As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.

As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.

You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.

There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

What you'll do:

Work proactively with Slough Hostel team to handle the service caseload and support an effective team approach to meeting each customer's identified needs.
Motivate the team and champion a positive local culture within the service.
Contribute to a positive service environment, ensuring the service is supportive and a place of safety and stability for customers. This includes working flexibly and being proactively in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns.
Deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. This may include practical tasks, such a facilitating group actives
Support customers to undertake domestic tasks, including practical assistance to ensure a high quality living environment that promotes customers' dignity, respect and allows customers to feel safe and welcome in their own home.
Participate in support planning, risk management and assessment of needs in line with support planning.
Work with customers to promote social inclusion to promote independent living, encouraging customers to maximise their skills and choices.
Involve customers in the design, development and delivery of the service through co-production.
Report and react to any observations relating to customers welfare.
Proactive development of links with local statutory and voluntary organizations to provide a range of engagement opportunities for customers in the community.
Competent use of relevant systems, company policies and frameworks.
Adhere to all the policies and procedures of Look Ahead Care & Support plus those specific to the project including any statutory requirements.
Carry out other duties commensurate with the designated role level, as may be deemed reasonable by Look Ahead including working at an alternative patch, if such a need arises.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.

The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.

The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

What you'll do:

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.
The initial and co-produced assessment will incorporate models of co-production, observation, and person-centred planning. This is with a view that it is solution focused as to provide a framework for progression towards independent living.
Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.
Create support/ action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.
Liaise with partner agencies regarding making referrals where an unmet need has been identified via the multi-agency assessment.
Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.

Support customers to undertake all domestic tasks as past of the assessment of daily living skills, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.

Engage in learning and development activity to increase knowledge and skills.
Provide day-to-day instruction of domestic staff/Personal Support Assistants where appropriate.

Support Worker

We're looking for a kind, compassionate and resilient Support Worker to join our Hope House service in Slough.

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.
This is a great opportunity for any support worker to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT) We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

The role 

We are looking for Housing Support Workers who can provide intensive housing management & support to service users in our supported housing projects. 

You will work proactively with service users who have been homeless and have complex needs to sustain their tenancies and move on into independent living.

You will also work empathetically to empower services users, actively engaging them with the community and helping them to support each other, to develop resilience, to live independently, and to be an asset to the community. 

The right fit 

• You will have prior experience with service users who have been homeless and/or have complex needs including, Mental health, substance misuse, domestic abuse & chaotic lives etc. 

• You will also have strong ICT skills. 

• You will be competent in facilitating support sessions and devising support plans to achieve successful outcomes. These will be person centred in their approach, identifying individual needs and strengths of each Service User.

The Hepatitis C Trust has developed projects nationally whereby peers use their lived experience of injecting drug use and hepatitis C, to provide education and training to service users and staff in drug and alcohol, hostel and homeless services, increasing hepatitis C awareness, access to testing and referral to treatment for people living with hepatitis C.

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.

* Assessing referrals received by the service, within the 4-hour target, and providing rapid response to these.

* Building a relationship with the local borough and any key agencies. For example: no recourse to public funds teams, homelessness teams, community mental health teams etc.

ROLE BACKGROUND:

Youth Enquiry Service (YES) is a charity that has supported young people in High Wycombe and the surrounding areas for over 40 years. We have an established reputation for offering accessible, friendly, and safe services for young people, which includes one-to-one counselling, sexual health information, and drop-in information, advice, and guidance.

This is an exciting time for YES, as we have recently been awarded funding to expand our services as an early-intervention mental health support hub for young people. Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.

The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking.
