About the role

The Post Holder will work within Spires’ Women’s Service. This service is comprised of:

A dedicated nighttime outreach service to support women who are involved in street-based sex working. This includes follow up work, casework, and multi-agency working.

Centre-based support for women with complex needs who are homeless or at risk of homelessness. This also includes casework and supporting Spires’ Housing and Homelessness team.

Key responsibilities will include:

· Supporting a caseload of clients

· Completing one nighttime outreach shift a week (this is carried out in the Spires car)

· Supporting in the running of Spires’ women’s drop in services

· Supporting Spires’ Housing and Homelessness service where required

· Representing Spires to external stakeholders at multi-agency meetings

· Ensuring detailed and accurate records are kept of all client work.

About you

You will have previous experience of working with people exhibiting complex needs and people experiencing homelessness. Ideally you will have experiencing supporting women with some of the following support needs: substance addiction, mental health needs, domestic violence, no recourse to public funds and sex working.

You will also bring with you the following skills and experience:

· Experience of challenging appropriately and maintaining clear and appropriate boundaries.

· Networking and effectively liaising with specialist service providers or agencies in order to establish and/or improve access to services for clients.

· Sound understanding of the issues faced by women and other individuals involved in sex work.

· Understanding of the legislation around supporting the victims of Domestic Violence.

· Understanding of the effects of trauma on people and how to work in a trauma informed way.

· Good understanding of safeguarding practices in outreach work, social care and/or day centre environment.

About the organisation

The organisation was established in response to homelessness, which had become a growing concern in Streatham in the late 1980s. Initially providing a hot meal, clothing and advice to those who came for help, they have developed into an organisation that provides a range of services, including the Housing and Homeless Team and Woman’s Team.

Aside from the 50+ young people we see every day at our day centre, New Horizon’s Outreach service also works with young people on the streets and in the community. The team works pan London to deliver a unique, youth-specific outreach service to young people currently or at risk of rough sleeping. We use our extensive housing expertise to place those young people into accommodation that is appropriate and safe whilst encouraging them to access the day centre to benefit from the wide range of services we offer. The Outreach Worker will work as part of Rough Sleeping Team to deliver at least two street outreach shifts per week and transitional casework to young people facing rough sleeping. We have a vacancy for a Flourish (Refugee and Asylum Seekers) Comminity Outreach Worker to join our team, supporting refugees and asylum seekers in the community with multiple needs who have mental health as a primary need, through providing advice, information and holistic support.

Complex needs are defined by the National Complex Needs Alliance as a combination of health needs and social needs.

About the Service

‘Flourish’ is a highly flexible and responsive outreach service developed to meet the needs of refugees and asylum seekers in Hertfordshire. The aim of the service will be to support refugees and asylum seekers in the community with multiple needs who have mental health support as a primary need. 

Purpose of Post

• To provide advice, information, onward referral and holistic support to clients with mental ill health.
• Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.
• To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
• To remain a source of independent support for all clients.
• Embed a person centred and solution focused approach in all aspects of the role.

Key Responsibilities:

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.

We are looking for a Lead Worker  to join the Hillingdon Streat Outreach Team.

You will:

• Lead outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation. 
• Represent the team at multi-agency meetings 
• Accurately record all work completed within given timeframes 

To succeed as the Lead Worker, you will have:

About this role…

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.

We are looking for an  Assistant Support Workers  to join the Street Outreach Rough Sleepers Team

Thames Reach welcomes applications from people with transferable skills.

You will:

• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Work alongside senior colleagues to identify the fastest and most appropriate route away from the streets for the people that we are working with 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Maintain and support relationships with partner agencies 
• Accurately record all work completed within given timeframes

Community Support Worker

We are looking for a very motivated and able Community Support Worker to support the Community Connections Team to meet the relevant requirements of the Community Connections contract.

The Role

The Community Support Worker will be required to work alongside the full-time area Co-ordinators to support activities and groups and carry out one-to-one initial appointments for people residing in Surrey.

This will involve attending some groups in venues across the Surrey Downs area, to assist in the facilitation of groups to ensure they run safely and meet the needs of the clients.

The role will also involve supporting the full-time co-ordinators to complete initial appointments with new clients, including the completion of recovery stars and plans and updating client records.

Key responsibilities include:

• To support groups, courses and activities and work with the Community Connections Team to produce a quality service.
• Supporting the full-time co-ordinators to complete initial appointments with new clients, including the completion of recovery stars and plans and updating client records.
• To support the area co-ordinators in maintaining contact with those using the service via differing methods.
• To work with the area co-ordinators to contact clients who are not currently engaging to ascertain their current needs and update records accordingly.
• To be able to assess risk on a daily basis and react accordingly.
• To motivate people attending groups, activities, and one-to-one sessions.
• To promote “if you don’t do it, it doesn’t happen” attitude.
• To work with volunteers helping out with groups and activities.
• To work with the team of professionals and communicate well with colleagues working from different locations.
• To maintain databases and undertake any other administrative duties.
• To work collaboratively with other departments.
• About You

You will have experience of working directly with vulnerable adults (preferably with mental ill-health)

You will also have:

• Experience of working within the health & social care sector.
• A demonstrable understanding of the needs of vulnerable groups.
• Knowledge of Mental Health sector and relating issues.
• Good communication skills (oral and written)
• Ability to work on own initiative as well as part of the team.
• IT literate in MS Office, Excel, Virtual Platforms, Social Media and Networks.

Please note: A driving licence is desirable, however candidates without a driving licence will be invited to demonstrate how they could fulfil the requirements of the role without one.

About the Organisation

The charity is a mental health and emotional wellbeing charity, supporting Surrey residents since 1994 in Banstead, Elmbridge, Epsom & Ewell and Mole Valley.

Offering a variety of face-to-face and online services, available to adults (aged 16+) living in Surrey who would like to restore and/or maintain their mental and emotional wellbeing (no diagnosis needed). The services are free of charge, with no waiting list and no time limit.
