Purpose of the Job:

This role is within our crisis alternative service – Safe Space - which is an out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 12:00pm(midday)-11:30pm across all of our boroughs. The shift pattern for this role is 4:30-11:30pm. This will be on a rota basis across support workers.

Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health crisis. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate and prevent further crises.

The objectives of the service include:

• To improve the mental wellbeing of people experiencing mental health crisis in HFEH.
• To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To treat service users with respect, dignity and personalised support
• To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
• To increase self-management skills of those accessing the service
• To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
• To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.

Key Responsibilities:

• Providing a person centred and recovery orientated approach in all aspects of the roles and responsibilities.
• Promoting people’ rights and responsibilities
• Working collaboratively with clients to understand their needs and developing flexible and realistic crisis support packages/person centred plans
• Listening to clients and encouraging positive steps towards self-management of crisis and recovery
• Providing advice, information, practical and emotional support to clients

You will have:

• Minimum of 1 year working in mental health services and with clients experiencing mental health distress and crisis

We’re looking for passionate and dedicated Project Worker to join our Complex Needs Services in Camden. In this role, you will be part of a dynamic team providing tailored support to clients with multiple and complex needs, including mental health challenges, substance use, offending behaviour, and physical health issues. • Conducting needs and risk assessments, and work collaboratively to develop and implement support plans.

• Supporting clients to develop life skills, manage relationships, build social networks, and access external services and community resources.
• Facilitating group activities and structured interventions as part of our in-house recovery programme.

About the Role:

As a Specialist Multiple Disadvantage Worker, you will be responsible for delivering targeted, person-centred interventions for individuals within SHP’s Camden Adult Pathway Hostels. Your role will involve holding a designated caseload and working closely with the wider client group through our in-house recovery programme. With a strong focus on harm minimisation, dual diagnosis, and strengths/recovery frameworks, you’ll help to empower service users to make positive changes and progress towards independence.

This is a unique role that requires you to build relationships of trust with service users, many of whom have faced significant barriers such as mental health challenges, substance use, and complex trauma. Your support will enable them to access crucial assessments, interventions, and treatments, enhancing their ability to utilise external statutory and community resources. You will also provide guidance and best practice standards to Project Workers, helping to set a benchmark of excellence in delivering complex needs support.

Key Responsibilities:

• Provide specialist interventions and personalised support for service users, including those with complex needs such as mental health issues, substance use, and physical health challenges.
• Develop and implement person-centred support plans, monitor individual progress, and ensure best practice is adhered to within the service.

• A strong understanding of the health and social care sector, including how to navigate between and across specialties. A deep understanding of the barriers service users face on their journey towards independence, such as domestic violence, anger management, and learning impairments.
• Excellent person-centred engagement skills, group work facilitation, and the ability to manage complex caseloads. Strong team-working skills with the ability to collaborate effectively with other professionals.

Our mission is to help children and young people have a fair chance to be who they want to be. We do this by providing a safe home, increasing life skills and self-confidence, and improving emotional wellbeing and mental health.

Our Brighton & Hove Services provide supported accommodation for 50 young people at risk of homelessness across two sites. Gareth Stacey House is one of our 24-hour supported housing projects offering high and medium levels of housing-related support for young people aged of 16-25 with 15 bedspaces and shared communal facilities. We provide support to our residents to help them build essential life skills, identify their goals and work towards aspirations so that they can move on to enjoy fully independent futures. Our staff team comprises of Support Workers, Night Staff and Managers. Each Support Worker holds a caseload of residents and meets with them weekly to build a support plan and help them achieve their goals.

As a Supported Housing Support Worker you will hold a caseload of residents, working with them on a one-to-one basis with also facilitating group work sessions. You will identify their needs by listening to them, with employing psychologically informed approaches to enable residents to make better choices and take responsibility for their actions. You will work closely with other services such as, Social Services, Police, Probation, education providers and local organisations. Princess Court is a mental health, supported housing, 24 hour supporting housing and community-based service, working to a recovery model. We provide a range of support services for people with mental ill health to access in their local community, including outreach support, peer support and supported housing. Our Peer Support Service started 10 years ago, with 2 Wellbeing Centres offering this new service. Since then, we have seen our service grow and develop in ways we could not have imagined. This service is the key service most requested by people and we now offer Peer Support across all of our Wellbeing Centres across Hertfordshire.

Working in co-production with clients, staff, volunteers and stakeholders, we were able to create our own Peer Support Charter Values (this can be found on our website). These values form the ethos we work towards within our peer support team. These values also complement HMN’s values of Hope, Courage, Togetherness and Responsive, which form the basis of all the work we do as an organisation. 

About the Role

Working with the team to offer 1-1 peer support to anyone accessing HMN. The Peer Support service provide tailored support for individuals to support their needs. This includes using different tools and techniques to help an individual improve areas of their life they may be struggling with. Having a lived experience of mental health issues or supporting someone who has is a key element to what makes this service so unique.

The Peer Support approach includes empathy, warmth, acceptance, authenticity and compassion. Peer Support is based on the recognition that there is no better person to support the path towards recovery, than someone who has walked a similar path as that person.

To meet client need, and work to this personalised approach, we do support people out in the community, within their home, as well as in one of our Wellbeing Centres. Being able to drive and having access to your own vehicle (or equivalent) is essential for this role.

The successful candidate will need a personal understanding of mental ill health, either from your own lived experience or by caring for someone with a mental health condition and have awareness of issues in mental health service provision.

Peer Support Workers will also provide support within our Peer Support Groups, Peer Support Learning Sets and Peer Support Academy when needed.

What you’ll be doing? 

This role has been developed to work within a specific Lambeth Primary Care Network (PCN) to support their older population access the right support and services that they need to live and age well. This is a new post, and the successful candidate will be key to ensuring the development of new partnerships and local relationships to support Lambeth’s ambitions to ensure older residents can live and age well. Your main responsibilities will be:

• Undertake mapping of the local community, using existing Age UK Lambeth knowledge and resources and creating new tools for this project to identify services and support available to older adults.

• Focus mapping exercise on identifying community interventions that will address health inequalities and improve wellbeing for older adults. 

• Using Age UK Lambeth Guides as a template, develop a specific, local guide/ directory for this project. 

• Organise, facilitate and deliver training sessions to PCN staff on how to use the guide, developing and building their understanding on local resources and provision in the local area.

We are recruiting for a Duty Worker to join our team in Lambeth (The Gaia Centre), London.

Job Title: Duty Worker

Salary: £24,883.20 per annum, including London weighting if applicable
Contract type: Permanent, Part Time 

Hours: 30 hours per week. Operating hours of the service 8AM- 6PM Monday- Friday with the expectation to work on some days 8- 4pm or 10- 6pm to cover

The Duty Worker will be working closely with survivors of domestic abuse and other VAWG (violence against women and girls) to provide high quality independent support to survivors at all risk levels and their children. The post holder will play a crucial role in the service delivery, being the first point of contact for survivors who are referred to the Gaia Centre by contacting the survivors, assessing their needs and risks, carrying out and implementing safety plans and needs and will ensure an effective handover to the relevant GAIA team for ongoing support or referring to another appropriate agency for support.

The duty worker will also empower survivors by providing them with emotional, practical, and personal welfare support. You will ensure that survivors are provided with a safe, supportive environment, enabling them to access their rights, make decisions and increase their life options.

The job involves working within a multi-agency framework consisting of the MARAC and local partnership protocols and procedures that prioritise the safety of survivors. The job involves informing survivors of the full range of civil, criminal and practical options that might increase their safety.

TLC: Talk Listen, Change is seeking an engagement worker to conduct individual sessions and group work with adults experiencing homelessness who are seeking to change their harmful behaviours. In this role, you will be part of a broader partnership that delivers a range of interventions to support individuals, families, and the wider community. Your work will focus on helping clients reduce harmful behaviours and work towards safer, healthier lifestyles, and ensure clients are supported for other complex needs within a partnership approach.

With the support of senior staff, you’ll implement effective risk management strategies and serve as a mentor, delivering targeted programme materials focused on harm reduction, early intervention, and behaviour modification in both individual and group settings. This role will be to support the perpetrator intervention across Wiltshire.

You will be primarily based in Wiltshire supporting the Domestic Abuse Serial Perpetrator (DASP) project, providing bespoke specialist support to perpetrators of domestic abuse. The post holder will work as part of the IOM team within a dynamic, fast paced, crisis intervention, advocacy and support service to ensure that clients who show abusive behaviour are supported through every stage of the process of change. Evening appointments are necessary for this post in Salisbury and Chippenham.

In addition, you will support the delivery of the CPR (Choosing Positive Relationships) Domestic Violence Perpetrator Programme (DVPP), which is an intensive behaviour change programme, designed to increase knowledge and understanding around domestic abuse, the impact on children and healthy relationships. The course teaches tools and strategies to help someone change their behaviour. The CPR group will require weekly evening work in Wiltshire.

The post-holder will be expected to work to Respect standards.

Full training will be available for this role. 

Client Based Duties

• Identify and assess the risks and needs of individuals who are abusive in a relationship or other domestic setting, using an evidence-based risk identification checklist.
• Develop personalised risk assessments and safety plans for each service user.
• Advocate for individuals who wish to address their abusive behaviour and show motivation for change with agencies who can help to address the domestic abuse by:
  • Understanding the role of all relevant statutory and non-statutory services available to those who are abusive and how your role fits into them.
  • Providing advocacy, emotional and practical support and information to individuals, including in relation to drug and alcohol, housing, health and finance.
  • Working directly with all key agency partners to address the impact of the abusive behaviour on the safety of high risk victims and ensuring that their safety plans are coordinated.
• Support the client to recognise the abusive behaviour and the effect that it has on their families and assist them in recognising the features and dynamics of domestic abuse present in their situation and help them change unhealthy patterns of behaviour.
• Understand multi-agency partnership structures and work within a multi-agency setting which may include participation at the MAPPA/MARAC.  You will contribute interventions and help design a plan to protect victims and any children, while maintaining an independent role on behalf of your client, keeping the victim and any children’s safety as central to any response. 
• Ensure support provided is accessible to clients in terms of location and times.
• Be proactive with your line manager in carrying out regular case reviews based on a review of risk and abuse which:
  • Feeds back into action planning to further progress, signpost or close cases and;
  • Provides feedback to your clients/agencies.
• Help maintain accurate and confidential case management records and databases and contribute to monitoring information for the service.
• Respect and value the diversity of the community in which the services works in, and recognise the needs and concerns of a diverse range of survivors ensuring the service is accessible to all.  
• Remain up-to-date and compliant with all organisational procedures policies and professional codes of conduct and uphold standards of best practice.
• Utilizs evaluation and monitoring systems to ensure high standards of service are consistently achieved e.g. Orchards Database.
• Deliver high-quality group work, working in co-operation with your co-facilitator and following a clear manual.
• Have an appropriate attention to data, providing feedback to CPR , your line manager, FearFree and commissioners.
• Work at all times to Respect standards.
• Work closely with other FearFree staff to ensure appropriate support and attention is given to the victim’s needs and those of any children involved.
• Respond proactively to safeguarding concerns, working in partnership with Children’s and Adult’s Social Care.

Working with other agencies

• Work alongside other project partners to ensure that all elements of the project are incorporated into working practices.
• Work in a multi-agency framework to support perpetrators to change their behaviour and help victims and families access relevant support.
• Participate and prepare reports
• Prepare reports for FearFree management as requested.
• Comply with data protection legislation, confidentiality and information sharing policy and procedures and all legislation connected to your work.
• Support colleagues and partner agencies, through awareness raising and institutional advocacy, in order to provide the best possible service for those who are abusive within a family setting.

General

• Work at all times in accordance with the requirements of the Lone Working Policy and Procedure.
