About the role:

We are looking to appoint a Kentown Family Support Worker to deliver a high-quality family support service as part of the pilot Kentown Children’s Palliative Care Programme, in collaboration with Kentown Wizard and Together for Short Lives.

Reporting to the Kentown Project Lead out of our Lancaster team base and working in partnership with health, education and social care professionals, you will take responsibility for providing needs-led emotional, social and practical support to families where a child/young person has a life threatening or terminal illness.

Horsham Y Centre provides 24-hour supported housing services offering medium levels of housing-related support for young people aged 16-25 with 50 bedspaces and shared communal facilities. We adopt a trauma informed and psychologically informed approach to supporting our residents to help them build essential life skills, identify their goals and work towards aspirations so that they can move on to enjoy fully independent futures. We have a dedicated team of Support Workers, Night Workers and additional Bank Workers who provide support, guidance, and signposting around areas such as housing, budgeting, living skills, jobs, relationship building and wellbeing. Situated in the middle of Horsham, the service has strong links with and contributes to the local community.

We are looking for a Housing Night Worker to join our Horsham Y team. Main areas of responsibilities are:

Safety and security

• Provide a friendly and professional front-of-house welcome to residents, visitors and staff, and ensure the building is safe, secure quiet and welcoming 
• Maximise the wellbeing of residents and visitors by ensuring adherence to health and safety protocols and house rules, all within the terms of tenancy agreements
• Assess and monitor the risks presented by residents to ensure they can keep themselves safe and, where possible, continue their development
• Record all incidents and accidents and share appropriately with the wider team, your manager and, if necessary, the central safeguarding team 

Engagement with residents

• Welcome residents home and provide informal and responsive support so they can articulate their aspirations and ambitions and acquire the skills they need to live independent and fulfilling lives
• Ensure resident’s views, aspirations, concerns, and ideas are sought so they can play an active role in influencing the services they receive
• Proactively promote the services on offer and ensure residents are encouraged to engage with those services
• Contribute to, or support, evening groupwork activities so that residents can access tailored solutions that meet their needs, wishes and aspirations

Administration and housekeeping

• Undertake tasks as directed, including but not limited to: inputting data, assessments, reviews, case notes onto our client database and entering new residents details/closing ex-resident records
• Completing health and safety/compliance checks, and recording estate inspectors and room checks
• Adding maintenance requests to our property services system

General

• Work as part of a team, on a rota four on four off shift pattern, alongside another Housing Night Worker, ensuring young people at the service have non-judgemental, objective at night, along with taking responsibility for personal safety during periods of lone working
• Contribute to a great working environment, with a calm, yet assertive manner, being able to handle potentially difficult situations
• Participate in relevant continuing professional development and utilise Reflective Practice Supervision as part of leading psychologically informed practice

Age UK Bromley & Greenwich is seeking a proactive and compassionate Care Navigation Coordinator to lead a small team supporting older people as they transition from hospital back to the community. Based at the ICN Unit at Orpington Hospital, you will work closely with NHS colleagues and local services to ensure people on the Proactive and Frailty pathways receive the non-medical support they need to stay independent and well.

The Learning and Skills Development Co-Ordinator will lead on developing a program of training, learning, volunteering and work experience for the residents of Homeless Oxfordshire that increase self esteem, engagement and motivation. You will work collaboratively and creatively across the organisation, as well as with partners, stake-holders, and the wider community. Improving community relations and help us secure support from local training providers and employers where resident participation in the wider community will help break down stigmas surrounding homelessness and addiction.

Some of your responsibilities will include:

• Devise and deliver a structured programme of training and education (both formal and informal) to improve our residents' skills and confidence which prepares them for the next stage of their lives.
• As part of the LEAP team work closely with the Activities Co-Ordinator to deliver a comprehensive programme of engagement, developing skills to move away from homelessness.
• Work with partners and residents to design training that is tailored to the needs of our residents.
• Develop and maintain external relationships with local training providers, organisations and employers to find various opportunities and maintain those relationships.
• Support on design of the peer mentoring training and traineeship programmes.

About LEAP

LEAP is built on the foundation of strong, supportive relationships that empower residents to move towards stable, independent living. Through one-to-one key worker support, trauma-informed care, and community engagement, residents rebuild trust, form friendships, and gain confidence. Activities like gardening, arts and crafts, and creative writing foster connection and personal growth, while group involvement helps residents feel part of a wider community. LEAP also supports residents to take steps into peer mentoring, volunteering, and training, helping them build positive networks beyond their immediate circle and develop the skills and confidence to thrive independently.

At LEAP, community involvement is at the heart of their work. Empowering residents through co-production is a key organisational goal. LEAP offer a range of inclusive engagement methods—from informal gatherings like group breakfasts and creative activities, to structured sessions such as support needs mapping and Residents’ Forums. These forums provide regular opportunities for feedback and co-design, with input reaching senior management and trustees. They are enhancing resident engagement by developing a mutual support network for staff with lived experience of homelessness and/or addiction. LEAP also collaborates with local partners like RAW, a social enterprise offering training and employment to those with lived experience, and strengthens ties with adult education providers such as Abingdon and Witney College. We also create pathways to volunteer with cultural institutions like the University of Oxford Museums.

LEAP empowers people with lived experience of homelessness and/or addiction, helping them build the confidence and skills needed to move towards independent living and employment. Over three years, the project aims to support 750 individuals. Through a wide range of engaging and meaningful activities, residents will experience positive outcomes such as increased confidence, self-esteem, and a sense of inclusion. They will build supportive networks, learn key life skills and access training, volunteering, and employment opportunities. Many will gain qualifications and secure long-term accommodation. The project strengthens peer support and enables residents to shape services through formalised Residents’ Forums, ensuring their voices influence senior decision-making. For Homeless Oxfordshire, LEAP enhances our capacity to deliver peer-led support, builds staff knowledge, and informs service improvement. Wider community benefits include reduced stigma around homelessness, stronger partnerships with employers and organisations, and improved understanding of the issues faced by our residents—ultimately contributing to a more inclusive and compassionate society.

Young People Case Worker

We have a new role available for Case Workers to work as part of a team delivering an assessment, support and move on service to vulnerable clients in supported and community-based accommodation, based in Bishop Auckland and Tow Law County Durham.

Position: Young People Case Worker

About the Role

As Young People Case Worker you will work under the direction, guidance and support of a Service Manager and an Assistant Area Director, you will be responsible for a caseload of clients as well as share responsibility for the safety and wellbeing of all clients, volunteers and staff in the service.

The service consists of two hubs, 1 in Bishop Auckland and 1 in Tow Law and we have an Outreach service to 7 dispersed properties in Bishop Auckland. Your role will span all three areas for job rotation and continuity of service.

The team follow a 5-week shift pattern of 8-hour shifts between 0730 – 2200 hrs including weekends and bank holidays as one service is 24/7 and both sites have dedicated night staff.

Early shifts 0730-1530, Late shifts 1400-2200 and Main shifts 0900-1700/1000-1800 (Depending on hub)

The accommodation services are based in Bishop Auckland and Tow Law in County Durham. You will ensure that the accommodation is properly maintained and support the client to sustain their tenancy and participate in positive and successful move on. You will be responsible for a caseload of clients as well as shared responsibility for the safety and wellbeing of all clients, volunteers and staff in the service. This includes outreach support to clients in dispersed properties in the area.

The role does involve lone working.

Key responsibilities include:

· Ensure the safety and wellbeing of all clients, following safeguarding and emergency procedures as needed.

· Act as key worker for a caseload of clients, leading on risk and needs assessments and SMART, outcomes-based support plans.

· Provide personalised, flexible, and creative support, involving clients in decision-making and encouraging participation in activities.

· Collaborate with external agencies to support clients, adhering to data protection and information sharing protocols.

· Support clients to access Education, Training, Employment, and Volunteering opportunities aligned with their goals and aspirations.

· Contribute to maintaining a safe and welcoming environment, including preparing accommodation for new clients and supporting their transition.

· Assist in coordinating and supporting volunteers and promoting client involvement initiatives and residents' meetings.

· Maintain clear, accurate records and participate in team rotas, including evenings and weekends, carrying out additional duties as required.

About You

You will need to have the following skills and experience:

· Experience working with young people or those affected by homelessness.

· Understanding of the needs of individuals facing homelessness, mental health issues, substance misuse, or the care system.

The shift pattern for this role includes 4 days on, 4 days off with 12 hour shifts.

What you'll do:

Building supportive, trusting relationships with customers and creating a positive atmosphere
Carrying out holistic assessments of new customers which incorporate relevant statutory referral information
Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
Create support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customer to progress in their recovery
Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking
Provide support and access to relevant services to enable customers to continue their recovery and empower customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making
Undertake support work in partnership with external stakeholders to compliment their interventions
Developing productive relationships with partner organisations to improve service outcomes
