The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.

South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.

The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.

You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

As an Assessment and Response Worker you will:

• Triage online referrals from the website and the app, to connect people who are rough sleeping with local outreach teams as quickly as possible.

• Carry out comprehensive telephone assessments, identifying immediate options for accommodation and support while confidently responding to emergency situations and safeguarding concerns.

• Advocate on behalf of people with relevant statutory and non-statutory services to ensure clients can access the support and accommodation they are entitled to as quickly as possible.

The Minorities Community Gambling Link Worker will deliver an awareness-raising and personalised, holistic support programme for minority community groups in Lambeth experiencing gambling and gambling addiction-related harm, or those impacted by the gambling of a close family member. You will:

• Deliver an outreach programme by regularly attending community events, visiting local groups/ spaces and organisations frequented by minority community groups in Lambeth.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.

The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.  

You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

As project manager, you will oversee the day-to-day operation and delivery of this vital  service, using your experience to develop the team, sharing best practice and seeking creative opportunities to continually improve the support we offer unpaid carers. You’ll develop trusted partnerships with local stakeholders, practitioners, businesses and services to promote and embed the service in the local area, supporting the development of a positive ‘carer friendly’ culture in Lewisham.

This role will suit a people person who can quickly develop relationships, lead a team, and work closely in a supportive role with Adult Carers, Young Adult Carers and Young Carers. You’ll have overall responsibility for the Carer’s hub, where carers can drop in for advice and support, manage a small caseload, and support the delivery of the service’s range of activities as required, including respite activities, online groups, face-to-face sessions.

We are looking for a great motivator with excellent communication and organisational skills. Applicants should have relevant team or project management experience in social care, health, education or the voluntary and community sector. You will have experience of working with vulnerable adults and/or children and operational knowledge of multi-agency working. A practical understanding of barriers faced by unpaid carers, and an awareness of cultural differences will be key in this role.

This is a full-time role and the postholder can either be based in the community in Lewisham or our New Ash Green office, with travel to the other area/ office 2 days a week. The postholder must have access to their own ULEZ compliant car.

We are seeking an Advanced Recovery Practitioner to support in the Women and Families team, within Mind in the City, Hackney and Waltham Forest, working in partnership with Turning Point and Antidote.  The successful candidate must have demonstrable experience in working within a substance misuse treatment and recovery setting, must have specialist training and supervision in one or more complexities alongside substance misuse. This may be mental health, long term conditions, criminal justice, offending behaviour, homelessness, motivational interviewing, safeguarding, family work.  Have an understanding of risk and risk management, as it applies to substance misuse and complexity.  Have a working knowledge of Safeguarding of Vulnerable Adults framework, the ability to follow procedure and development of a culture of safe practice.  Ability to work under pressure and have significant experience of working in health and social care.
