We are seeking two Regional Caseworkers to join our team, one in the Northern Circuit and one in the Northeastern Circuit. This role has a dual focus, combining casework responsibilities with stakeholder management, requiring the successful candidates to build strong relationships and rapport across their assigned circuit. You will also represent Advocate by attending events and delivering information sessions to raise awareness of our services and promote our work.

We are seeking candidates with a strong understanding of trauma-informed practice and experience supporting people through the impact of traumatic bereavement and/or injury.

You will provide a specialist trauma-informed and bereavement-informed approach to care, helping individuals and families navigate the immediate and long-term impact of sudden loss or catastrophic injury. You will undertake a comprehensive needs, risk and safety assessment from which a bespoke support plan will be agreed with the person and/or families, ensuring that immediate wellbeing needs, vulnerabilities and safeguarding considerations are identified and addressed.

Addiction Family Support is the leading UK-registered charity supporting adults affected or bereaved by a loved one's harmful use of alcohol, drugs, or gambling.

The role: Family Support Worker (Bereavement)

As a Family Support Worker at Addiction Family Support, you will be responsible for delivering a range of services to provide crucial emotional and practical assistance to individuals bereaved (and affected) by the loss of a loved one due to addiction. Through both individual and group support, you will offer a compassionate and understanding presence, helping people navigate grief and loss in a safe and supportive environment.

Our service delivery takes place via several methods, including taking calls on our telephone Helpline, responding to emails through our email service and delivering remote 5-Step Method informed intervention to individuals and groups via an online platform.

Working patterns are flexible and you may be expected to work on a rota basis, and we would hope to be able to offer more hours per week in time when funding allows it.

You will need to be confident in your ability to provide support via a variety of remote and digital platforms, being able to respond to immediate requests for support. You will be an active and involved member of our team and contribute to all aspects of our work to help us achieve our strategic goals and work towards achieving our vision.

Responsibilities:

• Offer empathetic and non-judgmental structured emotional and practical support to those affected by the bereavement of a loved one due to addiction (via a Helpline and 5-Step Method informed one to one and group support).
• Provide or signpost information, literature, and resources to help individuals and groups manage grief and cope with their situations.
• Facilitate or co-facilitate bereavement support group meetings, creating a safe space for participants to share their experiences and provide mutual support.
• Follow guidelines and safeguarding practices - ensuring safeguarding concerns are addressed promptly and signpost individuals to additional services where appropriate.
• Maintain accurate and confidential records in line with GDPR and the charity’s policies.

The Role

Social prescribing link workers will work as a key part of the primary care network (PCN) multi-disciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience and reduces health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

Responsibilities include:

• Working with direct supervision from the Programme Lead/Senior Link Worker and input from the PCN’s Clinical Lead GP, take referrals from staff in GP practices within the local PCN.
• Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team.
• Developing trusting relationships and taking a holistic approach, based on the person’s priorities and co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
• Work collaboratively with all local partners to contribute towards supporting local community groups and organisations, making themselves aware of local resources and “assets” through mapping and then building a relationship with these organisations.  Also sharing intelligence regarding any gaps or problems identified in local provision with commissioners and local authorities. 
• Social prescribing link workers will work collaboratively with GP’s and have a role in promoting social prescribing and educating non-clinical and clinical staff within their PCN multi-disciplinary teams on the benefits of social prescribing
• Raise awareness of other services available within the community and how and when patients can access them. Linking into the work of Neighbourhood Network Schemes, locality Adult Social Care & Community Mental Health Teams.

About You

As a Social Prescribing Link Worker, you will have a Level 3 qualification in a relevant field (Health & Social Care, Health Trainer, Health Improvement, Community Family Worker, Psychology) or be working towards/willing to work towards.

You will have:

• Demonstrable commitment to professional and personal development
• Experience of working directly in a community based context (including unpaid work)
• Experience of supporting people with a mental health need and or complexities/vulnerabilities
• Experience of delivering one to one support
• Experience of partnership/collaborative working and of building relationships across a variety of organisations
• Knowledge of VCSE and community services in the locality
• Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers
• Understanding of the barriers faced by those living in isolation
• Knowledge of IT systems including ability to use word processing skills, emails, the internet and able to create simple plans and reports
• Ability to complete paperwork, keep records and carry out own administration

The Role

Working as part of a team, the job may involve working weekends, unsociable hours, and bank holidays and you will be required to work flexible hours to meet the needs of children and young people. 

Duties include:

• Design and deliver an age-appropriate programme of structured and free play activities to support children’s wellbeing and development.
• Identify individual support needs and coordinate with colleagues and partner agencies (e.g., schools, health and social care) to ensure needs are met.
• Build positive relationships with children and young people, ensuring sessions are inclusive, engaging and safe.
• Promote safeguarding and follow child protection procedures, including advocacy and attendance at relevant multi-agency meetings when required.
• Maintain accurate records and monitoring systems, producing monthly/quarterly reports against agreed outcomes.
• Create opportunities for children, young people and parents/carers to participate, provide feedback and shape service delivery.
• Plan and organise resources and activities (including offsite and holiday provision), evaluating sessions to drive continuous improvement.

About You

You will have experience of working with children and young people aged 0 – 18 years old and be able to speak at least one South Asian language.

You will also have:

• A good standard of education, including a minimum of level 4 or equivalent in Maths and English.
• NNEB Diploma/BTEC in Nursery nursing / NVQ3/CQS in childcare or equivalent
• Experience of devising and delivering creative play activities for children that assist in their holistic needs.
• Experience of working as part of a team and able to demonstrate your positive influence on team dynamics
• Able to communicate clearly, both verbally and in writing
• Able to work flexible hours, including evenings and weekends.
• Able to listen actively and adapt your communication style to meet the needs of the recipient.
• Able to be calm and resilient whilst under pressure and to remain optimistic and persistent.
• Be IT literate and familiar with Microsoft Word, Excel, Access and PowerPoint.
