We are seeking a Fundraising Manager to help us achieve our income goals, which will, in turn, enable the Trust to have an even greater impact within the Brentford heartlands. The Role

We are looking to recruit a number of job coaches to support interns and graduates of these programs in their jobs.

Background

Action on Disability is the supported employment service partner on a number of supported internships for young people with learning disabilities around West and South London. They are looking to recruit a number of job coaches to support interns and graduates of these programs in their jobs.

The Post

The purpose of the job is to provide coaching, support and guidance to young people with learning disabilities (17+) within their workplaces. You will support interns/graduates to become independent in their roles and provide the necessary coaching, training and encouragement to enable them to succeed. You will work closely with the employer to ensure they are supported and can confidently manage and train their intern/employee once your support has been withdrawn.

Main Duties and Responsibilities

• To carry out a detailed job analysis of potential jobs / work placements
• To carry out detailed job matching of service users and jobs
• To comprehensively learn jobs across different sectors – not restricted to but potentially including – Catering, Customer Service, Administration, IT and Retail
• To implement training/coaching plans which will enable the candidates to fulfil their roles to employer standards
• To conduct regular reviews to ensure candidates are progressing within their roles and to identify and meet new support needs
• To build up natural support within employers and taper off one 1:1 job coaching over time

Horsham Y Centre provides 24-hour supported housing services offering medium levels of housing-related support for young people aged 16-25 with 50 bedspaces and shared communal facilities. We adopt a trauma informed and psychologically informed approach to supporting our residents to help them build essential life skills, identify their goals and work towards aspirations so that they can move on to enjoy fully independent futures. We have a dedicated team of Support Workers, Night Workers and additional Bank Workers who provide support, guidance, and signposting around areas such as housing, budgeting, living skills, jobs, relationship building and wellbeing. Situated in the middle of Horsham, the service has strong links with and contributes to the local community.

We are looking for a Housing Night Worker to join our Horsham Y team. Main areas of responsibilities are:

Safety and security

• Provide a friendly and professional front-of-house welcome to residents, visitors and staff, and ensure the building is safe, secure quiet and welcoming 
• Maximise the wellbeing of residents and visitors by ensuring adherence to health and safety protocols and house rules, all within the terms of tenancy agreements
• Assess and monitor the risks presented by residents to ensure they can keep themselves safe and, where possible, continue their development
• Record all incidents and accidents and share appropriately with the wider team, your manager and, if necessary, the central safeguarding team 

Engagement with residents

• Welcome residents home and provide informal and responsive support so they can articulate their aspirations and ambitions and acquire the skills they need to live independent and fulfilling lives
• Ensure resident’s views, aspirations, concerns, and ideas are sought so they can play an active role in influencing the services they receive
• Proactively promote the services on offer and ensure residents are encouraged to engage with those services
• Contribute to, or support, evening groupwork activities so that residents can access tailored solutions that meet their needs, wishes and aspirations

Administration and housekeeping

• Undertake tasks as directed, including but not limited to: inputting data, assessments, reviews, case notes onto our client database and entering new residents details/closing ex-resident records
• Completing health and safety/compliance checks, and recording estate inspectors and room checks
• Adding maintenance requests to our property services system

General

• Work as part of a team, on a rota four on four off shift pattern, alongside another Housing Night Worker, ensuring young people at the service have non-judgemental, objective at night, along with taking responsibility for personal safety during periods of lone working
• Contribute to a great working environment, with a calm, yet assertive manner, being able to handle potentially difficult situations
• Participate in relevant continuing professional development and utilise Reflective Practice Supervision as part of leading psychologically informed practice

About the Role

KEY OVERVIEW

Reporting to the Housing First Team Leader, the Housing First Support Worker is responsible for coordinating and providing a personalised, trauma informed support service to people with complex needs facing multiple exclusion in line with Housing First Principles.  The Housing First Support Worker will be responsible for coordinating and providing a person centred, strength-based support service to create long term sustainable tenancies by empowering customers, increasing independence, and maximising connectivity with the local community.

ADDITIONAL INFORMATION: 

• Valid driver's license and access to a car business purposes
• Please note the working pattern for this role is Monday to Friday, with occasional weekend work required 
• This post will include a 6mth probation period.

KEY RESPONSIBILITIES:

1. Delivering wrap-around intensive, trauma informed support to people with multiple and complex issues within their own independent tenancies in the community.

2. Working with Housing First participants to help them articulate their goals and aspirations offering personalised support and promoting choice and wellbeing. Supporting Housing First participants to access community resources, to develop positive peer networks and meaningful leisure opportunities.

3. Maintaining accurate up to date records on the Inform case management system of progress against individual support plans, safety plans and outcomes information.

4. Regularly review the person-centred support plan to update, record and monitor progress of customer caseload.

5. Develop, maintain and co-ordinate effective working relationships with external partners and agencies, promoting effective communication with Housing First residents. Ensuring Connected Housing First is widely known and promoted in Mendip and Somerset.

6. Liaise with landlords to assist the person living in accommodation of their choice; including practical help with furniture; benefits advice and accompanied visits.

7. With the agreement of Housing First participants, to liaise with other agencies to facilitate support including substance misuse and recovery, offending and criminal justice, mental health, Adult social care, debt, domestic abuse, welfare and housing benefits, employment and training.

8. Promote, and actively encourage peer involvement within the service.

9. Complete dynamic risk assessments and maintain own safety by following lone working procedures.

10.  Adhere to good practice safeguarding procedures with a positive risk-taking approach to ensure the wellbeing of vulnerable people.

11.  To identify barriers of accommodation and managing tenancies, providing support including practical help with benefits, budgeting, furniture, neighbour relations, and liaising with landlords.

12.  To contribute to regular reports on the service evaluation project as required.

13.  To participate in reflective practice to support and enhance wellbeing in the workplace and personal development.

14.  Record all petty cash and other expenditure of project monies in line with Connect Centre policy.

15.  Work within the Connect Centre Lone Working Policy and other Connect Centre Health and Safety Policies as appropriate at all times

About the role:

This role is based In Pontypool Police Station however we have currently adopted a hybrid model. As a Triage and Early Interventions Officer you will:

• Act as the first point of contact for clients referred in to the service, ensuring that comprehensive impact and risks assessments are completed and a tailored response to each client's needs is provided.
• Carry a caseload of clients who require more immediate and short term interventions, referring on those with more long term needs either internally or externally. Ensure each victim receives an individually appropriate tailored support and information service that fully meets their needs, keeping complex needs central to all processes and decisions.
• Ensure that support plans are developed with clients where appropriate.
• Maintain accurate and confidential case management records and contribute to monitoring information for the service.
• Provide accurate and timely performance information for internal and external use.
• Work with the client and other statutory and voluntary agencies to address the identified needs.
• Respond appropriately to safeguarding concerns including both child protection and vulnerable adult issues.
• Ensure that all clients are of aware of their rights under the Victim's Code of Practice and the Witness Charter.
• To work alongside a team of staff (and volunteers where applicable) to reflect the diverse community and the needs of the victims, including those with particular vulnerabilities, utilising national and local resources to deliver individual, bespoke services with respect, dignity and sensitivity.
• Comply and keep up to date with data protection legislation, confidentiality and organisational policies and procedures, as well as all legislation connected to your work.
• Work as part of a team to ensure that all outcomes and KPI measures are met relevant to the local contract.

You will need:
