About this role…

Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. Staffed 24 hours to ensure this quick turnaround, they are a key service to ensure people are off the streets and in stable accommodation.

We are looking for a Night Support Worker to join both the brand new Snow Hill Court team and the City Inn Assessment Service team.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.

• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Work alongside senior colleagues to identify the fastest and most appropriate route away from the streets for the people that we are working with

As a Outreach Support Worker, you will provide a service where planned support and crisis intervention is delivered to vulnerable people with low, medium and high support needs living in independent accommodation with the aim of them maintaining their home.

In conjunction with colleagues and Senior Managers, you will be responsible for the effective operation of the Support Service, in line with its policies and procedures, including encouraging a co-operative and supportive environment within the Service, listening to customers’ views and developing new services to meet these needs.

Some of the key results for the role include: 

• To provide pre tenancy guidance and ongoing support to customers on all tenancy matters, including assistance to furnish and move into properties.
• To enable customers to abide by their tenancy agreements, giving advice and assistance on dealing with police and courts where this relates to housing matters
• To provide basic Welfare Benefits advice, assisting customers in claiming all benefit entitlements and assisting with accessing specialist support. 
• To work with customers to identify the skills required to manage their own tenancy, including assistance to deal with difficult situations.

The rota for the post is earlies (8 am - 4 pm) and lates (2.30pm - 10.30pm).

This role is home-based with egular travel to our office in Plymouth & throughout Devon and Cornwall. As a Domestic Abuse Caseworker you will:

• Develop, deliver and monitor safety and support plans for those affected by crime (primarily domestic abuse) who accept ongoing support which will include providing resources, interventions, and information to all service users

About this role…

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.

We are looking for a Lead Worker  to join the Hillingdon Streat Outreach Team.

You will:

• Lead outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation. 
• Represent the team at multi-agency meetings 
• Accurately record all work completed within given timeframes 

To succeed as the Lead Worker, you will have:

• A strong commitment to providing a high standard of service 
• Experience working with people who have rough slept or vulnerable adults and supporting them to access services 
• Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently. 
• Good communication skills and confidence in public speaking when representing Thames Reach externally 
• Experience in forming strong working relationships with partner agencies, being able to challenge them when required 
• Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone 
• A clean driving license (desirable)

What you’ll be doing? 

You will be part of the MYcommunity Linkworkers team. Your main role will be to:

• Provide personalised brief intervention and social prescribing support to individuals, their families and carers.

• Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me.

• Provide person-centred support to enable clients to identify their health and wellbeing needs and goals. Co-produce support plans with clients to discuss what’s important, where they would like to be in the future and how we can support them to achieve this. 

• Manage and prioritise a caseload to ensure clients' needs, priorities and urgent support are met.

• MYcommunity Lambeth works in partnership with a large number of statutory and community services and organisations across the borough. You will link clients with appropriate services to meet their needs. It is vital to have a strong understanding of when it is necessary to refer someone back to health care professionals/ agencies when the client’s needs are beyond the scope of the MYcommunity team.

Dementia Wellbeing Worker 

Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis. 

The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.

The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.  

The successful candidate will: 

• Offer a holistic and person-centered assessment for people living with mild cognitive impairment or Dementia to ensure meaningful interventions.

• Manage a case load of clients, providing practical and emotional support to the most isolated and vulnerable in our community.

• Provide information, advice and support to carers (family/friends), health/social care professionals and other voluntary organisations and to disseminate knowledge and best practice so that individuals living with cognitive impairment in relation to dementia or undiagnosed dementia have the best care possible.  
   

• Maintain accurate and appropriate records via Charity Log database and in accordance with UKGDPR. 

Salary: £33,525.36 to £34,743.24 pa incl. London Weighting.

Richmond Carers Centre, a network partner of the Carers Trust, is a registered charity providing information, support, advice and a voice for carers living and caring in London Borough of Richmond upon Thames.

Helping Carers to Live Well is our organisational aim. We provide a targeted service to young carers, aged 5 – 18 years. Young carers, provide vital emotional and practical support to a family member with a care need including sibling young carers. Their care role can have an impact on their wellbeing, education and social opportunities. We are looking for someone with the skills and ability to offer a coordinated approach in supporting young carers in the London Borough of Richmond upon Thames.

As Young Carers Support Worker you will be part of a small, dedicated team which directly deliver a range of services for young carers, including information and advice, emotional support and mentoring, promoting awareness of young carers and delivering a wide selection of group activities, both term-time and in the school holiday programme. Using all your skills, you will work with the young carers’ team to deliver, review, and continually develop services for young carers to best meet their identified needs.

With your proven knowledge of working with children and young people, families, carers or other vulnerable groups on an individual and/or group basis, you will recognise the importance of being client focused and have a commitment to enhance the lives of young carers and their families, including knowledge and awareness of sibling young carers and vulnerable families. You will be able to demonstrate a broad knowledge of young carer’s needs, a wide range of skills in relation to service delivery to support young carers and have a sound understanding of social care provision in both statutory and voluntary sector environments.

You will be self-motivated and positive in your approach to working with both young carers, their families and team members, and have a willingness to embrace our organisational values.  Being innovative and creative, you will identify and implement a range of support mechanisms for young carers, enhancing their wellbeing and supporting them to achieve identified goals.  Strong administrative, time management, IT and organisational skills are essential.  Your ability and willingness to regularly work outside normal working hours is required.
