The Role:
We are looking for a compassionate and proactive Mental Health Outreach Worker to join our team. You will deliver community-based crisis prevention and mental health support, with a focus on underrepresented groups, particularly racialised communities and LGBTQI+. 

This role combines crisis intervention, peer support, and holistic outreach to individuals with complex mental health needs, including co-occurring conditions, substance use, trauma, and suicidal ideation.

Key Responsibilities:

· Provide timely, community-based crisis prevention and support, including outreach to individuals leaving inpatient care.

· Engage marginalised communities to improve access to mental health services and reduce stigma.

· Build relationships with LGBTQ+ and BAME individuals and organisations, co-producing peer-led support initiatives.

· Deliver culturally appropriate engagement and awareness activities in partnership with local services.

· Provide holistic, person-centred support for individuals facing complex mental health challenges, addressing co-occurring issues such as substance use, trauma, and housing instability.

· Collaborate with statutory and voluntary sector partners to remove barriers to care, promote recovery, and improve wellbeing outcomes.

· Act as a peer and advocate, creating inclusive and safe spaces for individuals at risk of crisis.

The ideal candidate

To be considered for this role, you should be able to:

• Provide outreach and in-reach support to people affected by substance use across Tower Hamlets, including those who may have a history of rough sleeping, be vulnerably housed, or otherwise disconnected from mainstream services.

• Carry out person-centred assessments and work collaboratively with service users to develop support and recovery plans.

• Make timely and appropriate referrals to specialist services (e.g. treatment, health, housing, and social care) to ensure holistic support.

• Develop strong working relationships with partner agencies to coordinate joint support and reduce barriers to engagement.

• Promote harm reduction and recovery-focused approaches, ensuring that people are empowered in their choices.

• Maintain accurate records and contribute to monitoring and evaluation of the Reset service.

Our Youth and Play Practitioners will play a pivotal role in the Children, Young People and Families team, working as a core team of practitioners to ensure the smooth running of our programmes and services. In this role you will be part of a small and dynamic team which delivers a wide range of programmes and activities, both from Tulse Hill Adventure Playground and within local schools. In this role, the individual will support people experiencing mental health challenges by managing a caseload of clients who require early intervention or preventative support. They will deliver group activities and co-produced programmes, clearly communicating the service's aims and pathways to encourage active participation. The role involves empowering service users to engage with external professionals and community services, helping them work toward their goals. Working flexibly as part of a team, the individual will contribute to developing peer support networks and community-based activities, while also ensuring the safety, wellbeing, and confidentiality of all service users. Key responsibilities include maintaining accurate records, supporting service operations, ensuring timely referrals and reviews, and participating in regular supervision and development activities.

About the Senior Support Worker role:

You will be working in a dynamic service that provides short break accommodation for up to 6 adults with a wide range of learning, Autism and physical disabilities, sensory needs, challenging behaviours and complex healthcare needs.

The service provides 24/7 in-house respite for adults with learning disabilities, if you feel you can make a real difference by supporting people in all aspects of their care, to ensure they enjoy their stay, to enable them to reach their goals, to have fun, to go out in the community, to experience new things, to be as independent as they can be.

Skills and experience of our ideal Senior Support Worker:

• A caring, patient nature
• Have comprehensive knowledge and experience supporting adults with learning disabilities, Autism and challenging behaviour.
• Developing good practice and supporting the manager managing the service on a day to day basis including rota planning.
• Supervising, managing, and mentoring members of staff
• Ability to communicate effectively at all levels.
• Ability to develop and review support plans and risk assessments.
• Providing personal care, administering medication and lone working.

What you'll do:
Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. This includes providing personal care, domestic duties, emotional and social support in a person centred way. Staff will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices.
About you:
- If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. In some services, this will include delivering personal and physical care as appropriate
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
- Participate in the support planning and risk management, as facilitated by the lead Support Worker
- Carry out security duties to ensure the safety of the customers and premises
