Mental Health Support Worker Jobs in Home Based
Using a gender and trauma-informed approach, the Outreach & Engagement Worker will deliver 2-4 daytime outreach sessions per week in Manchester alongside partners, colleagues and volunteers. They will also attend the Street Engagement Hub weekly to provide specialist support to women attending this multi-agency hub. They will provide brief interventions and casework support to women supporting their needs holistically.
You will work with other members of the MASH team to achieve the following outcomes:
- To improve women’s health
- To improve women’s wellbeing
- To increase women’s safety
- To enable women to identify and achieve their goals
- Women who sex work are heard, understood and experience less stigma
- Other agencies and policymakers understand the needs of women who sex work and are more effective at supporting them
Since 1991, MASH has been at the forefront of supporting women in and around Greater Manchester who are, have been or may start sex working and who are facing multiple disadvantage. We were founded as a social justice organisation, starting out as a grassroots, community led response to the HIV/AIDS crisis and we have continued to support women at the sharpest end of inequalities ever since.
Whilst the nature of sex work, the context in which we operate and the support we provide has changed dramatically over the last thirty years, one thing has remained constant: MASH continues to be a haven for some of the most marginalised women in Greater Manchester – women who fall through the gaps, are under served by mainstream services and face stigma, discrimination and multiple barriers to accessing the support they need. At MASH, we meet each woman where they are at and stay with them for as long as it takes for them to get to where they want to be. Crucially, our support is gender and trauma informed and responsive and we bring decades of experience of appropriately supporting women in ways which work for them.

Lincolnshire Crisis Service have two houses in the county, one in Boston and another in Lincoln, we provide a range of support for people with mental ill health and both work to a recovery model. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards. This role does require you to work varied hours as the service is 24/7 and may include some sleep in shifts, these are paid in addition.

In delivering the role, you will work with the Deputy Supported Housing Manager and the wider housing team to provide effective day to day management of our transitional accommodation service. You will contribute to the smooth and safe running of the service by providing consistency and reliability. Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.

Windsor Road service offers 11 beds to people experiencing mental ill health. We are a CQC registered residential care home. We offer individual one to one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days. We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. provides supported accommodation for young people aged 16 - 25 at risk of homelessness, staffed 24 hours a day providing a safe and encouraging environment for residents. Our staff team comprises of Support Workers, Night staff and two Managers. Each Support Worker holds a caseload of residents and meets with them weekly to build a support plan and help them achieve their goals.
In delivering the role, you will work on a rota shift pattern, ensuring young people at the service have non-judgemental, objective, and supportive staff during the day. You will work with the Managers and the wider housing team to provide effective day to day management of our accommodation project. This includes delivering one to one sessions and group work sessions to assist residents to develop personally and socially in order that they grow in confidence and move towards independence. You will help to contribute to a great working environment, with a calm, yet assertive manner, being able to handle potentially difficult situations. You will also support young people to safeguard themselves against abuse and risk.
Based on site, this role is on a seven-day rolling rota, including weekends and bank holidays. There is a small amount of lone working, but you will get to know the team and project, along with extensive training prior to starting on a rota.

About the Role
Community Connectors work as part of an integrated, blended mental health offer to support people with emotional, social and practical needs to access a range of local, non-medical and non-clinical services, to improve confidence, social inclusion and independence.
This role will involve working as part of a multi-disciplinary team with statutory, third sector and NHS colleagues to support clients who are suffering from moderate to severe mental health difficulties. You will need to have experience working with those suffering with mental health illness as well as being able to demonstrate in depth knowledge of the services, support networks and local resources that may assist in improved outcomes for those dealing with complex and enduring mental health issues. The ability to be adaptable, work independently and the resilience to work in some challenging environments are key attributes for this role.
Service Delivery
- Assess suitability for community connection support as part of a blended team approach and support individuals to access the service by utilising in depth knowledge of local voluntary and community services and networks in your respective area
- To work with individuals to coproduce a plan to address their mental health and wellbeing goals. Am enable people to achieve these goals through facilitating access and connecting with appropriate resources
- Enable people to access community resources which may include accompanying them.
- Offer support, guidance and help where possible to people to gain confidence, overcome initial fears and seek solutions to any barriers inhibiting attendance.
- Take a holistic approach, based on the individuals’ priorities and wider determinants of health that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.
- To work as part of an integrated team to ensure the best outcomes for each person accessing the service.
- Identify and build good working relationships with the community groups, relevant services and activities for partnership work and signposting as well as keep updated information about them.
- Competently work as part of a blended team, under direction from Mind BLMK and ELFT, working with independence and in an agile way to support people with complex mental health challenges.

About the Role:
Oaklands & St Kilda is a pair of 6-unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.
The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.
Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.
This is a Waking Night role working to support young people throughout the night at the service. This role completed 10 hour shifts 4 days a week across all days of the week.

Liverpool SHS is a mental health, 12 hour supported housing and community based service, working to a recovery model. Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service.
