We are looking for self-motivated individuals who like to be part of a team but equally can work on their own. The post holder must have a desire to make a difference in promoting liver health and hepatitis C among services and affected communities, and by increasing access to treatment and liver disease care. The Hepatitis C Trust develops projects nationally where peers use their lived experience to provide education and training, one to one support and increase awareness, and access to testing and treatment for people who may be at risk of various health conditions. This exciting new role supports the delivery of the liver cancer surveillance project across Nottinghamshire working closely with the clinical team and being a core element of the day to day running of the mobile van service on which the project operates. We are looking for a passionate and skilled peer lead who has a willingness to travel and we welcome creativity and innovation in all our work. We are a patient-led organisation – you will be working in an environment where the patient/service user/client is placed at the centre of all that you do. It is essential the post holder hold a driving licence.

As a Mental Health Care Navigator, you will play a vital role in supporting individuals with SMI to reconnect with their communities, addressing barriers such as social isolation, cost of living, debt, and physical inactivity. You will provide personalised, strengths-based support, helping people set and achieve wellbeing goals, and link them to local community groups and services. The role also involves building partnerships with VCFSE (Voluntary, Community, Faith, and Social Enterprise) organisations and statutory agencies, ensuring safe and effective referrals, and supporting the development of an alliance of mental health support providers across the city.

Key Responsibilities

• Provide one-to-one, person-centred support to individuals, including home visits, to co-produce wellbeing goals and address wider determinants of health.
• Build trust and offer non-judgmental, strengths-based guidance, respecting diversity and lived experience.
• Forge strong links with VCFSE and statutory partners, ensuring individuals are referred to safe, accessible, and effective community groups.
• Support local groups to meet safeguarding, health and safety, and information governance standards.
• Review and evaluate grant applications, supporting the distribution of resources to local groups with the greatest potential impact.
• Capture and track data on service user progress, ensuring feedback is collected and shared, and that all information governance and data protection standards are met.

Citizens Advice Richmond is launching an exciting new project and seeks a passionate Financial Resilience Navigator to join our team. This rewarding role involves providing holistic support to vulnerable households across the Richmond Borough, helping them achieve greater financial sustainability.  

You will serve as a single point of contact, assisting clients with welfare benefits, income maximization, debt management, housing, and related issues. This role emphasizes connection, compassion, and coordination, requiring collaboration with various organizations and council services to ensure comprehensive client support. Key responsibilities include client advocacy, community outreach, case management, and stakeholder collaboration.

Temporary cover (6-8 weeks may be up to 12 weeks) must be able to start 2nd June 2025 (plus 6% contributory pension. 35 hours per week).

Due to the immediate need for this post we will be interviewing suitable applicants as they are received.

The Care Navigator will support those with long term conditions to access timely care and community support services. Working closely with Camden GP practices and Integrated Care Teams is required as is accurate record keeping using IT systems. Through timely intervention and care co-ordination, unnecessary hospital admissions, GP appointments and residential placements will be reduced.

The successful candidate will come from an information/advice, social care or health background, must be computer literate, and have excellent people and communication skills, and able to work in a multi-disciplinary team. Experience of person centred planning is required. It is important that a candidate promotes a culture that values, protects and uses information for the success of the organisation and benefit of its clients/patients.

For this role it is essential that you to have the ability to travel across Camden as the role involves visiting clients in their own homes. This is a full time role with no job share opportunity and works out of GP practices in Camden. AUC offers a contributory pension, and season ticket or bicycle loan facility.

We are seeking an individual to raise awareness of the Fund’s welfare offer amongst social welfare agencies across South and Central Scotland but also, crucially, to help address the issue of social isolation and feelings of loneliness experienced by some members of the RAF Family.

The post holder will be working with socially isolated individuals to identify meaningfully enriching opportunities to reduce feelings of loneliness, some of whom may have complex and challenging welfare needs, to develop actions plans and coordinate support to overcome any barriers preventing successful connection into these enriching opportunities, assisting them by identifying appropriate support from within the RAF Benevolent Fund and the wider statutory and military charity sector.

The successful candidate will have experience of working in a social welfare role supporting individuals, have strong people and relationship-building skills, as well as empathy with or understanding of the issues affecting the Armed Forces community.  Individuals with a social prescribing or working aged veteran support background are encouraged to apply.

This is a community-based position working from home but covering South and Central Scotland, delivering both remote and face to face support.  The candidate must have their own vehicle to use to travel around these regions.  Occasional travel to Edinburgh and London will be required.  The successful candidate for this role will need to be Enhanced DBS checked and prove they have the right to work in the UK.
