We are looking for two Adult Community Workers to join our team.

This is a deeply relational, hands-on role working alongside LGBTQ+ adults to build community, connection and belonging. You will assist in the facilitatation of inclusive groups and events, provide one-to-one support, and create spaces where people feel safe to be themselves and explore their identities, experiences and aspirations.

You’ll work directly with individuals and groups to support their personal, social and emotional wellbeing, while also ensuring that the voices of those we work with shape and influence the services we provide. Whether you’re running a group, supporting someone one-to-one, or helping deliver a community event, your role will centre on empowerment, inclusion and care.

You will collaborate closely with a small, passionate team to respond to community needs, develop meaningful programmes, and represent Proud2Be within the local community, including at events such as Pride.

We’re looking for a Senior Adult Community Worker to join our team.

This is a hands-on role supporting LGBTQ+ adults through inclusive groups, events and 1:1 support. You’ll help shape and deliver our adult services, while also supervising and supporting staff and volunteers.

Felstead Street is a CQC registered care home, which has a heavy focus on supporting customers with enduring mental health care and elderly needs. We provide 24hr high support to 24 customers both female and male. Our building is a purpose-built home on two floors, offering customers individual bedrooms and the use of shared communal facilities and gardens. The service is commissioned by East London Foundation Trust with the care contribution from London Borough of Hackney.

What you'll do:

• Building supportive, trusting relationships with customers and creating a positive atmosphere in the home.
• Following lone working protocols and maintaining high levels of awareness to health and safety for self, colleagues and customers.
• Supporting customers to maintain a healthy and clean living environment, where appropriate, provide assistance to maintain their living areas are clean and safe from hazards.
• Providing support and assistance to customers during the day who may be experiencing difficulties with their own mental health and seeking appropriate support.

At the heart of Camden’s mission to end rough sleeping, the Gray's Inn Road project offers short-stay accommodation for 16 individuals experiencing some of the most challenging periods in their lives. Funded by the Department of Levelling Up, Housing and Communities and the Greater London Authority, and delivered in partnership with One Housing Group and Camden Council, this service plays a pivotal role in ensuring rough sleeping in Camden is not only rare but brief and not repeated. Residents typically stay between three to six months, during which time the focus is on creating pathways into safe, sustainable housing and renewed independence.

You’ll be part of a dynamic, forward-thinking team dedicated to delivering trauma-informed, psychologically informed support. You’ll work closely with clients facing overlapping challenges including homelessness, poor physical and mental health, substance use, trauma and histories with the criminal justice system. With your own caseload, you’ll conduct in-depth assessments, build tailored support plans and facilitate interventions that reflect the needs, strengths and aspirations of each person. Every day brings new opportunities to connect people with education, training or employment, to help them rebuild practical life skills, and to foster progress towards long-term goals.

About the Role

Headway East London is looking for a compassionate and motivated Community Support Worker to support adults living with brain injury across our 13 London borough catchment area. You will support members in their own homes and local communities. This is a varied, people-focused role where you will work one to one in the community, helping individuals live with greater independence, confidence and choice.

Principal Duties and Responsibilities

1. Conduct support sessions to a regular caseload of members in the community as a lone worker with remote office assistance.
2. You will work to maximise and maintain their autonomy and independence in their home environment or within the local community.
3. You will work collaboratively with the member to plan, organise and implement daily activities by developing daily and weekly structure and wellbeing plans.
4. You will be responsible for being the first responder, with remote manager support, in the event of any incidents, safeguarding issues or behaviour that challenges during support sessions.
5. Accompany and support members to appointments with health and statutory care providers and provide accurate notes for future reference.
6. Accompany and support members to access social, leisure, educational and vocational activities in the community.
7. Maintain a maximum income for members through completing benefit and grant entitlement and assist with financial budgeting.
8. Provide appropriate and relevant advice, information and advocacy to members and their families. Where appropriate liaise with other professionals and statutory and non-statutory organisations to make referrals or signpost to appropriate support services.
9. Maintain accurate and factual monitoring notes on our online database and report any incidents.
10. Conduct and monitor specific therapy / support / rehabilitation and risk management plans in accordance with case managers, therapists and other cprofessionals as required.

This will include working with local communities and social groups who maybe ‘excluded’ and those groups currently unrepresented and/or deemed as hard to reach within the community and wider areas.

As a Forensic Mental Health Recovery Worker you will identify individual resident needs, and provide person centred, tailored support to residents to help them overcome their personal challenges and achieve their goals. We support our residents in developing coping strategies and building supportive relationships, with a commitment to professionalism, teamwork, and customer service. 

You will support with tenancy matters proactively, providing responsive support and fostering a positive environment where residents feel secure and valued. You will do this through a combination of producing tailored support plans, activities, and partnership working. You will support us continue to work in a trauma informed way, and create a psychologically informed environment, which supports us in creating a safe, respectful, and responsive environment which puts our people and communities at the forefront. Join us on our mission to empower independence through trauma-informed solutions and dynamic partnerships that keep people out of prison, out of hospital, and off the streets.

Key Responsibilities Include:

• Build and maintain positive therapeutic relationships with residents, offering both practical assistance and emotional support.
• Equip residents with the necessary skills, experiences, networks, and training to help them transition into semi‑independent or independent living.
• Provide support with personal needs as required by residents.
• Take part in, and encourage residents to engage in, the delivery and development of projects, initiatives, training opportunities, and volunteering activities.
• Manage a caseload of residents, contributing to the creation and review of support plans, risk assessments, and progress reviews.
• Identify residents’ interests, needs, and preferred activities through assessments, observations, and conversations.
• Recognise early indicators of declining mental health and implement suitable interventions to prevent crisis situations.
• Promote financial and economic wellbeing while supporting residents in reintegrating into the community.
• Complete safeguarding referrals and other relevant referrals when required.
• Additional duties may include, but are not limited to: keeping environments clean and organised, disposing of waste, reporting maintenance issues, ordering supplies, and undertaking other general tasks as needed.

Key Responsibilities Include:

• Build and maintain positive therapeutic relationships with residents, offering both practical assistance and emotional support.
• Equip residents with the necessary skills, experiences, networks, and training to help them transition into semi‑independent or independent living.
• Provide support with personal needs as required by residents.
• Take part in, and encourage residents to engage in, the delivery and development of projects, initiatives, training opportunities, and volunteering activities.
• Manage a caseload of residents, contributing to the creation and review of support plans, risk assessments, and progress reviews.
• Recognise early indicators of declining mental health and implement suitable interventions to prevent crisis situations.
• Promote financial and economic wellbeing while supporting residents in reintegrating into the community.
• Complete safeguarding referrals and other relevant referrals when required.

What are looking for:

• Experience of working with people of complex backgrounds, ideally within mental health
• Knowledge of risk management and risk assessment
• Knowledge of license conditions and public protection
• Knowledge of tenancy management, able to share knowledge with others in the team and residents
• Understanding of the housing and social needs of people with multiple and complex needs
• The ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs.
• Excellent interpersonal skills, both written and oral. Ability to form and build effective relationships and rapport with others
• Ability to influence and negotiate positive outcomes with others; Proactive in making decisions
• Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents

Villagers Connect Project – a great opportunity to join our team at Action in rural Sussex

Villagers Connect is a community development project which is working to sustain a strong and vibrant community in the villages of Colgate, Faygate and Rusper. It aims to build connections and relationships which increase confidence, independence and empowerment, particularly for the over 65’s.

We are seeking a part-time community development worker to work as part of a small team that is active in this rural location in the Horsham district. This role involves a good deal of community-based activity, engaging with older people and the wider community. This does therefore require someone who is understanding of older people and their interests, abilities and needs.

We champion an asset based approach to community development, which is about building on existing strengths and supporting local people to flourish; undertaking community led projects that create community cohesion and have lasting impact.

Key Objectives

· Support older people to access services and support locally that will meet their needs and improve their quality of life

· Improve the lived experiences for older people in these rural communities

· Build connections and relationships across and within the three parishes to reduce social isolation

· Develop new and support existing activities to enable a vibrant community

· Increase confidence, independence and empowerment amongst individuals, groups and community based organisations.

The Role
