Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services. We are recruiting Waking Night Support Workers to join our dynamic service and team. In the role of Waking Night Support Worker; individuals will ensure continues support is provided to young people at the standards of day staff with particular focus on safety of vulnerable young people.

About the Role:

Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. The role requires individuals to work on a rota pattern including weekend and bank holiday working, as the service is open 365 days a year. Hours worked: 21:30 - 07:30.

What you'll do:

Ensure the welfare of customers providing emotional and practical support to customers who are in distress, in crisis or require medical assistance such as emergency first aid or the intervention of medical professionals.

Ability to work empathically and compassionately with customers who present with complex needs such as mental health, substance misuse, safeguarding and physical health support needs.

About you:

• An understanding of best practices in supporting people with complex needs and an ability to support and guidance colleagues.  

• The ability to effectively and intensively case management a small case load of client with complex needs. Also, can create SMART support plans and effectively assess risk.  

• Experience and commitment to working flexibly and creatively to response to changes in sectorial best practice, external environment, organisational requirements, etc.  

• Experience of working with or supporting vulnerable people with support needs around any of the following issues; rough sleeping, mental ill-health, physical health, neuro diversity, problematic substance misuse, ex- offending.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.

Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

This is a Waking Night role working to support young people throughout the night at the service. This role completed 10 hour shifts 4 days a week across all days of the week.

Are you passionate about improving support for families affected by substance misuse?

This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth. 

Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project. 

The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Hillside Clubhouse is recruiting for a mental health employment specialist to join our dynamic Camden team supporting people with mental health conditions to gain and retain employment.  The role is working within the nationally recognised IPS (Individual Placement and Support) model within an NHS secondary care mental health team based in the heart of Camden.

Who we are: 

Hillside Clubhouse is a co-produced mental health employment support charity working across Camden and Islington. We support people with mental health conditions into training, volunteering, and employment opportunities. People can access our services via our Clubhouse, where they can share their skills and expertise with others and support the running of the charity.  Alternatively, they can receive support out in the community from one of our employment specialists based within primary and secondary care.

The Role: 

The role is within our IPS Service and will be based within a Camden NHS secondary care team. As an IPS Employment Specialist, you’ll play an important role in supporting people with lived mental health experience on their recovery journey by supporting them back into paid employment. This is an opportunity to offer ongoing support, to motivate and provide people with hope, while developing the tools and skills required to recover, ultimately transforming their lives.

A variety of skills are needed in this post including but not limited to -  a strong ability to develop empathy and rapport with clients; a determined approach to seeking out job opportunities; a person centric-approach to working with people; strong communications skills to motivate clients; strong administrative capabilities and an attention to detail engage with prospective employers via business development and face-to-face meetings, and to be able to challenge some perceptions around mental health. 

The postholder will be directly employed by Hillside Clubhouse but work under an Honorary Contract from Camden and Islington Foundation Trust.  They will be managed directly by the IPS Operations Manager but work as team member within the clinical team. The role is dynamic and involves being integrated in a clinical team, working within the community, and we operate a flexible working policy where working from home is also permitted up to two days per week.

Overall aim: 

To enable people with mental health issues to gain and sustain competitive employment by working within the evidence based Individual Placement and Support (IPS) model.

Primary Duties: 

• Manage a caseload of up to twenty clients and work within and apply the 8 principles of IPS in all aspects of role. 
• To work with and enable clients to find work using a mix of conventional approaches and employer engagement. 
• To generate ongoing referrals from the clinical team and to manage those effectively. 
• To engage directly and collaboratively with the clinical team while maintaining the existing multi-disciplinary approach to assisting clients seeking work. 
• To report and meet regularly with clinical team service managers and the IPS Operations manager, and to participate in both casework and clinical supervision from IPS Operations and clinical team services managers. 
• To be an active member of both the clinical and Hillside teams 
• To be responsible for own management of administrative duties tied to both Hillside Clubhouse and clinical team and adhering to existing Hillside and NHS protocols. 
• Source job opportunities specific to client needs from a range of sources including those advertised via current and conventional means, as well as through contact with local and Pan London employers. 
• To build up a network of employers to secure available and future vacancies. 
• To maintain existing and seek out new relationships with a range of external agencies of use to the post. 
• To provide ongoing support to both client and employers (where appropriate) when starting work and until such time as the client makes the decision to end the support provided. 
• To work with clients to collaboratively manage job searching activities and engagement with the service. 
• To manage and meet all KPIs, statistics and documentation as required, including but not limited to: referrals, first meetings, job outcomes, job sustainments of 6, 13 and 26 weeks, and work retention. 
• To provide guidance on the impact and/or the potential change to benefits when clients start work. 
• To work and manage time effectively in both community/external settings, and at clinical team.

• To work out of hours as and when required, working in line with Hillside Clubhouse risk management procedures. 
• Undertake training when required and participate in ongoing learning and development. 
• To maintain professional boundaries with all internal and external stake holders including clients, and staff members of both Hillside and the clinical team. 
• To engage and participate in external IPS support services, training, and events. 
• To undertake additional tasks and duties as and when required, and to work outside of normal office hours when necessary.

Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available
Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
Develop and maintain links with all key agencies and service providers in the local community
Empower customers to ensure they receive the service and benefits they are entitled to
Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards
Encourage and enable tenants to pay their rent and to ensure that rent accounts are managed effectively in conjunction with the Housing Management team
Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible

The service operates 9am to 5pm, seven days per week, 365 days a year supporting older people for up to six weeks post discharge from hospital.

You will provide practical and emotional support (excluding personal care) to prevent avoidable readmissions and admissions, and enable our older people to recover safely at home. Offering a helping hand and moral support when they need it most.
