As a Family Support Co-ordinator you will join our team of compassionate, experienced Family Support Co-ordinators, working to build strong relationships with local families facing complex challenges;

You will work one-to-one with families to identify their needs, match them to a trained volunteer and regularly review their progress.

You will provide on-going support and supervision to skilled, diverse volunteers, and work hard to build and maintain links with referring partners throughout our communities, ensuring holistic support for families.

As needs are identified, you will work together with our team of Family Support Co-ordinators to develop and facilitate supportive groups and activities for families.

About the role:
One role is based at Kings College Hospital the second role is based at Princess Royal University Hospital. As a Hospital Independent Domestic Violence Advocate you will:
- implement effective ways of working with male victims and those supporting them to increase safety and reduce harm
- provide a high-quality, front-line service to male victims of domestic abuse, delivering a premium service prioritised according to risk, primarily focusing on victim/survivors aged 16+
- initiate, develop, maintain and monitor multi-agency links through procedures and protocols to keep safety central to all services for victims of domestic abuse
- specialise and be leading practitioner for Male victims of domestic abuse; providing advice, guidance and single point of contact for a male victims
You will need:
- Have a knowledge of help-seeking barriers and support needs of victims of domestic abuse (in heterosexual or same-sex relationships).
- Have a good understanding of domestic abuse including the impact of domestic abuse on victims and their children.
- Understand the principles of risk assessment, safety planning and risk management for victims of domestic abuse and their children
- Understand safeguarding issues, and the legal responsibilities surrounding these issues.
- Direct service delivery to victims of domestic abuse or other vulnerable people.
- Have excellent communication, negotiation and advocacy skills, both written and verbal when interacting with a range of agencies and individuals. Understand and be committed to equal opportunities and diversity issues in policy and practice.
- Have a strong crisis management skills and the ability to deal with stressful and difficult situations.

Focusing on both personal skills and practical ones (CVs, applications, interviews), the course is delivered either in person or online to small, single-sex cohorts over a 6-week period. To succeed in this role, you will have experience of supporting people in group scenarios and be confident presenting both in person and online. Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. Teresa House is a fast stream service where customers are supported up to two years while Hamlets Way is a slow stream service where customers can expect support for a minimum of two to a maximum of 4 years.
The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. As a support worker you will play an integral role in developing effective support and risk management plans. You will also support customers with aspects of their day-to-day living including but not excluded to; social and physical activities, medication, benefit entitlements, maintaining living standards, exploring employment, education and training opportunities.

The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.
South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.
The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.
You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

Summary of Role

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. What you'll do:
- Undertake key-working responsibilities for a caseload assigned by Management staff

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. StreetLink London is a bespoke assessment and response telephone service for people who are rough sleeping in London, supported by volunteers. Building on St Mungo’s extensive experience of supporting people sleeping rough through a range of face-to-face services, this innovative service aims to reduce the incidences of subsequent rough sleeping amongst people who have not slept rough before and increase the speed with which those who have a history of rough sleeping can access support and accommodation.
As an Assessment and Response Worker you will:

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

- Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
