Responsibilities
· Create a safe and accepting psychologically informed environment for vulnerable residents, whilst following safeguarding measures.
· Develop the recovery hub, embedding the “Rethink Recovery Toolkit”
· Deliver quality training, advising staff and residents on substance misuse and assist on the admin side of negotiations and conferences.
· Line manage a small number of staff
· Attend service user and care programme approach review meetings.
· Create trusting relationships with residents, motivating and empowering them to take next steps.
· Interview new referrals and recovery workers, ensuring your project meets their needs, and assessing physical and mental health for interventions.
· Develop links and effectively communicate with internal and external partners / connected organisations, providing covering when necessary.
· Organise peer support activities, assisting with money management following Finance policies and procedures.
· Ensure residents are assisted with managing their medication, strictly adhering to PRHA’s Medication policy.

Recovery Worker
We're on the lookout for a dedicated and empathetic professional to join our team as a Recovery Worker. The perfect fit for this role will have a genuine passion for assisting individuals battling substance misuse disorders, and a strong commitment to positively impacting their lives. Responsibilities:
- Deliver individual and group counselling sessions to clients grappling with substance misuse disorders.
- Conduct comprehensive assessments to identify the unique needs and objectives of each client.
- Collaborate with clients to develop tailored treatment plans, taking into account their specific circumstances.
- Lead psychoeducational workshops and support groups covering topics like relapse prevention, coping mechanisms, and life skills enhancement.

Experience Required:

- Knowledge of evidence-based practices in addiction treatment and recovery.

East London Apartments provides 24 hour accommodation based support for up to 31 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness as well as individuals who have been diagnosed with severe and enduring mental health conditions who are under the Care Programme Approach (CPA).
6 week rolling rota with early, late and some weekend shifts.

Are you interested in supporting women at immediate risk of rough sleeping and making a real difference to the lives of women who are homeless?
This is a 6 month fixed term contract role.
The Westminster Women’s Safe Space is a specialist gender and trauma informed Assessment Centre for women with complex needs who are at risk of, or have experienced domestic abuse, violence against women and girls (VAWG); and have multiple disadvantage. We have a great opportunity for people interested in working as part of an innovate women’s only service in an Assessment and Reconnection Worker role and becoming part of this exciting team achieving positive outcomes to support the women in their recovery and move on journey.
In this role you will:
Deliver support to female clients with multiple support needs who have also experienced domestic violence and VAWG (Violence Against Women and Girls).
You will be carrying out comprehensive client assessments; providing clients with personalised case management support to help them be connected into specialist external services, turning assessments into a practical action plan which fully involves the client.
Working within a small staff team, you will work closely with the Case Coordinator to case work the clients, assessing new referrals, welcoming them into the service, and planning their moves on from the service to more appropriate accommodation.
You will work a rota that will involve some evening and weekend work. This role will pay unsocial hours.
About you
You will have experience of working with vulnerable people and an understanding of the needs of homeless women. Strong communication, influencing and reasoning skills are also key, as is a flair for solving problems. Other qualities include knowing how to network and build effective relationships with people at every level, and a keen interest to keep abreast of current DV and VAWG related issues, housing legislation, welfare benefit legislations and other matters relevant to successful support and move on of clients.

About the role:
The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.
Main duties:
- To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
- Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
- Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
- Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.
General responsibilities:
- In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
- To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
- To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
- To ensure that all The Passage policies and procedures are being adhered to, particularly those relating to Code of Practice and Confidentiality.
- To contribute to the effective implementation of The Passage’s Diversity and Equality Policy as it affects both The Passage and its work with vulnerable adults.

The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. IBIS House MH Step Down & Crisis
Working in partnership with the London Borough of Newham and East London Foundation Trust, IBIS House supports people who are medically fit for discharge from mental health inpatient stays but have other barriers to discharge. These may include housing, living skills, accessing benefits or the community, or other interventions and will be on a person-centred basis. The main aim of the service is to provide short-term support to customers to link with longer-term services to reduce escalations in need, and reduce inpatient readmission and reduce pressure on inpatient beds.
Shifts will be on a rota pattern and include morning shifts (08:00-16:00) and afternoon shifts (14:00-22:00).

Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. The SHP Somerset Cluster is based in the heart of the London’s theatre and shopping districts – the main hub is just off Drury Lane, with the other sites near Oxford Street and Tottenham Court Road. The aim of the service is to support people over the age of 18 to navigate various support needs and develop the skills required to maintain independent accommodation long term.
As a multiple disadvantage (MD) referral and move on coordinator, you will work as part of a larger support team to ensure that consideration is given to the individual experiences of multiple disadvantage when it comes to accessing and moving on from our service.
You will work with people from their first interaction with the service: at the point of referral, ensuring that all aspects of someone’s experience is considered by undertaking a trauma informed, culturally sensitive assessment and supporting your colleagues to develop their awareness of these experiences.
From here you will then support their journey through the service by working alongside other support staff to foster a culture of move on, supporting the development of holistic and realistic move on goals.
This work will be underpinned and supported through the development of a move on programme.
