Job Purpose

This role sits within our Adult Services, across Hammersmith, Fulham, Ealing, Hounslow. You will be responsible for the operational delivery and service development, of a portfolio of services. Namely (but subject to change):

• Safe Space Community Drop In Service 
• Hounslow Helpline
• Nova Roots (Forensic Community Hub)

One large service you will be responsible for is our crisis alternative service – Safe Space - which is a tri-borough core and out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 14:00-22:00 across all of our boroughs. You will be responsible for managing peripatetic teams: we currently have 3x in person Safe Spaces, a Helpline and a Forensic Community You’re your portfolio may expand as new services are acquired.

Our aim of the service is to keep individuals well in the community and prevent readmission into hospital. The service operates 365 days a year from 14:00-22:00pm across all of our boroughs.

Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone or virtual on a one-to-one basis to individuals experiencing mental health crisis or preventing crisis. 

The objectives of the service include: 

• To improve the mental wellbeing of people experiencing mental health and social crisis in HFEH. 

• To proactively work with keeping people well in the community to reduce re-admitters into hospitals by working with WL NHS teams (e.g. discharge, LPS, CATT, MINT and SPA) 

• To provide a true alternative to A&E via a non-clinical drop-in service to support clients 

• To provide support to clients accessing the service- for instance: signposting, de-escalation and crisis recovery planning. 

• To contribute to an improvement in individual mental wellbeing. 

• To remain a source of independent support for all clients. 

• To treat service users with respect, dignity and personalised support 

• To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation 

• To increase self-management skills of those accessing the service 

• To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis via a drop-in service. 

• To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual. 

The Role

The role of the Crisis Alternative Service Manager will provide operational management, mobilisation and service development of all Crisis’ Services within Adult Services. The ideal candidate will have experience of mobilising and developing high quality, large services. In addition to managing and supporting staff working within challenging environments and experience of working with challenging behaviour and complex needs. The Service Manager will hold all operational risk.

Excellent organisation and decision-making skills are essential for this role, as it requires being able to manage time effectively to meet tight deadlines and work unsupervised during evenings and weekends. You will also be required to be rostered into the on-call rota.

The Crisis Alternative Service Manager will be required to work collaboratively with the Director of Adult Services and will be proficient at communicating effectively with staff at all levels, a wide range of key stakeholders and service users. They will ensure the service is well resourced from a staffing perspective and embed high-quality operational plans and processes. The Service Manager will also be expected to build and sustain working relationships with statutory services and represent HFEH Mind at external meetings.

They will be responsible for the delivery, progression and continual improvement of the service. They will have responsibility for ensuring Crisis Services operate in line with organisational H&S processes and procedures, HFEH Mind policies and CQC requirements

Key Responsibilities

· To oversee the management of the Crisis Alternative Drop-in Services and other services, in line with organisational values and service quality expectations.

· To ensure all risks are recognised and mitigated in line with the serious incident and safeguarding policies.

· To ensure safe working and risk management

· To ensure feedback is provided on risk mitigation through serious incident reports.

· To continually drive enhancements to operational effectiveness and maximise outcomes against targets (key performance indicators)

· To complete audits and high quality reports for senior internal and external stakeholders

· To ensure interpret performance and impact data for crisis services and be responsible for reporting against service targets

· To provide effective line management supervision to Team Managers, Senior Support workers and Support Workers where applicable.

· To collaborate with the Director of Adult Services and the Head of HR to deal with performance management/disciplinary issues confidently and effectively in accordance with organisational policies and procedures.

· To mobilise the crisis services in line with agreed project plans and ensure they are promoted and marketed well.

· To pro-actively develop relationships with external stakeholders in order to improve and develop crisis provision for people with mental health needs.

· To develop robust referral routes and pathways into the service with both statutory and non-statutory bodies.

· To ensure excellence in the recruitment, training and ongoing support of staff and volunteers where applicable.

· To support the Deputy Manager, Team Managers to ensure adequate and appropriate staffing at all times within the service.

· To ensure high levels of meaningful service user involvement in service provision, responding to the needs of service users locally.

· Work collaboratively with West London Mental Health Trust staff to support, develop and promote both the service.

· Establish excellent working relationships with core and out-of-hours services such as Emergency Services, Crisis Teams and GPs, around inward and outward referrals.

· Ensure quality and compliance (in line with CQC standards, NICE guidelines, MQM and other relevant standards)

·        Work within HFEH Mind’s policies and procedures including; Performance Management, Confidentiality, Safeguarding, Equal Opportunities, Service User Involvement, Health & Safety, GDPR and Professional Boundaries.

· To embed all operational policies across managed services in line with a consistent organisational approach.

· Ensure service users satisfaction surveys are undertaken.

· Ensure data and qualitative information is collected, collated and communicated to key partners and key stakeholders, as required and directed by the Director of Adult Services.

· Ensure that all managed services are effectively informed by service user feedback.

· Keep up to date with best practice and contribute to the continuous improvement of the service.

· Monitor the quality of the service and ensure appropriate data is collected (in line with GDPR) to monitor outputs and outcomes.

· Produce regular service summary reports.

· To be rostered on the on-call rota

· Undertake such duties not included in the job description as are reasonably requested by the Director of Adult Services

Person Specification

· A minimum of three years managing a crisis service or similar setting with high complexity and high incident rates

· Experience of managing and mitigating risks

· Experience of managing safeguarding risks and understanding legal requirements for safeguarding adults and children

· Understanding of how to report and mitigate risks

· Understanding of suicide prevention

· Understanding of trauma informed care

· Understanding of mental health and safety planning

· An understanding of CQC requirements for mental health

· Managing services for vulnerable people in a service delivery organisation.

· Excellent people and project management skills and significant experience of supporting and managing staff.

About the Role

As a Night Support Worker, you will work under the direction, guidance and support of a Service Manager and an Assistant Area Director you will work as part of a team covering the night shift hours delivering an assessment, support and move on service to vulnerable clients in supported accommodation.

LMK Leaders are passionate and organised educators who run community workshops, aimed at young people aged between 11 and 24. Expectations of LMK Leaders

• Always adhere to LMK and host organisations health & safety and safeguarding practices and procedures. 
• Capable of maintaining confidentiality and professional boundaries with young people, peers, and professionals.
• Respond to comms (emails/WhatsApp) sent out by LMK team members in a timely manner, to support planning and organising of sessions.
• Plan and deliver interactive LMK workshops in educational and community settings in line with LMK delivery style and ethos. 
• Meet with co-facilitators to carry out pre-session planning, minimum three working days before delivery.
• Use your experience and knowledge of the issues affecting young people to tailor the workshops accordingly. 
• Ensure all youth participants and the adults in school/community organisations who attend a LMK workshop complete the post session feedback surveys.
• Complete a reflective post session Leader survey after each workshop to support practice and programme development.
• Debrief with LMK’s Programme Development Manager and provide ongoing feedback about your sessions through LMK’s reflective practice processes.
• Accommodate a maximum of two workshop observations by LMK staff/supporters per academic year, designed to bolster our funding and thought leadership strategy.

• Engage with LMK’s Youth Advisory Board Members, if and when invited to do so.
• Lead by example and promote healthy relationship behaviours in your own lives/communities and within LMK (team player; acknowledging and working with diversity; being receptive to new ideas and developmental feedback, etc)
• Use your own networks to recruit new schools and youth groups into the programme.

Knowledge, Skills and Experience required

• Extensive experience of teaching or working with young people (10 to 24 yrs old) in educational or community settings.
• Minimum 2 years’ experience co-designing, delivering and evaluating group sessions, consisting of minimum of 12 and maximum of 30 participants.
• Experience or co-designing session plans and co-facilitating with others; able to collaborate equally with others.
• A strong commitment to young people and knowledge and understanding of issues affecting their relationships.
• Excellent communication, interpersonal skills with the ability to create a safe and non-judgmental space, that allows young people to speak freely about their relationships and experience.
• Able to adapt and be flexible in workshops to get the best engagement from a range of participants. 

Must also:

• Be entitled to work in the UK.
• Hold a DBS certificate or be willing to let LMK undertake a check on your behalf. 
• Have a recent safeguarding qualification from a recognised provider or be willing to undertake additional training in this area.
• Be flexible to work non-traditional, after-school and occasional weekend / holiday hours.

KEY OVERVIEW

Reporting to the Housing First Team Leader, the Housing First Support Worker is responsible for coordinating and providing a personalised, trauma informed support service to people with complex needs facing multiple exclusion in line with Housing First Principles.  The Housing First Support Worker will be responsible for coordinating and providing a person centred, strength-based support service to create long term sustainable tenancies by empowering customers, increasing independence, and maximising connectivity with the local community.

ADDITIONAL INFORMATION: 

• Valid driver's license and access to a car business purposes
• Please note the working pattern for this role is Monday to Friday, with occasional weekend work required 
• This post will include a 6mth probation period.

KEY RESPONSIBILITIES:

1. Delivering wrap-around intensive, trauma informed support to people with multiple and complex issues within their own independent tenancies in the community.

2. Working with Housing First participants to help them articulate their goals and aspirations offering personalised support and promoting choice and wellbeing. Supporting Housing First participants to access community resources, to develop positive peer networks and meaningful leisure opportunities.

3. Maintaining accurate up to date records on the Inform case management system of progress against individual support plans, safety plans and outcomes information.

4. Regularly review the person-centred support plan to update, record and monitor progress of customer caseload.

5. Develop, maintain and co-ordinate effective working relationships with external partners and agencies, promoting effective communication with Housing First residents. Ensuring Connected Housing First is widely known and promoted in Mendip and Somerset.

6. Liaise with landlords to assist the person living in accommodation of their choice; including practical help with furniture; benefits advice and accompanied visits.

7. With the agreement of Housing First participants, to liaise with other agencies to facilitate support including substance misuse and recovery, offending and criminal justice, mental health, Adult social care, debt, domestic abuse, welfare and housing benefits, employment and training.

8. Promote, and actively encourage peer involvement within the service.

9. Complete dynamic risk assessments and maintain own safety by following lone working procedures.

10.  Adhere to good practice safeguarding procedures with a positive risk-taking approach to ensure the wellbeing of vulnerable people.

11.  To identify barriers of accommodation and managing tenancies, providing support including practical help with benefits, budgeting, furniture, neighbour relations, and liaising with landlords.

This new service will offer a safe, stable, and supportive environment for young people facing homelessness, many of whom may have mental health challenges, substance misuse issues, or experience of the care or criminal justice system. Initially providing a hot meal, clothing, and advice to those who came for help, Spires has developed into an organization that provides a range of services, including day centre style drop ins for homeless people, a Women’s drop in, a Refugee drop in and outreach targeted at supporting Women who are sex working.

Overview  

At Young Roots, we want to see a compassionate and welcoming society for young refugees and asylum seekers in the UK. We work alongside young people seeking safety in the UK, building trusted relationships, providing practical and emotional support and promoting young people’s rights and power.   

Our youth clubs and casework are transformative for young refugees, allowing young people who have fled danger, had traumatic journeys and who are often here alone, to find community and connection, have a space to be a young person and access support in addressing a whole range of practical challenges they face. We also draw on our evidence from working every day with young refugees and asylum seekers to call for change to the laws and policies which are harming young people.  

The Youth Development Coordinator is responsible for the development and delivery of our youth development programme for young refugees and asylum seekers. 

You will be focused on ensuring our programme activities achieve our outcomes, are in line with our strategy and identify areas for change and development under the direction of the Head of Services. An excellent communicator and skilled at working with partners, you will be key in the implementation and running all of our weekly youth activities, including a Youth Club and Advice and Support Hub working closely with the Youth Development Worker and having oversight of our weekly Young Women’s group. 

As an experienced manager, you will lead our team of skilled youth workers, working alongside the Brent team to provide holistic support to young asylum seekers and refugees in Brent.  

You will prioritise the participation of young people in all activities, ensuring excellent safeguarding by following our policy and protocols, and ensure that we collect excellent data for monitoring and evaluation and to aid our future planning. You will also assist with staff and volunteer recruitment and reporting to our funding partners. 

The Youth Development Coordinator will hold a Designated Safeguarding Officer role at Young Roots for which they will receive full training.

The Snowdrop Project is the first organisation in the UK to provide long-term, community based support to survivors of modern slavery and exploitation. We aim to empower survivors of modern slavery and exploitation to recover from their experiences and thrive.
