This role is based at our centre a short walk from Clapham Common tube station, in London. We support homeless people from the local area and further afield. Our services are open access, so the people who come to the Ace of Clubs are diverse and from multiple walks of life. Consequently their needs can be complex, so we are looking for a skilled communicator who can support people, many of who live chaotic lifestyles. Our purpose at the Ace of Clubs is to address people’s basic human needs by providing food, clothing, toiletries, and practical support. We help people link up with housing, healthcare, education, and employment. Our Support Workers work to help individuals overcome barriers to achieving wellbeing, whether it be through guidance in the support system, learning new skills or supporting service users to build the confidence and skills to enable them to navigate their life challenges and advocate for themselves. You will be responsible for supporting young people aged 14-25 years, enabling them to access a range of social, emotional, and practical support as they navigate the complex and challenging processes of the UK asylum system.You will help us run activities for young people at our advice hub on Wednesday evenings in Brent and run our youth group on Thursday evenings in Brent n as well as our sporting activities and trips.

The Creighton Centre is a local charity with a long history of working in Hammersmith & Fulham, caring for older, vulnerable and disabled people. We tackle problems caused by social isolation and disadvantage by developing and providing programmes that offer people practical solutions and emotional support.
The Learning Disability Outreach (LDO) project needs a Literacy Tutor to teach people with learning disabilities, helping them to improve their reading and writing skills, to enhance their lives and achieve their aspirations. The teaching will be mostly in a 1-to-1 capacity, or with small groups. You will need some experience of teaching literacy skills and a compassionate attitude.
An interest in gardening and educating people in a small community garden would be a bonus, although not essential.

Aside from the 50+ young people we see every day at our day centre, New Horizon’s Outreach service also works with young people on the streets and in the community. The team works pan London to deliver a unique, youth-specific outreach service to young people currently or at risk of rough sleeping. We use our extensive housing expertise to place those young people into accommodation that is appropriate and safe whilst encouraging them to access the day centre to benefit from the wide range of services we offer. Purpose of Post
- To provide advice, information, onward referral and holistic support to clients with mental ill health.

About the role:
The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.
Main duties:
- To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
- Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
- Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
- Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.
General responsibilities:

Mind in Croydon is a charity concerned with the welfare of people with mental health problems living in the London Borough of Croydon and the surrounding areas. Mind in Croydon delivers Mental Health Transformation Program services alongside Croydon BME Forum, these include Croydon Health and Wellbeing Space and Mental Health Personal Independence Coordinators. This exciting role will manage the provision of a non-clinical Recovery Space (crisis café) funded by South London Partnership Commissioning. This project operates in the day and evenings, people are referred by our Trusted Assessors in various Teams such as Croydon University Hospital, Psychiatric Liaison, Assessment and Liaison, Croydon College, Home Treatment Team and others.
The aim of the service is to divert people, who would be better supported elsewhere, away from Accident and Emergency (A&E) and towards services that can support with their immediate crisis within the Borough of Croydon. The Recovery Space Service Manager leads this service which is non-clinical but a warm and welcoming setting to support and hold individuals during the day and evening. The Recovery Space service referrals will not be accepted after 10pm for attendance at night.
The role will work closely at senior operational level with external partners, clinical, commissioning, and voluntary sector organisations to plan, develop and promote the highest quality service. They will manage a team of nine staff, three are Crisis Workers, operating a day service, one outreach crisis worker also operating during the day and five Recovery Space Workers in the evening. The role will need to produce reports, develop, and implement policies and procedures in line with best practice and to ensure the service is making a difference to people’s lives. The role will work with other service managers including those managers within the Transformation services to ensure a One Team approach to provide an effective and efficient service for clients.

The Community Support Worker will be required to work alongside the full-time area Co-ordinators to support activities and groups and carry out one-to-one initial appointments for people residing in Surrey.
This will involve attending some groups in venues across the Surrey Downs area, to assist in the facilitation of groups to ensure they run safely and meet the needs of the clients.
The role will also involve supporting the full-time co-ordinators to complete initial appointments with new clients, including the completion of recovery stars and plans and updating client records.
Key responsibilities include:
- To support groups, courses and activities and work with the Community Connections Team to produce a quality service.
- Supporting the full-time co-ordinators to complete initial appointments with new clients, including the completion of recovery stars and plans and updating client records.
- To support the area co-ordinators in maintaining contact with those using the service via differing methods.
- To work with the area co-ordinators to contact clients who are not currently engaging to ascertain their current needs and update records accordingly.
- To be able to assess risk on a daily basis and react accordingly.
- To motivate people attending groups, activities, and one-to-one sessions.
- To promote “if you don’t do it, it doesn’t happen” attitude.
- To work with volunteers helping out with groups and activities.
- To work with the team of professionals and communicate well with colleagues working from different locations.
- To maintain databases and undertake any other administrative duties.

- About You
You will have experience of working directly with vulnerable adults (preferably with mental ill-health)
You will also have:
- Experience of working within the health & social care sector.
- A demonstrable understanding of the needs of vulnerable groups.
- Knowledge of Mental Health sector and relating issues.
- Good communication skills (oral and written)
- Ability to work on own initiative as well as part of the team.
- IT literate in MS Office, Excel, Virtual Platforms, Social Media and Networks.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.
We are looking for a Lead Worker to join the Hillingdon Streat Outreach Team.
You will:
- Lead outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
- Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this
- Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation
- Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
- Represent the team at multi-agency meetings
- Accurately record all work completed within given timeframes
To succeed as the Lead Worker, you will have:
- A strong commitment to providing a high standard of service
- Experience working with people who have rough slept or vulnerable adults and supporting them to access services
- Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently.
- Good communication skills and confidence in public speaking when representing Thames Reach externally
- Experience in forming strong working relationships with partner agencies, being able to challenge them when required
- Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone
- A clean driving license (desirable)
