Job Title: Housing Support Worker

Location: Based in Brent. This is a floating support service which means locations may vary from outreach sites, personal homes, and our office space. Please be kindly aware that although there is step free access available at our base office, we cannot guarantee step free access in the external locations which form as part of this role.

Salary: £28,800

Shift Pattern: 37.5 hours per week Monday to Friday on a rota between 08:00 and 18:00 working 8 hour shifts. You may be required to work outside these hours as per service and participant requirements.

About the Role

We're hiring a Housing Support Worker to join our team based in Brent. In this role, you will support participants who have been or are at risk of homelessness, experienced social exclusion, mental health challenges, substance use challenges, learning disabilities and/or may have previous convictions. You will support participants on their journey towards recovery, reablement, and independence within a compassionate and trauma informed environment. 

In this role, you'll be at the heart of delivering high quality, person centered support through tailored support plans which provide practical interventions, to empower our participants to overcome personal challenges, develop life skills, and achieve their personal goals. You will also work closely in partnership with various organisations to support the different needs of our participants, which may include signposting. Join us on the journey to empowering people to be the best versions of themselves! 

Key Responsibilities Include:

• Support residents who may be facing challenges with their housing and long-term living
• Co-create tailored support plans through one to one sessions, group sessions, and multi agency meetings
• Support residents to build skills and confidence to build confidence to move towards independent living
• Signpost participants with local providers to support with their independence such as with Job Centres, employment support hubs, community support centres, mental health trusts and other as relevant.
• Lone working is part of the role, but you’ll always have a supportive team around
• Recognise support needs in an individual and initiate appropriate interventions to prevent further deterioration. 
• Complete safeguarding and other referrals where required.
• Keep support plans updated on our online portals as appropriate

About You

We're looking for a passionate individual who has a shared mission to help people reduce their risk of homelessness. You will understand that the needs of our participants may vary from mental health, substance use, lack of confidence, and social exclusion. You will be a self motivated individual who is able to be proactive in managing your own time during busy or quiet periods to ensure productivity. You will be able to work collaboratively as part of a team, and will have alignment with our SIG values. You will be keen to support our residents to achieve their individual goals and support them to be the best versions of themselves. 

• Previous professional or lived experience supporting people who have experienced homelessness and other complex needs
• Ability to engage with people from different backgrounds, build rapport, and empower them to achieve their personal goals
• Ability to show empathy and compassion and the different challenges people face; ability to come up with creative solutions to support them in their journey
• Ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs
• Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents
• Understanding of the different needs people with multiple and complex needs have and be aware of the social marginalisation that can be attached to people who face personal challenges

We are seeking a compassionate Activity Worker to join our 24/7 supported accommodation service based in Enfield which supports residents who face challenges with their mental health, either transitioning from acute mental health wards or other residential settings. Our team are here to support our residents with care and rehabilitation to help them to achieve their personal needs and goals.

In this role, you’ll work on the frontline delivering high-quality, trauma-informed support within a psychologically informed environment (PIE). You’ll create and deliver activities programmes designed to support recovery, reablement, and independence, helping residents overcome challenges and achieve their personal goals.

• Plan, develop, and deliver engaging group and one-to-one activities tailored to residents’ needs

About the Service

St Albans & District Foodbank is an independent charity within the Trussell network, operating nine foodbank centres across the district alongside a central warehouse and home delivery service. Over recent years, the Foodbank has developed a “More Than a Foodbank” model, recognising that food insecurity rarely exists in isolation and that many people accessing support are also experiencing poor mental health, debt, housing insecurity, social isolation and wider practical challenges.

Alongside emergency food provision, the Foodbank has developed strong partnership working with Citizens Advice St Albans District, local mental health organisations, statutory services and community groups to create a more joined-up and person-centred support model.

The Reaching Communities partnership between St Albans & District Foodbank, Citizens Advice and Hertfordshire Mind Network aims to provide integrated practical, emotional and wellbeing support within trusted community settings. About the Role

The Outreach Worker role is intended to feel fully embedded within the Foodbank environment and wider Foodbank Plus model, working relationally and practically alongside the Wellbeing Team, volunteers and partner organisations to support people experiencing hardship and complex life circumstances.

The purpose of the Hertfordshire Mind Network Community Outreach Worker role is to:

• Expand employment, health and wellbeing, and community engagement
• Strengthen existing service provision for financial advice, food security, and homelessness support.
• Build strong local partnerships, volunteer networks, and stakeholder relationships.
• Deliver measurable outcomes for residents, with a focus on those experiencing deprivation and marginalisation.
• To provide advice, information, onward referral and holistic support to clients with coexisting drug and alcohol difficulties, mental ill-health, these will be clients who are presenting to the police, anti-social behaviour, tenancy enforcement, housing and environmental health services within Hertfordshire.

We are looking for a Community Support Worker to join our team. The Primary Care Network Service works in partnership with GP surgeries in Hertfordshire to provide advice, information, onward referral and holistic support to individuals who are experiencing mental ill health or need support with their mental wellbeing. Supporting people in the community in their own homes and our wellbeing centres.

Community Support (PCN) Service Objectives

• To improve the mental wellbeing of people experiencing mental ill health.
• To increase early access to help for people experiencing mental ill health, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To remain a source of independent support for all clients.
• To ensure correct access via Primary Care into the HMN service therefore reducing the pressure on GP services

The Successful Candidate will

About the Role

The Mental Health Recovery Worker supports individuals within the community, to improve mental health and wellbeing, through focussed 1:1s and structured interventions. At times this may mean deescalating crisis.

The role will be part of a larger Recovery Lounge team made up of Mental Health Recovery Workers, Peer Support Workers and volunteers.

About the Recovery Lounge Service

Our Recovery Lounges offer a safe, welcoming and non-judgmental environment outside of normal working hours for those who feel in mental distress or crisis. The role will involve assessing needs and offering one to one support to service users to enable them to begin the process of improving their mental health and wellbeing and to providing clarity on services and pathways available in the area.

About You

The successful candidate will have experience of working with individuals experiencing mental health and wellbeing issues, experience of working in a small service within a wider organisation, including delivering a service against performance targets and have the ability to Establish and maintain constructive working relationships with a wide range of people. They will have good communication, motivational and negotiation skills and understand the wellbeing needs of and working with people with mental health needs, including those from culturally diverse communities.

About the role

The Community Link Worker will play a key role in supporting individuals who are economically inactive due to musculoskeletal issues and/or mental health needs, and with a desire to return to or get into work. An exciting opportunity has come up to oversee the operational management and delivery of our burgeoning volunteer-led music and mental health charity. • our inspiring and community-building events such as Open Mics and our annual Celebrate Seed Studios event

In 2025, We enabled over 800 hours of free volunteer-led sessions with an annual footfall of over 2500 as well as 24 hours of free, 1-to-1 music tuition support for referrals from our health partners every month.

We are looking for a passionate and committed Youth and Community Worker to join our Outreach Team at Croydon Drop In (CDI). This role involves working directly with children, young people and families across Croydon in community and educational settings, building trusted relationships and delivering high‑quality outreach, health education and emotional wellbeing support. You’ll work collaboratively with colleagues and partner agencies, including schools and statutory services, to respond to local needs and support diverse and vulnerable young people. Housing First Support Worker – Connect Community Trust (Somerset)
Salary: £27,669.10 per annum (pro rata) 35hrs a week

Are you passionate about walking alongside people as they rebuild their lives? We’re looking for a compassionate and committed Housing First Support Worker to help people move on from homelessness into secure, independent living.

This is a truly person-centred, community-based role where relationships are at the heart of everything. You’ll work with individuals who have often experienced significant trauma and exclusion, offering flexible, long-term support that adapts to their needs. Through patience, consistency, and trust, you’ll support people to maintain their tenancies, improve their wellbeing, and reconnect with their communities.

You’ll hold a small caseload, enabling you to provide meaningful, tailored support – whether that’s helping someone set up their new home, access health and recovery services, manage finances, or simply being a steady, reliable presence in their lives. Working collaboratively with local partners, you’ll play a key role in helping people build a stable and hopeful future.

About you:
You’ll bring experience of working with people facing homelessness, mental health challenges, or other complex needs, alongside a genuine belief that everyone deserves a safe place to call home. You’ll be resilient, empathetic, and able to work both independently and as part of a wider team. Strong communication skills are essential, as is a full driving licence and access to a vehicle, given the rural nature of the role.

About the Service

The Complex Needs Housing Service works as a multi-disciplinary team with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs. The service has been running since 2022 with a dual-district model, however, is now expanding to offer a single district outreach service.

The objectives of the Complex Needs Housing Service are to:

• Improve the mental wellbeing of people experiencing mental ill health.
• Increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• Contribute to an improvement in individual mental wellbeing.
• Remain a source of independent support for all clients.
• Reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
• Reduce the use of crisis services by people experiencing mental ill health without positive outcomes for the individual.
• Provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
• Work collaboratively within a Multi-Disciplinary Team (MDT) consisting of Change Grow Live (CGL), District and Borough councils, housing teams and other professionals.

About the Role

The purpose of the Hertfordshire Mind Network Community Outreach Worker role is to:

• To provide advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. These will be clients who are presenting to District and Borough councils as homeless and need intervention and support with their mental health and housing.
• To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
• To remain a source of independent support for all clients.
• Embed a person-centred recovery orientated and trauma informed approach in all aspects of the roles.

Main Purpose of Role:

· Devise and implement a mixed programme of community fundraising activities, to generate a sustainable source of unrestricted income to meet agreed targets.

Specific Responsibilities:

• Launch, implement and run innovative income generating projects to maximise donations to SWA, to look after relationships with existing donors and build on relationships created to ensure fundraising potential is optimised.

We are seeking a proactive, organised and relationship-focused Community & Volunteer Events Coordinator to lead the coordination and delivery of community projects, family activities and volunteer engagement opportunities across Camden and Islington.

This is a hands-on operational role focused on strengthening community connections, increasing family engagement and supporting inclusive, welcoming activities for families accessing our services.
