Key Responsibilities for this role

About the role

This role is a great opportunity for someone who is passionate about supporting those who struggle with complex needs, substance use, and dual diagnosis. We are looking for someone who can work flexibly to support residents and participants who struggle to engage with the support available to them through various providers. You will be at the heart of engaging our participants to regain motivation and the strength to step down from acute psychiatric wards and other supported living, by empowering them with the relevant skills and knowledge to support their journey. You will support them towards achieving their personal goals including independent living. You will be based across various homes, hospitals, wards, and supported living accommodations based around Lewisham.

You will provide pathways, support, and appropriate interventions that will enable successful resettlements, and deliver to achieve more safer, integrated communities for our stakeholders. The areas of support will include: Prevention, Transition, Activities of Daily Living (ADL), Support, and Social Inclusion. 

This role will work alongside South London and Maudsley NHS Foundation Trust (SLAM) who provide the widest range of NHS mental health services in the UK. They also provide substance misuse services for people who are addicted to drugs and alcohol with 4,600 staff serving a local population of 1.3 million people.

• Support a caseload of participants, contribute to the development of support plans, risk assessments and reviews.
• Positively promote the pathways available towards independent living within the community. Ensure participants understand what tools and resources are available to them to regain independence.
• Participate in, and encourage R&Ps to participate in the running and development of projects, initiatives, training and volunteering.
• Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent a crisis.
• You may need to attend external appointments when required.

Key to this role is person-centred support and building trusted relationships. The Community Connector will connect people to a broad range of services and interventions in their local area, supporting and empowering people to access opportunities and take positive steps to improve their mental health recovery goals.
 
The Community Connector will support people through:

• One-to-one-person centred support focussing on what matters to the individual

Our aim is to support them and help them prepare for independent living, whilst managing their individual risks. You will be at the heart of providing person centred support in a tailored way, to help our residents improve their quality of life, and achieve their personal goals through providing flexible support. 

In this role, you will be at the centre of researching, sourcing, and providing various opportunities for our residents. This can include partnering with external employers for employment opportunities, liaising with colleges for courses and upskilling opportunities, or community based projects which residents can get involved in. The role is dynamic and you can put your own ideas forward to bring new ideas to the team to help support our residents have access to wider resources. You will work closely with the rest of the care and support teams to enhance the opportunities available to our residents. 

You will work directly with our residents to support them to overcome their personal barriers and challenges to achieve positive outcomes. You will do this through one to one support sessions, group sessions and activities, and general support with their daily living.

Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.

This role consists of three weekly shifts from 22:00 to 08:00, with double cover for each shift.

What you'll do:
- Building supportive, trusting relationships with young people and creating a positive atmosphere
- Working proactively with other members of the team to handle the service caseload and administrative responsibilities
- Conducting key work sessions as required, that are innovative and engaging in order to achieve Support Plan goals
- Co-producing ongoing assessment and management of risks in collaboration with young people with an attitude of 'positive risk taking'

The Heaney Cluster an intensively supported housing scheme for the most challenging customers in the borough.
