Job description
Team Leader – Social Prescribing

Team Leaders at the Centre are responsible for the day-to-day delivery by their respective team. This new role will contribute to and support the Delivery Manager to achieve the overall objective of the Centre, which is to deliver services that enable our clients community to thrive.

The Team Leader – General Social Prescribing will line manage the Social Prescribing Link Workers and Coordinator within Generalist Social Prescribing and also deliver frontline services and 1-2-1 support. The split between front-line delivery and management for this role will be agreed with the Delivery Manager but they expect it to be around 80%-line management and 20% delivery (as floating delivery support to the team). The role will oversee the delivery of the social prescribing service, supporting individuals in the team and providing a reflective space to discuss caseloads. It will also involve managing relationships with key stakeholders.

The Team Leader will promote a positive culture of collaboration and innovation amongst their direct reports and team and support the development of viable service propositions to reflect the needs of the community.

The Team Leader will play a key role in continuing to develop our internal culture within teams, specifically driving forward our commitment to being a truly antiracist and inclusive organisation and creating space for honest conversations and feedback.

Our clients Social Prescribing service helps to strengthen personal and community resilience and reduce health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity. The service aims to increase people’s connection with others and their local community and empower them to take control of their own health and wellbeing.

Their service provides:

• Rapid social prescribing: Urgent, one-off support often around isolation, food, money and medication. People are contacted within two day
• General social prescribing: Longer term emotional and practical support. People are contacted within two weeks and offered three to six sessions to explore challenges, set goals and offered a coaching style of support
• Social welfare advice social prescribing: Specialist support delivered by a qualified social welfare advisor to address housing and benefit needs.
• Mental Health Community Connectors: Supporting people with serious mental illness (SMI) to achieve better mental health and wellbeing, increasing independence and reducing reliance on services.

Location: Bromley by Bow
Job Type: Part time, 21 hours per week
