Are you passionate about supporting vulnerable adults during critical moments in their care journey? As a Hospital Care Navigator, you’ll be a vital part of ensuring patients leave the hospital safely and confidently, with the right support in place.

In this rewarding role, you’ll be the single point of contact for adult patients preparing for discharge from the hospital. You’ll work closely with the Adult Social Care team, health professionals, hospital practitioners, and community services to:

• Identify needs and barriers to safe discharge
• Achieve safe and timely patient discharge
• Coordinate outpatient appointments and follow-up services
• Provide clear, compassionate advice and support to patients and their families
• Ensure patients understand their discharge plan every step of the way.

You’ll meet with patients, their families, friends, or carers to assess ongoing needs, ensuring a safe plan is in place for their return home. This could include ensuring that medication is arranged from the hospital pharmacy, someone is at home to greet them, food and heating are available, and the condition of the home is safe to return to. You may be required to liaise with Adult Social Care and other home providers to coordinate home support. You’ll follow up on calls or visits to ensure the patient is managing well at home. By providing the proper support, patients can be discharged sooner, and the risk of readmission is significantly reduced. 

We’re seeking someone who:

• Has experience working with vulnerable adults in social care, healthcare, or the community/voluntary sector
• Communicates with empathy and confidence across different teams at different levels, and with patients
• Is highly organised, with excellent time management skills
• Has awareness and respect for cultural differences

About the Role

As Research Fellow, you will lead research and evaluation for the Maudsley programme and contribute to the wider evidence and insight function of WCC. You will use both qualitative and quantitative methods, support digital innovation with KCL, and help ensure that young people’s experiences, outcomes and priorities shape service development.

Your work will inform commissioners, clinicians and system partners working to reduce health inequalities and strengthen support for adolescents across South London and beyond.

About You

Essential Criteria

• Postgraduate degree (e.g., MSc) in epidemiology, biostatistics, public health, social sciences or related discipline

• Excellent written and verbal communication

LSFRC is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.

The role of our Support Worker plays a crucial role in fulfilling our customers' expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Workers are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers.

An essential part of this role involves developing good partnership working with a range of professionals and maintaining excellent record keeping in line with our compliance management. You will also be required to oversee support planning, Risk Management Plans, and Key-working.

You will be working for a charity that transforms the lives of care-experienced young people, helping them gain the skills, confidence, and inspiration to achieve sustainable employment. Position: Careers Specialist

Location: Hybrid – London Waterloo office
Salary: £32,000 – £34,500 per annum (depending on experience)
Contract: Fixed term until December 2026 (with possibility of extension)

About the Role

As a Careers Specialist, you will play a key role in supporting and empowering young people to overcome barriers to employment and progress towards meaningful, sustainable careers. You will build trusted relationships, broaden horizons and inspire ambition, using a person-centred and strengths-based approach.

Through personalised Information, Advice and Guidance (IAG), workshops and employer engagement, you will help young people develop confidence, employability skills and clear career pathways, ensuring they feel supported every step of the way.

Main Duties and Responsibilities

• Provide individual, person-centred IAG to young people, supporting them to identify and achieve their career goals
• Identify and create opportunities for employment, education and training aligned to participants’ interests and aspirations
• Deliver workshops, group sessions and outreach activities across multiple London boroughs
• Support young people with CVs, applications, interview preparation and professional skills development
• Maintain an inclusive, client-centred approach, sensitive to diverse backgrounds and experiences
• Record and monitor participant progress using internal databases and contribute to reporting for funders and partners
• Work collaboratively with delivery partners to achieve agreed outcomes and milestones
• Represent the programme at internal and external meetings, sharing learning and good practice
• Contribute to a positive, supportive and learning-focused team culture

About You

Experience, Qualifications and Skills

• At least 2 years’ experience supporting young people or adults from disadvantaged or underrepresented backgrounds into employment, education or training
• Proven ability to build trust and work effectively with individuals with complex needs
• Strong written and verbal communication skills
• Experience delivering employability workshops, training sessions or group activities
• Sound understanding of safeguarding and risk management
• Strong organisational, time management and record-keeping skills
• Confidence using IT systems and databases (e.g. Salesforce or similar CRM tools)
• Resilient, adaptable and committed to helping young people reach their full potential

About the role

We are currently looking for an impassioned and self-driven individual to join our activities team to deliver exceptional service to our students, through engaging with the 350+ student groups, which deliver an incredible range of world-class opportunities to their members.

You will coordinate the creation of new student groups and manage funding opportunities as well as lead on the onboarding for new committee members and oversee administration for the minibus fleet. You will review, advise on, and approve society event/trip budgets and finance, in collaboration with the Activities (Events) Team. You will also support clubs and societies to effectively manage their finances, and  review Society sponsorship contracts to make sure they are in-line with College and Union policies, that directly benefit and enhance the student experience.

A significant part of the role will be to deliver our annual budgeting process in line with Imperial College Union's grant allocation and financial policy, as well as supporting student-led groups to plan budgets effectively.

The role sits within the Directorate of Membership Services and the post will have a cross-departmental focus to ensure our training package, volunteer support and all processes are equitable and effective for all student groups.

What you would be doing

The Activities Coordinator (Operations) is a critical role in delivering the Students’ Union’s strategic objective of fun and inclusive communities through support, development and empowerment of student-led groups. The role is responsible for supporting the:

• Delivery of training relating to financial processes
• Processing financial documentation that enables diverse activity for student groups
• Coordination of finances associated with club and society activity, annual and event budgeting in conjunction with colleagues in the finance team.

Alongside the Activities Manager, elected Officer Trustees, and the wider Activities Team, the role will support the delivering of a high-quality support service for student-led groups.
