Sheffield Futures is delivering an innovative Young People’s Social Prescribing Service. This service aims to support young people with a wide range of health and wellbeing needs and to improve access to appropriate activities and emotional wellbeing support.
We are looking for experienced and self-motivated individuals to work as part of the Door43 Health and Wellbeing Service at Sheffield Futures.
The Role
The post holders will implement and deliver a Young People’s Social Prescribing service, working alongside GPs, Door43 and other primary care health professionals to identify, and engage with people aged 13-25. The post holder will build a database of social prescription activities and co-develop bespoke support plans with young people to engage them in these social prescriptions, to ultimately improve wellbeing and independence and self-management. The role will be based in GP surgeries and community settings, with requirements to travel, therefore holding a full driver’s licence is desirable. Evidenced experience working with young people with emotional wellbeing issues, or in a health and social care setting is essential.
The Candidate
The successful candidates will be Qualified to Level 3 or above in youth, health, social care, counselling or other related discipline. The candidate will also have experience of engaging and working with young people, excellent holistic assessment skills and experience of providing empowering support to young people to improve health, recovery, and wellbeing outcomes.
This role requires an enhanced DBS check.

When the Coronavirus Job Retention Scheme and other Covid-19 related protections for households eventually come to an end, there is expected to be a significant rise in financial difficulties and debt problems, especially amongst already vulnerable people and people with long term health conditions.
To help address this Citizens Advice Southwark is working with the local Primary Care Networks to create a healthcare pathway allowing general practice to identify local residents that would benefit from pro-active and targeted money advice and financial support.
To deliver this exciting new project we are looking for 4 full time generalist advisers who will be based at our offices and at GP surgeries in Bermondsey and Peckham and will work closely with Social Prescribing Link Workers employed by South and North Southwark Primary Care Networks.
In this role you will take referrals from local GPs surgeries and other services, and provide 1:1 advice and support to people with complex needs, developing personalised plans to tackle their debt and financial liabilities.
To undertake this role you will need:
- At least six months recent experience of providing Generalist Advice to the public including casework in welfare rights, housing and debt.
- Proven ability to interview clients using sensitive listening and questioning skills to get to the root of issues and empower clients in a non-judgmental way.
- An understanding of the financial challenges facing citizens, especially as a result of COVID.
- Ability to work closely with and develop relationships with a wide range of stakeholders across organisations and sectors, including health and voluntary and community sector.
- Ability to commit to and work within the aims and principles of Citizens Advice Southwark.

Main Duties & Responsibilities:
- Provide advice and support to asylum seekers, within the specification of the Grant Agreement and Contract with the Home Office
- Provide a support service to asylum seekers to complete asylum support application forms (ASF) for support under Section 95 and Section4 of the Immigration and Asylum Act 1999
- Ensure that all applicants are aware of eligibility criteria and their obligations to provide supporting evidence within required timescales
- Provide outreach for vulnerable clients and those with additional support needs, by appointment, at designated locations, as required by the Regional Operations Manager
- Provide telephone advice as may be required and support the telephone advice line by assisting clients to complete asylum support and Section 4 applications by telephone and by appointment
- Assist clients to notify the authority of prescribed change of circumstances
- Provide guidance on immigration related matters advice within the scope of the relevant Office of the Immigration Services Commissioner (OISC) accreditation level
- Provide advice on general matters e.g. sign-posting
- Provide advice on Voluntary Assisted Return
- Liaise and cooperate with relevant statutory and voluntary agencies on behalf of service users as necessary
- Act as a contact with the Home Office, Social Services, Healthcare Professionals and other relevant agencies for applicants with additional support needs.
- Deliver key touch point briefings to a high standard so that applicants fully understand the information provided. Ensure that information books are given out accordingly.
- Provide information to service users in a timely manner, being aware of other appointments, dispersal date.
- Make contact with dispersal accommodation providers and other relevant agencies, providing letters as appropriate, to ensure vulnerable clients are appropriately supported in their dispersal area.
- Link Asylum Seekers and Failed Asylum Seekers with other relevant organisations

- Participate in rota duties on a planned or ad hoc basis as may be required to cover telephone advice line out of hours service

- Participate in the Performance and Development Review (PADR) appraisal process
- Participate in all mandatory training and training identified in the PADR process
