Job description
We have an exciting opportunity to join our team as a Dementia Adviser, working within the Dementia Wellbeing Service in Bristol.

A partnership organisation bringing together the NHS and the Alzheimer’s Society, as a Dementia Adviser within the service you will work as part of a multi-disciplinary team to deliver dementia support, signposting and advice across the city.

This is a role that allows you to make a real difference in the lives of those affected by dementia.

About you
We are looking for someone who:

- is passionate about supporting people to maintain their independence, improve their sense of well-being, and help them to live well with dementia.
- has professional or personal experience of dementia and the challenges people affected by it may face.
- is comfortable working in the community – meeting people in their homes to deliver dementia support.
- is able to manage difficult conversations and adopt a non-judgemental approach.
- can network effectively and collaborate with other professionals to achieve positive outcomes for people.
- is capable of managing a caseload of service users effectively and independently, ensuring face to face reviews and telephone contacts happen within expected timescales.
- has strong written communication skills and can write case notes with clarity, accuracy and timeliness. Previous experience of writing case notes is not required.
- has the ability to travel across the local area independently as often as required

Interviews will take place on the 8th September

What you’ll focus on:

- Managing an existing caseload of service users as well as new referrals to deliver ongoing post-diagnostic support.
- Working collaboratively with NHS clinicians to ensure the right support is offered to people at the right time.
- Writing and updating of ‘Wellbeing Plans’ that capture a person’s history, current support needs, and future wishes. 
- Supporting our peer-support groups, delivering carers education courses and engaging in awareness raising events in the community.
- Identifying service user support needs and referring onwards to appropriate organisations (e.g. social services, other charity organisations).
