About the role
As a Peer Advocate, you will support people experiencing homelessness to review their health needs and access health services. You will support clients to register with health services, attend appointments and communicate with health professionals, ensuring their needs and wishes are understood. You will provide some support to our fantastic team of volunteer Peer Advocates, standing in for them if they are unavailable and modelling best practice to new volunteers.

Job Title: Northern Ireland Support and Development Worker

Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. Queens road and herons court are 9am-5pm Mental health services supporting 22 low-medium need customers.

What you'll do:
If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. In some services, this will include delivering personal and physical care as appropriate.
Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
Participate in the support planning and risk management, as facilitated by the lead Support Worker.
Carry out security duties to ensure the safety of the customers and premises.
Monitor the CCTV throughout the shift.
Enable customers to make full use of community facilities by providing support as directed.
Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities.
Report any observations relating to customers welfare.
Adhere to all the policies and procedures of Look Ahead Care & Support plus those specific to the project including any statutory requirements.
Ensure Look Ahead Health and Safety policies and local protocols are always adhered to.
Maintain records as required at the project under the direction of the support workers / management.
Participate in team meetings/reviews and the general development of the service.

Young Adults Diabetes service
Part Time - 30 hours per week

Poplar HARCA is committed to providing a high quality youth offer to young people. The tried-and-tested youth offer has been developed through exhaustive consultation with young people. This included 1,600 conversations with young people to define the programme offer.
The Youth Support Worker role within the diabetes service is a new one and one that will develop during your time with us. You will be expected to work independently and feedback progress to the rest of the team.
As a Youth support worker, you will be required to be present and participate during clinic, educations session and peer support for young people living with diabetes.
The Youth Support Worker will work with young people on a one-to-one or group basis, and effectively manage a case load of young people to achieve positive health and social outcomes. Working alongside partners within the NHS, they will receive referrals for young people/ adults who require youth support linked to their diabetes.

Job Purpose

The Role
Through sharing wisdom from their own experiences, the Mental Health Support Worker will inspire hope and belief that recovery is possible in others. As an integral and highly valued member of the team, you will provide formalised peer support and practical assistance to people using the service in order for them to regain control over their lives and their own unique recovery journey.
In particular, the Mental Health Support Worker will support the smooth transition of people using the service to other Mental Health services, by helping them to complete recovery and crisis plans for example. Within a relationship of mutuality and information sharing, you will promote choice, self-determination, and opportunities for the fulfilment of socially valued roles and connection to local communities.
Key responsibilities include:
- Work alongside an agreed number of people using the GPIMHS / MHICS service on a 1:1 basis, in person and / or virtually, from a lived experience perspective.
- Work as part of a multidisciplinary team.
- Deliver peer support interventions as agreed within the multidisciplinary team, and within a peer relationship.
- Use personal, experiential knowledge and to share lived experience when appropriate to benefit others.
- Assist others to create their own recovery and crisis plans, particularly people who are transitioning from the service to another Mental Health service (e.g. SABP Community Mental Health Recovery Service)
- Support people using the service to identify and overcome fears within a relationship of empathy and trust.
- Signpost to various resources, opportunities, and activities within the Organisation and in communities to promote choice and informed decision making.
- Accompany people who use the service to appointments / meetings / activities of their choice and performing a range of practical tasks, aligned to recovery goals.
- Assist in risk assessments with multi-disciplinary staff, highlighting any changes in service users’ presentation relevant to their safety and feedback accordingly.
- Maintain a working knowledge of current trends in mental health, recovery, and peer support by reading books, journals and accessing peer support networks.
About You
You will have Lived experience of mental illness or experience of caring for somebody who has mental health problems, along with:
- Experience accessing primary and / or secondary mental health services.
- Paid or unpaid experience of working in a lived experience role.
- Numeracy and Literacy at a Key Stage 3 level or equivalent.
- Computer literate in Microsoft Office, and willingness to use the Organisation’s IT systems.
- Experience of recovering a meaningful life.
- Experience of being in a supportive and enabling role.
- Ability to share personal story of recovery in a professional manner.
- Understanding and knowledge of recovery, of the issues and concerns of mental health service users, and of the impact of stigma and discrimination.

Senior Youth Participation Officer
Fitzrovia Youth in Action is looking for an experienced, passionate and enthusiastic person for our Senior Youth Participation Officer role. If you feel like this is you, we would love to hear from you!
The post holder will coordinate and develop our new young people drop-in programme and additional youth action programmes, aimed at engaging local young people into the work of FYA. The successful candidate will oversee the development and delivery of the drop-in sessions, as well as deliver some youth led social action programmes. The role will include recruiting young people, engaging them in co-producing fun and creative activities and workshops which will take place at the drop-ins, as well as supporting them in planning and delivering the social action programmes.
Typically, sessions will occur Sundays (TBC) and young people from the ages 8 to 18 will attend. We are also planning to start a second drop-in session from September, which is likely to run on a Friday evening.
The post holder will support young people to plan some social action programmes directly, as well as support young people into other youth-led community action programmes at FYA.
Examples of projects include youth-led football tournaments, street parties and festivals and peer education activities.

A bit about the role:
As an Outreach Support Worker, you will provide a service where planned support and crisis intervention is delivered to vulnerable people with low, medium and high support needs living in independent accommodation with the aim of them maintaining their home. Some of the key results for the role include:

Manchester Mind is looking to recruit a Mums Matter Project Worker to work alongside our existing Co-ordinator to expand delivery of the established project. The role will involve the delivery of courses, workshops, peer support groups and 1:1 support in Children’s Centres across Manchester to teach and develop self-care skills for women experiencing mental health difficulties in the first two years of parenthood. LGBTQ+ Project Worker (Supported Housing)
This role provides support to LGBTQ+ people who are living in Stonewall Housing supported accommodation or in the community. The goal is to help people prepare for independent living in their own tenancies. People usually stay with us for 6 – 18 months and the project worker will meet with each resident to assess their needs and agree goals which will help them achieve independence. The Project Worker then puts together a support plan and meets regularly with residents to provide support, challenge and encouragement as they make their journey.
Collaborating closely with our housing providers, our project worker will actively develop fresh partnerships within the local community to enhance the support and services available to our residents.
Key Responsibilities
· To provide a high quality, flexible and responsive support service to LGBTQ+ people, supporting them for independent living or suitable alternative housing through the provision of 1-1 support sessions and group work.
· To assess the individual needs of each person and provide a bespoke support plan.
· To ensure that effective service user participation mechanisms are in place.
· To ensure a high level of customer care and practice at all times.
· To develop links with relevant external agencies.
Main Duties of the Post
Support Sessions
· To give holistic support to LGBTQ+ people accessing our supported accommodation service and to provide proactive support to these service users who are sometimes hard to engage.
· To meet service users regularly to provide structured support, in relation to LGBTQ+ and Housing specific issues, such as gender identity pathways, mental health services, health issues, safeguarding, liaising with the professional network.
· To work with LGBTQ+ people accessing supported accommodation to develop and review individual support plans and risk assessments.
· To liaise with other service providers ensuring service users receive the necessary support to sustain their accommodation, acquire relevant independent living skills, maintain or improve positive physical and mental health and access into meaningful occupation of their time.
Financial Support
· To assist service users in maximising and managing their income including universal credit, ESA, PIP and Housing Benefit.
Semi-independent Living Support
· To work with service users to enable them to develop the life and social skills necessary to sustain their accommodation and prepare them for independent living.
· To assess individual service user’s suitability for independent accommodation.
· To act quickly to manage incidents and to advise, support and assist service users unable to maintain supported accommodation into more appropriate housing options.
Resettlement
· To support service users in the completion of application forms necessary to support move-on housing, including the private rented sector.
· To ensure that all service users are provided with information about local services whilst in supported accommodation and during their move on.
· To ensure all service users are fully aware of their rights and responsibilities in their home.
· To work with housing providers, both public and private, to negotiate move on options.
Groupwork, Consultation and Participation
· To work as part of a team in developing user participation.
· To identify and develop appropriate and flexible processes for consulting with our service users, via social media platforms, newsletters, events and workshops.
· To devise innovative and creative ways of involving LGBTQ+ people in the running of the schemes with an independent approach.
· Supporting our residents to participate in group and peer support and to access online support mechanisms.

Basis is looking to appoint a Navigation and Support Worker (Intensive Support) who can provide appropriate emotional and practical support to women in the sex industry and/or those who are being sexually exploited. The women we work with often require intensive practical and emotional support as well as help to navigate health and, social care, the criminal justice systems and other services. The women’s circumstances can impact their mental or physical health. They may also be at risk of or experience homelessness and come into contact with the criminal justice system.
