LSFRC is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.

The role of our Support Worker plays a crucial role in fulfilling our customers' expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Workers are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers.

An essential part of this role involves developing good partnership working with a range of professionals and maintaining excellent record keeping in line with our compliance management. You will also be required to oversee support planning, Risk Management Plans, and Key-working.

About The Role

In the role of Recovery Worker you will oversee a caseload of clients, working with a person centered approach towards their individual recovery. You will provide essential support and advice; empowering them to make relevant changes to thrive in their community. You will also work in partnership with a multi-disciplinary team of support staff, specialist staff and external partners and will be responsible for providing guidance and support to our residents, staff and visitors. 

Recovery Workers work flexibly on weekly shift rotas that include evenings, weekends and bank holidays.

Jigsaw4u’s Piece of Mind Service helps 11 to 25 year-old young people in SW London overcome depression and/or anxiety. We are seeking 1 full-time counsellor, or 2 part-time counsellors, to support 15 referrals over 3 months, in the London Boroughs of Kingston and Richmond. 

There is the potential for this fixed-term contract to become permanent. 

Each young person receives 6 to 16 targeted, evidence based therapeutic support sessions.  Support sessions will be delivered at our Kingston-Upon Thames hub, at school, home or in the community where appropriate. We’re looking for a motivated Health & Wellbeing Facilitator to join our dedicated team at Age UK Bromley & Greenwich.

About the Role
As a Health & Wellbeing Facilitator, you will play a pivotal role in delivering our Long-Term Health Conditions Programme. This includes leading self-management workshops, organising peer support groups, and empowering clients to make positive lifestyle changes. You’ll be a key player in helping individuals build confidence, enhance their health knowledge, and take control of their well-being. Working closely with colleagues and volunteers, you’ll connect clients to valuable services, create support networks, and make a tangible difference in their lives.

What You’ll Do:

• Work with the Lifestyle Coordinator to plan and deliver engaging health and well-being workshops for individuals with ongoing health conditions.
• Facilitate support groups, training and supervising volunteer peer mentors to provide targeted, condition-specific guidance.
• Collaborate with partner organisations to ensure a smooth referral process for additional services like Befriending, Carer Support, and Handyperson services.
• Provide personalised support through various channels, including face-to-face meetings, phone calls, and online communication.
• Help clients develop essential skills, such as managing medication, reducing stress, and communicating with healthcare providers.
• Monitor programme outcomes, maintain accurate client records, and contribute to continuous service improvement.

About You
We’re looking for someone with excellent communication skills, a proactive attitude, and a genuine passion for helping others. You should have experience working with individuals who face health challenges and a commitment to promoting independence and well-being. Knowledge of local resources and services is a plus, as is experience in organising health-related workshops or programmes.

Job Title: Resettlement Coordinator 

Location: This role is based in Royal Borough of Kensington and Chelsea (RBKC), the service is a short walk from Ladbroke Grove (Circle and Hammersmith) station. Unfortunately, this service does not have step free access.

Salary: £30,000 

Shift Pattern: 37.5 hours per week, Monday to Friday, 09:00 - 17:00. Flexibility may be required around these hours as per service and resident requirements, as well as activities which may fall outside these hours.

About the Role

We are hiring a Resettlement Coordinator to support residents who have complex needs and are ready to live independently out of residential support. You will provide practical support to residents who require help with finding and sustaining suitable accommodation and engaging with community based services such as councils, training providers, and local authorities.  Our residents face challenges such as with their mental health, substance use, and homelessness. In this role, you will support them in overcoming their personal barriers to they can move forward in achieving their goals. Our aim is to reduce inequalities and improve access to health, employment, training, and social care.

Some key responsibilities include: 

• Supporting residents to reach their goals, including to find suitable accommodation, through plans, assessments, reviews, and meetings.
• Developing and monitoring move on plans, liaising with partners to provide housing and resettlement support to residents.
• Developing and running clinics for residents to support with their move on journey through projects, individual sessions, group activities, and events.
• Providing advice, information, guidance, and life skills training to residents as required.
• Creating strengths based assessments and deliver holistic support and risk plans to help residents/participants achieve goals and stay safe

About You

We are looking for someone who is driven to provide high quality, effective, and person centred support to others. You will have a passion to help those who have experienced or face homelessness, wanting to make a difference to lives, and support those with multiple and complex needs. Our residents and participants come from all walks of life and previous experience and we put them at the forefront of our services. You will thrive in this environment and have a real desire to provide personalised support to others. 

• Confident leadership skills able to inspire and motivate staff and participants, ensuring effective delivery of move on plans.
• Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents
• Ability to promote the service and provide outreach based provision, with an ability to liaise and work effectively in partnership with stakeholders
• Understanding of the housing and social needs of people with multiple and complex needs
• Experience liaising with internal teams and external partners to progress housing outcomes and support resettlement plans.
• Ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs
• Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with complex needs

At an exciting point of growth, they're looking to recruit a Project Officer to support the delivery of advice and non-advice projects that make a real, tangible difference to people's lives.

About the role

As Project Officer, you'll play a key role in supporting the smooth delivery of a range of projects and campaigns. Working closely with the Projects & Partnerships Manager, you'll provide hands-on coordination, administrative support and data management, helping ensure projects are delivered efficiently, compliantly and on time.

You'll be part of a small, committed team, so you'll need to be confident working autonomously, using initiative, and building strong relationships across teams and with external stakeholders.

Key responsibilities

• Provide comprehensive administrative and coordination support across multiple projects

• Schedule meetings, prepare papers, take minutes and track actions

• Liaise with internal teams, external partners and lived experience groups

• Support the delivery of events, campaigns and engagement activities

• Maintain accurate project data and records using CRM systems (GDPR compliant)

About you
