The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community.

We are looking for the right people who can empower residents to take control of their lives. This will involve working with residents in their homes who may be experiencing a number of issues, so you will need outstanding listening and communication skills, empathy, problem solving and enthusiasm. You will need excellent organisational skills, be adept at seeking out information for relevant community services and have the tenacity to help people achieve sustainable improvements in their wellbeing.

Main duties include:

· Managing a caseload of households.

· Providing monthly home visits and providing a continuous point of contact for all members of the household.

· Working with the residents to assess their needs, create personalised action plans and locate support within the community.

· Supporting residents to prioritise and address practical, health, social and emotional issues in their lives which affect their wellbeing.

This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.

This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.

We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.

Main Responsibilities:

· Undertake 1:1, group, detached and outreach work with young people

· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.

· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.  

· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.

· To maintain confidential, up to date and accurate records of all sessions and plans.

· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:

o mental health and wellbeing of children and young people.

o Supporting vulnerable groups who would not otherwise receive support (levelling-up).

o Improving children and young people’s engagement with EET.

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting

· Experience of working with young people, delivering therapeutic groups and/or 1 to 1 interventions in a youth work or similar setting.

· To have an active interest in working with those affected by mental health issues and recognise the impact this has on their life.

· Experience of building positive, impactful relationships with young people responding to their needs

The Family and Youth Engagement Worker is a new role within our organisation, which will provide additional support to an existing team working in London providing comprehensive support to young people and families living with HIV.

The role encompasses family-based support, organising and facilitating group meet-up events, offering guidance and support around engaging with services, and providing mentoring and coaching support to individual young people.

The role will be pivotal in furthering the charity's mission to enhance the health and well-being of children, young people, and young adults growing up with HIV.

Background:

First Step Trust (FST) is a charity that provides work experience, training and employment opportunities for people excluded from working life because of mental health problems or other disadvantages (workforce members).

Our approach is about giving workforce members the chance to overcome some of the difficulties facing them by getting them involved as colleagues. We provide ‘on the job’ training, advice and support to enable each person to improve their basic skills and knowledge of the various work undertaken. This will include supporting workforce members to gain recognised qualifications and access to work placements with our customers and other organisations.

We are in the process of developing a more formalised training and employment support programme (SMaRT Pathways) particularly, but not exclusively, in the area of the Automotive trade.  SMaRT Pathways will include the delivery of vocational training through the use of various technologies including video, Virtual Reality, Augmented Reality and other mediums making the learning process more accessible to those who struggle with traditional learning methods due to issues around literacy, numeracy and low levels of self-confidence/anxiety. 

Job Role:

The main objective of this post is to enable people with mental health difficulties and other disadvantages (workforce members) to engage with and access open employment.  The EDTW will achieve this by taking responsibility for proactively recruiting people to the project, overseeing their development whilst at the project, including delivery of training courses as and when required and supporting them to move on to employment or further training.

Main duties:

Networking with other services in the area to raise awareness about FST, promoting work placements and training courses as well as promoting the business services.

Recruitment and development of potential workforce members - being proactive in following up on enquires regarding work placements, interviewing potential workforce for work placements at the project.

Employment support – assist workforce looking to move on to paid employment by providing support in job search, CVs and interview preparation. This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth. 

Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project. 

The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

You will support patients within Primary Care GP practices in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. Job Description 

LMK is looking for passionate and organised educators (LMK Leaders) to run community workshops, aimed at young people aged between 11 and 24. Through engaging, relatable, workshops you will help young people identify signs of healthy and unhealthy relationship behaviours, provide strategies for them to recognise the early warning signs of abuse and give them tools to keep themselves and their friends safe. 

The minimum hours required per workshop is 3 hrs. This includes arriving 30 minutes before delivery, 1.5 to 2 hrs delivery and then remaining back 10 minutes and being accessible to young people who would like further information or a one-to-one conversation, surrounding the subject of relationships.

There is also 30 minutes admin, which is usually completed off-site, such as inputting student surveys and a Leader survey.

Responsibilities 

• Always adhere to LMK and partner safeguarding practices and procedures.

• Respond to comms (emails/WhatsApp) sent out by LMK core team members, in a timely manner, to support planning and organising of sessions.

• Meet with co-facilitators to carry out pre-session planning, minimum two working days before delivery.

• Plan and deliver interactive LMK workshops in educational, sports and community settings in line with LMK delivery style and ethos.

• Use your experience and knowledge of the issues affecting young people to tailor the workshops accordingly. 

• Ensure all youth participants and the adults in school/community organisations who attend a LMK workshop complete the post session feedback surveys.

The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
• To ensure that all The Passage policies and procedures are being adhered to, particularly those relating to Code of Practice and Confidentiality.
• To contribute to the effective implementation of The Passage’s Diversity and Equality Policy as it affects both The Passage and its work with vulnerable adults.
• To at all times undertake your role in a professional manner maintaining a high quality standard of work, and to always work in accordance with the aims, values and ethos of The Passage.

Desired experience:

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.
• Experience of assessing client needs and supporting clients to address their needs through assertive outreach and case management.
• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

Desired knowledge:

• Understanding the support needs of the street populations with complex health, and housing needs.
• Knowledge of services and legislative environment regarding housing, health, work and immigration as it relates to people experiencing homelessness.
• Ability to communicate, both verbally and in writing and collate and evidence work and outcomes using case management databases.
• Knowledge of Psychologically Informed Frameworks.
• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.
• Knowledge of housing pathways available to dual diagnosis clients and how to access them.
• A working knowledge of how to support people with either substance or mental health needs.

The purpose of the role

The People Team provide advice, guidance and support to all National Theatre staff, line managers and senior leaders on people policies; terms and conditions of employment; recruitment and selection; performance management; staff engagement; diversity and inclusion; trade union and employee relations; discipline, grievance, and capability processes; and training and career development. We pride ourselves on providing a customer-focused, flexible, and agile service and aim to support and facilitate an open and inclusive culture where all staff can thrive.

The successful applicant for this role will lead on the development and review of employment policies, undertaking benchmarking and researching best practice to ensure the NT has a full range of accessible and inclusive policies. They will analyse and interpret business intelligence and workforce data to inform policy development and decision making whilst working with colleagues to develop guidance and training for managers and act as the department’s central advisory expertise. They will also lead on work related to the NT’s pay framework and policy and manage engagement surveys, developing routine reporting and analysis.

The successful candidate will have the following: 

• Experience across a range of People functions that demonstrates an ability to work autonomously across a range of functions, including generalist, policy development, reward and interpreting terms and conditions.
• Experience of analysing and presenting information both written and verbally in informal and formal settings.
• Experience of working with information, databases and ability to interrogate and analyse data.
• Experience of working in a flexible and pragmatic way to problem-solving. Able to seek and exploit opportunities to advance objectives.
• Proven ability to communicate effectively orally, digitally and in writing adapting the message for a diverse audience in an inclusive and accessible way.
• Experience of managing multiple stakeholders, with the ability to build strong collaborative working relationships.
