SEL Mind are working in partnership with Oxleas NHS Foundation Trust to deliver the Bromley Mental Health Hub (BMHH), as part of the ongoing transformation of mental health services in the London Borough of Bromley. The service integrates the expertise of local primary, secondary, and voluntary sector providers to offer flexible, holistic and person-centred support for adults experiencing mental health difficulties.

We have an exciting opportunity for a Peer Support Manager to join the BMHH team on a one year fixed-term basis. During the fixed-term period, the post holder will lead on reviewing, strengthening and developing the peer support pathway. You will play a key role in shaping support for our clients that enhances quality of life, choice and opportunity. You’ll be integral in leading the pathway to achieve its objectives to help clients improve confidence and resilience, reduce social isolation and encourage independence and integration into their local community. 

In addition, you will be responsible for the day-to-day operational delivery of peer support in the BMHH, including line management of a Senior Peer Support Coordinator who oversees delivery of both one-to-one and group-based peer support, delivered by the wider peer support team. The role also holds responsibility for supporting the team in dealing with risk and safeguarding matters that may arise. Occasional out-of-hours work may also be required to support the team in their group delivery outside of core office hours.

The role requires excellent interpersonal and organisational skills, a commitment to recovery-focused practice, and the ability to work flexibly and collaboratively with SEL Mind colleagues and within a multidisciplinary team in partnership with Oxleas NHS Foundation Trust. You will have strong line management experience and experience of delivering peer support services, including experience of working with people who have complex or heightened needs. You will also possess a strong understanding of the principles of peer support.

About the Role

Community Connectors work as part of an integrated, blended mental health offer to support people with emotional, social and practical needs to access a range of local, non-medical and non-clinical services, to improve confidence, social inclusion and independence.

This role will involve working as part of a multi-disciplinary team with statutory, third sector and NHS colleagues to support clients who are suffering from moderate to severe mental health difficulties. You will need to have experience working with those suffering with mental health illness as well as being able to demonstrate in depth knowledge of the services, support networks and local resources that may assist in improved outcomes for those dealing with complex and enduring mental health issues. The ability to be adaptable, work independently and the resilience to work in some challenging environments are key attributes for this role.

Service Delivery

• Assess suitability for community connection support as part of a blended team approach and support individuals to access the service by utilising in depth knowledge of local voluntary and community services and networks in your respective area

Job description

Job Purpose

To manage and develop the home from hospital settle service in the hospitals that you are responsible for, to support timely and successful patient discharges from hospital and prevent re-admissions.  You will work in collaboration with hospital teams and community teams to lead and manage your team to support the smooth transition from hospital to home for service users.  The service works with people for time limited support (usually up to 6 weeks) to adjust and settle back in their homes, and to prevent readmission into hospital. 

Key Tasks

• Work in partnership with the hospitals and other health and social care professionals to coordinate safe and timely discharges for referred clients.  This involves attending meetings with hospital personnel including Discharge Co-ordinators and Ward Managers to discuss discharges and plan who and how individuals will be supported as appropriate. 
• Manage a team working across two hospitals and ensuring the service is available between the contracted hours.
• Work collaboratively with other agencies providing support services.
• Conduct initial assessments of people referred to the services as necessary.
• Plan and develop person centred interventions to provide short term support for people after hospital discharge.
• Work with GPs as appropriate in identifying and supporting patients to reduce unnecessary hospital admissions.
• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital.
• Support the hospitals with delayed transfer of care (DTOC).
• Coordinate and monitor the caseload of referrals allocated to team members to ensure a fair and manageable distribution of work.
• Rota planning to ensure staff flexibility and availability, enabling planned rosters that best suit both hospital and patient needs.
• Develop your team which includes; supervision, appraisal, planning of continued professional development opportunities and training needs.

Location: Face to face service based in Royal Borough of Kensington and Chelsea (RBKC), the service is a short walk from Ladbroke Grove (Circle and Hammersmith) station. Unfortunately, this service does not have step free access.

We are seeking a compassionate and dedicated Support Mentor to join our team based in RBKC to support our residents who have multiple and complex needs which could include mental health, substance use, homelessness, and/or offending backgrounds. You will support in providing our residents with the support needed to help them overcome their personal barriers, and achieve their goals in a person centred approach. You will work within a residential setting, creating a safe and comfortable space which our residents can call their homes during their time within the service. The role includes:

• Supporting residents with daily tasks
• Holding a caseload, conducting key work sessions which are personable to their needs
• Carry out regular welfare checks, safeguarding checks, and general living checks
• Encourage residents to take part in activities and events
• Keep support plans up to date on a regular basis
• Work closely with the rest of the team to provide overall excellent service delivery, this may mean also supporting with their caseloads, and other tasks as necessary
• Carry out general housing management support

About you

We're looking for someone who has a true passion to support those who have multiple and complex needs, with a drive to help support those who suffer with homelessness, mental health, and substance use. You will be able to build rapport and trusting relations in a professional setting, and able to build positive outcomes. You will have an adaptable and flexible approach to helping others, with an understanding that each individual has different levels and types of support needs. 

• Experience of working with people of complex backgrounds and/or a good understanding of the sector, particularly around multiple and complex needs

About the Role
 
Responsibilities will include:

• Lead delivery of key Local Offer programmes, such as our successful Peer Review and Peer Network Programme, which are designed to support Local Authorities and organisations in meeting the needs of young people and in the case of Local Authorities meet their Statutory Duty (section 507B).
• Manage the delivery of traded services relating to Quality Standards and Organisational Development.
• Manage and regularly update both existing and new online resources related to Quality Standards, ensuring consistency and standardisation across all materials.
• Collaborate with the National Curriculum and Learning Manager to influence the youth work curriculum and share learning and insight from programmes
• Work collaboratively with the Youth Investment Fund Programme Manager to inform and develop Quality Standards.
• Work with Local Policy & Partnerships Manager and the NYA Knowledge Team to develop sector insights and to share learning and knowledge across NYA teams and the youth work sector.
• Collaborate with Local Authorities and partner organisations to promote awareness and encourage participation in NYA programmes and Quality Standards.
• Utilise NYA tools, including Customer Relationship Management (CRM) and Project Management systems, to generate timely and relevant performance and financial reports that support informed decision making and effective programme delivery. Experience of using project management platform (or similar) would be an advantage.
• Provide line-management and support to peers and officers as required.

 
Our ideal Local Quality Improvement Manager should have the following:

• Extensive experience working directly with young people across diverse settings, demonstrating a deep understanding of youth development and engagement.
• Demonstrated expertise managing youth work programmes and improving practice standards, including within Local Authority contexts.
• Proven experience in assessing youth work against established standards and implementing quality assurance processes for effective management, delivery, and evaluation.
• Comprehensive knowledge and experience in embedding best practices in youth voice, participation, and leadership within services and programmes.
• Strong understanding of the current challenges facing Local Authorities and youth sector organisations, and the implications for young people’s needs and service delivery.
• Ability to analyse complex organisational and Local Authority structures, reflect critically, and articulate clear recommendations for improvement and recognition of effective practice.
• Excellent organisational and leadership skills, with experience supporting and managing peers and officers in a collaborative environment.
• Strong interpersonal skills with the ability to motivate teams and work effectively with partners and stakeholders.

We are seeking an experienced project manager to lead WHA’s delivery of the SAFEStart+ project. Focused on accelerating the elimination of mother-to-child transmission (EVT) of HIV, syphilis, hepatitis B, and Chagas disease in endemic areas. The project will focus on demand generation, adoption and scale-up of innovative EVT programmes through strong community and stakeholder leadership. It will do this in part, through understanding the values and preferences of stakeholders, addressing barriers around awareness and stigma and discrimination, providing training to both community groups and health care workers and undertaking community led monitoring. These community led-activities will run alongside work in countries to strengthen health systems and supply chains and address gaps in product access.

The duties and key responsibilities of the Community Implementation Manager include:

• Plan and coordinate WHA activities related to the project to ensure that goals and objectives are accomplished within prescribed time and funding parameters.
• Manage community implementation partners within project countries, working closely with them to develop advocacy and awareness strategies, implement community-based research , monitor budgets in line with approved policies and measure impact.
• Support the coordination of global and country community advisory boards.
• Be the main point of contact for all project partners, fostering positive working relationships to ensure effective coordination throughout the projects’ lifecycle.
• Support capacity building efforts for community groups and health care workers in project countries.
• Work with teams across WHA, including communications and advocacy, to ensure the project outcomes are being appropriately disseminated.
• Coordinate all internal and external reporting related to the project.
