The Croydon Health and Wellbeing Space Support Workers will provide advice, information, support and representation to Croydon residents who are referred to the Space through the local Integrated Care Network Plus multi-disciplinary clinical and community mental health support teams (MDTs) based in the local Mental Health and Wellbeing Spaces in North, Central/East Croydon and/or South Croydon.
In addition to casework, the role will be expected to assist with reception and initial referrals in the Space when required and will also support Space volunteers. The Health and Wellbeing Space team will ensure that support is person-centred; focussed on supporting recovery; that needs, and wishes are identified by people themselves; that they have a stronger voice in relation to issues that affect their lives; and that they are supported and empowered to improve and/or maintain their mental health and wellbeing and to live their best independent lives.
The multi-disciplinary team during the initial one-year test period will inform the development and growth of the Health and Wellbeing Space to meet local needs and to ensure that it can support community-based mental health and wellbeing across the whole population and in all 6 Integrated Care Network Plus localities by 2023.
The role is an integral part of the ICN+ which brings together clinical and community-based support and services in co-located teams based in the Health and Wellbeing Spaces. We are looking for a Social Prescribing Link Worker to work with adults in Sutton, supporting them to find lasting solutions to social issues and personal challenges.
Social Prescribing offers patients attending Primary Care (e.g. their GP) about non health-related issues, personalised support to set goals, connect with local services, and improve their quality of life. This service is based at Age UK Sutton as we are the lead contract holder, but is for adults of all ages, and involves extensive partnership work with the voluntary and community sector in Sutton.
You will have some experience in a relevant role, and understand the way health and social needs can interact. Good communication, research, and record keeping skills are essential. You will have access to training, clinical supervision, and regular support meetings with your peers in the service including specialist nurses, GPs, and other Link Workers.

Age UK Sutton is the hosting organisation for the London Borough of Sutton’s all adults Social Prescribing programme, working with Sutton GPs, the Sutton GP Federation and Sutton Health and Care programme.
Social Prescribing Sutton is commissioned jointly by Sutton’s four Primary Care Networks and operates as an integral part of Sutton’s integrated health and care services.
Age UK Sutton is the employer but the work in this role will encompass all adults as Social Prescribing is for the whole population (18+). Purpose of the role
The Social Prescribing service will be delivered by Community Link Workers working from GP surgeries in Primary Care Network (PCN) areas. The Community Link Worker will be directly employed by Buckinghamshire Mind.
Social prescribing empowers people to take control of their health and wellbeing through GP referral to non-medical Community Link Workers (CLW’s) based in GP surgeries who give time, focus on ‘what matters to me’ and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Community Link Workers work collaboratively with all local partners.
Social prescribing can help to strengthen community resilience and personal resilience and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing, and physical inactivity, by increasing people’s active involvement with their local communities. It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.
Key Responsibilities
- Take referrals from GP practices within a primary care network.
- Provide personalised, holistic support to individuals, their families, and carers to take control of their wellbeing, live independently and improve their health outcomes.
- Develop trusting relationships by giving people time and attention to their priorities and ‘what matters to me’.
- Co-produce a personalised support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
- Managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload.
- To refer people back to other health professionals/ agencies when the person’s needs are beyond the scope of the Community Link Worker role.
Key Tasks: Referrals
- Promoting social prescribing to patients; its role in improving health outcomes, reducing social isolation and the ability to remain independent in one’s own home for longer.
- Build relationships with key staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing.
- Be proactive in developing strong links with GP surgery personnel to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals.
- Work in partnership with all local agencies to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care.
- Provide referrers with regular updates about social prescribing, including information on the types of people most likely to benefit from the service, how to refer and outcomes of referrals
- Seek regular feedback about the quality of service and impact of social prescribing on referrers.
- Work in close collaboration with the Prevention Matters service to maximise the availability of social prescribing across the county whilst ensuring there is no duplication of resources.
Provide personalised support
- Meet patients on a one-to-one basis, making home visits or using digital technology such as Skype where appropriate. Give them time to tell their stories and focus on ‘what matters to me’. Build trust, providing non- judgemental support, respecting diversity, and lifestyle choices. Work from a strength-based approach focusing on a patient’s assets.
- Be a friendly source of information about wellbeing and prevention approaches.
- Help patients identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.
- Work with the patient, their families and carers and consider how they can all be supported through social prescribing.
- Help people maintain or regain independence through developing skills, adaptations, enablement approaches and simple safeguards.
- Work with patients to co-produce a simple personalised support plan – based on their priorities, interests, values, and motivations – including what they can expect from the groups, activities, and services they are being connected to and what the person can do for themselves to improve their health and wellbeing.
- Where appropriate, physically introduce patients to community groups, activities, and statutory services, ensuring they are comfortable. Follow up to ensure they are happy, able to engage, included and receiving good support.
- Where patients may be eligible for a personal health budget, help them to explore this option as a way of providing funded, personalised support to be independent, including helping people to gain skills for meaningful employment, where appropriate.
Support community groups and VCSE organisations.
- Develop supportive relationships with local Voluntary, Community, Social Enterprise groups/organisations and statutory services, to make timely, appropriate, and supported referrals for the patient being introduced.
- Work with commissioners and local partners to identify unmet needs within the community and gaps in community provision.
- Support local partners and commissioners to develop new groups and services where needed, through small grants for community groups, micro-commissioning, and development support.
- Encourage patients who have been connected to community support through social prescribing to volunteer and give their time freely to others, in order to build their skills and confidence, and strengthen community resilience.
- Encourage patients, their families, and carers to provide peer support and to do things together, such as setting up new community groups or volunteering.
General tasks: Data capture
- Work sensitively with patients, their families, and carers to capture key information, enabling tracking of the impact of social prescribing on their health and wellbeing.
- Encourage patients, their families, and carers to provide feedback and to share their stories about the impact of social prescribing on their lives.
- Support referral agencies to provide appropriate information about the patient they are referring. Use the case management system to track the patient’s progress. Provide appropriate feedback to referral agencies about the patients they referred.
- Work closely with GP practices within the PCN to ensure that social prescribing referral codes are inputted to EMIS and that the patient’s use of the NHS can be tracked, adhering to data protection legislation and data sharing agreements with the clinical commissioning group (CCG).

- Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the roles and responsibilities.
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
- Work with your line manager to access regular ‘clinical supervision’, to enable you to deal effectively with the difficult issues that patients present.
- Attend team meetings with other Community Link Workers, organised by the Team Manager.

In Bracknell Forest, we are about to launch the Green Volunteering scheme.
In this role, you will promote and enable volunteering across Bracknell Forest, administering and maintaining the online volunteer portal whilst helping individuals and groups to find opportunities that are right for them. In addition, you will support the development of those community groups, charities and social enterprises that offer services within the borough, deputising for the Chief Executive Officer and/or Operations Manager for business matters that associate to our Bracknell Forest delivery.
Core Tasks and Responsibilities
- Administer and manage the online volunteer portal ‘Bracknell Forest Get Involved’
- Engage with and support all individuals and groups interested in volunteering locally, helping to source opportunity in line with interest and need.
- Advertise and promote the Volunteer Centre to the public, third sector, and corporate partners, marketing through various platforms including social media, Involves website, leaflets, and events.
- Engage with charities and community groups to develop their understanding of volunteering, encouraging them to make best use of volunteers for the furtherance of their charitable objective and to increase reach.
- Proactively recruit volunteers from older and younger age groups, as well as people with additional needs and those with protected characteristics.
- Develop and source volunteering options that are accessible to younger people.
- Identify and promote national volunteering campaigns in line with local need.
- Promote good volunteer management, offering those organisations and groups that use volunteers best practice guidance.
- Promote the Bracknell Forest Volunteer Pool, encouraging sign up from individuals who would like to volunteer but are unable to do this routinely.
- Engage with schools and colleges to encourage and promote the volunteering and citizenship of children and young people.
- Manage telephone, email, and face to face volunteering enquiries, signposting and matching people to opportunities in accordance with need and interest.
- Host and attend events that encourage local volunteering.

- Proactively gather feedback from both those who volunteer and those services using volunteers so to measure retention success and placement satisfaction.

- Liaise and network with fellow local volunteer coordinators and volunteer centres, so to share best practice and develop new ideas.
- Meet the requirements set by the Volunteer Centre commissioner, ensuring that necessary data and statistics are available and accurately recorded.
- Champion and adhere to any legal or regulatory obligations regards volunteering.
- Assist and nurture the growth of new community groups, charities, and social enterprises across Bracknell Forest in tandem with other involve services.

The post-holder will work with patients to plan the support they need to be discharged home; to support voluntary, community and social enterprise organisations to respond to the needs of people returning home from hospital; and to raise awareness of the wide range of assets available in the VCSE sector that enable people to self–care or provide short-term support.
The Home First Support Worker will use a person-centred approach to support individuals preparing for discharge or recently discharged from Salford Royal Hospital who have been identified as needing additional support within their home and community. The post-holder will work with individuals and families as part of a Home First Community Hub multi-disciplinary team, as well as within the Salford CVS team.
The Home First Support Worker will build strong working relationships across the Home First multi-disciplinary team (MDT) and raise awareness of the wide range of activities and services available in the voluntary, community and social enterprise sector (VCSE) within the Home First Hub as well as the wider Salford Royal Hospital staff team. The post-holder will work pro-actively in partnership with a range of statutory and VCSE colleagues in the Home First MDT to ensure that the needs of individual patients are addressed wherever possible.
The Home First Support Worker will also work with the wider CVS team to map current community provision of activities or services (Age UK Salford, Helping Hands, and the Carers Service, for example) offering support for people to return home and become re-engaged within their community. They will also support a range of VCSE groups / organisations to develop services and activities that meet the needs of individuals.
The Home First Support Worker will be aligned to Salford’s flagship Wellbeing Matters programme, a partnership between Salford CVS and Salford Third Sector Consortium, and draw on existing knowledge from within the Community Connector and Development staff teams from the programme, working closely with that wider team at all times.

Hertfordshire's NHS has commissioned "Link Workers" throughout the county to work closely with GP surgeries supporting patients to find non-clinical solutions to improve their wellbeing. Link Workers will work intensively with a small number of individuals, enabling them to access support within their local community.
You will be responsible for linking patients to services across the Health, Statutory and Voluntary sectors. You will work alongside patients to help them achieve their goals, undertaking assessments and building relationships to agree personalised plans to help the patient make the necessary changes to live the life they choose. You will work closely with GPs, other health and social care professionals, and local voluntary organisations.
You may have worked in a similar role previously - but equally this role will be suited to someone with good local knowledge, appropriate experience and a passion for personalised care. The roles will be based at one or more local GP surgeries and we will pay travel expenses within the geographical area covered. Applicants must be licenced drivers with access to a vehicle. An enhanced DBS check will be required.
The role
We are recruiting Link Workers to work with people who are frail or vulnerable or unwell, or people who are in danger of losing their independence. Link Workers will receive referrals from GP surgeries. Other partners may join.
You will be assigned to work with one or more GP surgeries and play a key role within their team. You will also be part of a locality team comprising other social prescribers led by a Senior Navigator.
You will need to operate independently and as part of several different teams, each providing support and supervision. You will use case management databases and communications systems, understand safeguarding and privacy rules, and provide high quality information, input and feedback.

The Role
As an Advice Centre Manager your ability is to ensure that each surgery delivers high quality person centred advice and support to clients and the public that could have a real impact on people’s lives.
The role involves working from all 3 current surgeries during the week.

If successful, the main duties of your role will be to:
- Manage the practicalities of the advice sessions and ensure adequate staffing and resources.
- Provide an appropriate level of support and supervision to individual workers (both paid and voluntary) depending on their level of competence.
