Job description
Job Purpose
The hospital to home service works collaboratively with hospitals to support people when they are discharged from hospital to return home. As a Hospital to Home Outreach Worker, you will provide temporary, practical and emotional support to service users within their home and make sure they understand the care and local community services available to them.
The service is time limited, usually up to 4 or 6 weeks, to adjust and settle back in their homes.
Key Tasks
Assess the needs of service user’s either in hospital before discharge, or in their homes, and understand what barriers and enablers there might be to moving home.
Aid the discharge process and improve patient experience through tasks such as:
Key cutting
Co-ordinating with colleagues for the setup of the home environment such as checking fridge for in-date food
Checking heating, electric and gas is functional prior to discharge
Allowing access to the property for cleaning/repairs and receiving equipment
Provide and/or arrange the practical and emotional support needed to enable service users to return home and/or remain safely at home.
Be vigilant and make referrals/signpost for additional needs that our wider team, or partners can support. E.g. for benefit checks, energy advice, carers support, befriending.
Help maximise service user independence by adopting an enabling approach.
Develop a listening, caring and professional relationship with the service user and their families/carers.
Provide domestic practical support within the home such as:
Light cleaning,
Washing (e.g. bedding),
Changing bed sheets etc.
Prepare and service drinks and simple meals with or for the service user ensuring nutritional needs are met.
Accompany service user outside the home e.g. hospital/GP appointments etc.
Support service users with other tasks e.g. making telephone calls on their behalf, reading and responding to correspondence, completing simple forms (training will be provided) as directed by the service user’s etc.
Ensure services provided by AUKEL in the community are safe and person centred.
Report safeguarding concerns, in accordance with AUKEL safeguarding procedures.
Provide cover in the case of sickness and annual leave periods of colleagues.
Support service user reviews, ensuring all visiting information is uploaded on to Salesforce.
Support volunteers in the team and provide support and guidance to their development.
Work with a range of professionals including:
Supporting health and social care professionals with patient discharge
Liaising with the hospital health and social care professionals (e.g. Social Workers) to facilitate smooth supported discharge.
Handypeople & contractors.
Administration
Input all records, reviews, and visiting information to service user’s case notes in a timely manner to maintain up-to-date and accurate records on Salesforce.
Report all “no access” failed visits, or changes in service user’s condition or circumstances in accordance with AUKEL’s policies and procedures.
Ensure service user comment sheets are completed.
Ensure service user’s feedback questionnaires are sent at the end of each intervention and record returns.
Submit travel expenses regularly for sign off by manager.
Quality
Be familiar with and to implement AUKEL policies and procedures in line with our Values.
Ensure the service is delivered in accordance line with Care Quality Commission principals.
Ensure integration with other AUKEL projects, and service users are referred and/or sign-posted to appropriate support services available to them.
Respond to safeguarding or other concerns and make appropriate reports and management action.
Liaison
Work in collaboration with other agencies providing support within the service user’s home.
Work under the direction of the service manager and project officer/s.
Represent AUKEL and participate in appropriate external meetings and events to remain aware of local, regional, and national issues affecting quality and compliance issues affecting care and home support services.
General
Meet regularly with your line manager for support, supervision, and appraisal.
Attend team and staff meetings, and other meetings as required.
Undertake any other duties within the competence of the post holder as may be required from time to time for the continued smooth running of AUKEL.
Complete any training which is required to fulfil the role.

As Principal Policy Adviser (interim), you will collaborate with Uplift’s team and key partners to drive the design, development and delivery of work that generates influential policy and ideas addressing the challenges of a rapid and fair transition away from oil and gas production in the UK.

This role is a 9 month long contract to cover a secondment within the team.
Core responsibilities will include:
Policy: Lead Uplift’s policy work, spearhead the development and testing of innovative policy initiatives, and identify and work through knowledge gaps relevant to Uplift’s strategy.
Coordination and Strategy Delivery: Work with teams to set and execute policy priorities, provide in-house policy support, contribute to the management of the Research Policy and LEgal Tea, and develop systems for quality assurance and knowledge management.
Advocacy and Network Support: Build capacity amongst other Uplift teams to understand the intersection of their work with policy topics and represent Uplift to policymakers and partner organisations.

Digital Fundraising Officer - 12-month Maternity Cover
The Veterans' Foundation is entering its second decade with a clear ambition to build a more sustainable, diversified and data-led fundraising model. Following ten years of rapid growth driven primarily through lottery acquisition, the organisation is now evolving its approach to strengthen supporter engagement, improve retention and maximise fundraising performance across a wider range of income streams.
This role will play an important part in that transition. As the Foundation launches a new website, develops its use of Salesforce and strengthens its supporter communications, the Digital Fundraising Officer will help deliver effective digital fundraising and engagement activity across multiple channels. The postholder will support campaign delivery, manage digital content and platforms, and use data and insight to improve performance and supporter experience.
Principal Duties
The main duties of the Digital Fundraising Officer are:
· To plan, create and deliver fundraising, supporter engagement and stewardship communications across digital channels including email, website and social media
· To manage content across the Veterans' Foundation website, ensuring it remains accurate, engaging, accessible and aligned to fundraising and organisational priorities
· To support delivery of integrated fundraising campaigns across digital channels, helping to maximise income, engagement and supporter retention
· To use Salesforce and other digital systems to support campaign delivery, audience selection, reporting and data management

Person Specification

Position Summary
The Head of Fundraising will lead GSG Impact’s fundraising function at a critical stage of organisational growth and income diversification. The role is responsible for delivering GSG Impact's fundraising strategy, supporting the achievement of annual income targets of approximately £3 million per year, converting strategic priorities into fundable propositions, and building the systems, processes, relationships, and team capabilities required to secure sustainable long-term funding.
Working closely with the Chief of Engagement and Strategic Partnerships, senior leadership, fundraising team, Trustees, and National Partners, the Head of Fundraising will oversee the development of a diversified funding portfolio and will be responsible for building and managing a robust fundraising pipeline, strengthening donor stewardship, improving cost recovery, and ensuring fundraising activity is aligned with organisational priorities and financial sustainability goals.
This role requires a proactive fundraiser who can originate opportunities, open senior relationships, develop compelling narratives, lead complex proposal processes, and create a culture of shared fundraising responsibility across the organization.
Key Responsibilities
Business development, Prospecting and Pipeline Conversion
Build and maintain a prioritized list of top institutional, philanthropic, bilateral, multilateral, DFI, corporate, and family-office, and high-net-worth prospects aligned with GSG Impact's strategic priorities.
Proactively identify, research, qualify, and cultivate new prospects, with a particular focus on funders aligned with impact economies, domestic capital mobilisation, climate adaptation and resilience, impact transparency, policy reform, investment vehicles, and emerging markets.
Develop and manage a rolling pipeline of high-quality funding opportunities, moving prospects from initial identification through cultivation, concept development, proposal submission, negotiation, and grant close in collaboration with programme staff and National Partners.
Translate GSG Impact’s strategy into compelling fundable propositions, including unrestricted/core support, restricted programme grants, regional funding, National Partner support, and special initiatives.
Personally lead the development and conversion of the highest-value opportunities, especially prospects requiring senior-level cultivation.
Identify and develop new revenue opportunities, strategic partnerships, and funding models that support GSG Impact's long-term sustainability and income diversification
Proposal Development and Grant Acquisition
Lead the development of high-quality funding proposals, concept notes, and donor engagement materials.
Ensure strategic alignment of all proposals with organisational priorities and donor interests.
Ensure all proposal budgets meet or exceed GSG Impact's cost-recovery targets
Set and ensure implementation of standards for proposal quality, narrative framing, and budget methodology across the fundraising team
Donor Management and Stewardship
Support the Chief of Engagement and Partnerships with relationship management of GSG Impact's most significant donors and strategic prospects
Lead the annual stewardship strategy, ensuring renewal and growth of key funding relationships
Oversee narrative and financial reporting, ensuring high quality, consistency, and timeliness
Functional Leadership
Manage the fundraising team, providing coaching, oversight, performance management, and professional development
Ensure compliance with Fundraising Regulator, GDPR, and relevant UK legislation
In collaboration with colleagues across the organisation monitor and update the information in CRM for fundraising contacts and other relevant information.
Implement strong financial tracking, reporting, and forecasting processes
Foster a culture of fundraising responsibility across GSG Impact teams

External Representation
The Head of Fundraising plays a key role in relationship management with GSG Impact's most significant funders and prospects. They represent the organization at key sector events and convenings as delegated by the Chief of Engagement and Partnerships.
Qualifications
Demonstrable track record of originating new donor relationships and converting them into six- or seven-figure grants.
Experience in building prospect pipelines from a limited starting base.
Strong understanding of institutional philanthropy, bilateral and multilateral funding, DFIs, and/or impact investing funders.
Proven ability to develop fundable propositions from complex, technical, organisational strategies, including on policy, systems-change, market-building, or ecosystem-development initiatives
Experience working directly with CEOs, Boards, Trustees, or senior principals on donor cultivation.
Strong commercial discipline: pipeline management, probability weighting, forecasting, and cost recovery.
Ability to write or lead the development of high-quality proposals under tight timelines.
