support of budgeting and debt management to garner their tenancy readiness.
