Dementia Advisor Support Jobs in Home Based

An exciting opportunity has arisen for two Policy Advisors to join our dynamic and well-respected Policy and Public Affairs Team.

We want to ensure people affected by dementia benefit from progress in research, by providing evidence and insights to influence decision-makers. This ranges from making the case to increase investment in dementia research, making the case for earlier, more accurate diagnosis of the diseases that cause dementia, through to preparing health systems and to be ready for future dementia treatments.

We work closely with key stakeholders, including government and other decision makers, to shape policy on dementia prevention, research, and access to future treatments.

At a historic time in the field, with the first drugs that tackle Alzheimer’s becoming available, this is a fantastic opportunity for the post-holder to develop specialist knowledge, working closely with the Policy Manager, and to contribute to significant projects that can change the future of dementia.

We are offering two permanent, full-time positions with a focus on; early and accurate diagnosis and clinical engagement and health systems.

Each role will require weekly attendance at our offices on Granta Park near Cambridge, with occasional travel to meetings and events in London and other locations.

 

Main duties and responsibilities of the role:

· Develop and maintain up to date and detailed knowledge of the policy landscape for a given policy area to position Alzheimer’s Research UK as a thought leader and enable us to effect positive change.

· Proactively identify and develop opportunities to engage with relevant stakeholders.

· Co-ordinate and manage assigned projects within the policy team.

· Gather and synthesise necessary evidence, including statistical information, health policies, and scientific outputs to develop insights, working with colleagues across the organisation as appropriate.

· Use insights to develop a range of outputs, such as scoping activities, briefing documents, literature reviews, reports, summaries and presentations for internal and external audiences.

· Develop relationships with internal and external stakeholders to help shape, deliver and influence the outputs of the projects, ensuring ongoing good communication with key stakeholders.

· Use public and patient involvement approaches to support insight development as appropriate.

· Analyse, interpret and present data using both quantitative and qualitative techniques as appropriate.

· Contribute to a range of submissions to government departments and their agencies, parliamentary committees, and other UK and international organisations as appropriate.

· Help develop and roll-out influencing and project ideas with key audiences that deliver against Alzheimer’s Research UK’s policy objectives.

· Support the work of Public Affairs colleagues to strengthen existing and emerging relationships with political and other parliamentary stakeholders. This will include helping to draft parliamentary questions, promoting tweets, sharing insight across and beyond the team.

Undertake any other relevant duties and projects delegated by the Policy Manager in line with the responsibilities of the post.

Additional Information:

Ways of working: As part of our Agile ways of working you will be required to work approximately 2 days a week from the office, which is subject to the requirements of the role and the business needs. Flexibility on where you work can be split between working from home and our office.

Roles that are classed as part of the Agile ways of working are not able to claim any costs for Mileage/Travel on Public Transport, Accommodation and/or Meals.  This includes when attending the office for various meetings/events.

Salary: Circa £29,000 per annum, plus benefits.

As a community-based worker, you will be required to attend a range of meetings, appointments and venues as well as working from home.

This role offers an exciting opportunity to grow and develop a Dementia Service that was launched at Age UK Westminster in 2022. Offering a weekly community-based Maintenance Cognitive Stimulation Therapy group that provides a vital contribution to living well with dementia and a monthly Memory Cafe, we are looking for someone with the knowledge, skills, experience and passion to ensure that more residents can benefit from this Service. 

Summary of role Age UK Westminster launched its Dementia Service in 2022 supporting older people (aged 50+) living in the City of Westminster who are experiencing memory and cognitive impairment and are experiencing confusion and dementia resulting in severe social isolation. The service also supports those who are caring for people with these issues.

In 2024 the ambition is to grow the reach of the successful monthly ‘Heart of Westminster' Cafe delivered in partnership with the Salvation Army’s Regent Hall and a weekly community-based MCST group and provide expertise to the Befriending team to manage some befriending matches. Demonstrating the value of these community-based activities and ensuring they are supported by a trained and supported volunteer team will contribute to securing continuation funding.

The role will involve close liaison with Westminster’s Memory Service and other community organisations that support older people living with memory loss and dementia.

Key responsibilities
● Co-ordinate the successful running of the monthly ‘Heart of Westminster’ Cafe in the Soho ward of the City of Westminster.
● Co-ordinate the successful running of up to two weekly MCST groups at Marylebone Church in Westminster including the co-ordination and triage of referrals for MCST groups
● Ensure the design and provision of activities and information that will enhance the lives of those attending and their carers. There is a significant resource bank of materials and activities
● Facilitate an environment where acceptance of dementia will bring about positive feelings for anyone affected by it
● Promote the service across Westminster to ensure that all those who would benefit are able to attend. This will include working with other professionals and ensuring that publicity materials are available
● Working with colleagues at Age UK Westminster and other professionals to identify and assess individuals who would benefit from being matched with a trained dementia volunteer. Home-visiting may be required.
● Deliver dementia awareness training to staff and volunteers
● Working with Age UK Westminster’s Volunteer Coordinator grow the volunteer resource
● Provide ongoing support and supervision to the team of Dementia volunteers
● Ensure that all records are kept up to date for the service and be responsible for the implementation of monitoring, evaluation and quality assurance procedures

Alzheimer’s Society have an exciting opportunity to join our talented team as a Dementia Adviser in the East Yorkshire area. We are working hard to improve diagnosis rates for dementia and this exciting role involves organising and attending ‘Brain Health Pop Ups’ around the whole East Riding to support people worried about their own brain health. Giving advice about how to approach their GP to gain a diagnosis and supporting people in a relaxed and supportive environment (usually libraries, leisure centres and garden centres). Working closely with the East Riding team and signposting for referrals is a key part of the role also.

You will;

- Assist people with dementia and their carers in identifying their needs, providing information and guidance to help maintain their independence, improving their sense of well-being, and putting them in more control of their lives.
- Build relationships with a range of local contacts, networking with health and care professionals, and providing dementia support. 
- Develop networks and pathways with partner organisations who work in the field of dementia, ensuring seamless and consistent support and empowering individuals affected by dementia to make informed choices.

We are looking for someone who is enthusiastic, supportive, shows empathy and is a good listener. You will be meeting people all over the East Riding who are a little anxious about their own brain health or just curious about what options they have.

South East London Mind is working with Oxleas NHS Foundation Trust and Bridge Support to deliver the Greenwich Mental Health Hub.  This will form part of the new transformation of mental health services in the Royal Borough of Greenwich under the NHS Long Term Plan. The service will bring together the expertise of local primary, secondary and voluntary sector mental health care providers with the aim of providing flexible, holistic, and integrated services for adults with mental health problems.

Unpaid carers in our communities are supporting relatives, friends and neighbours, often with limited or no support, to live the best lives they can.  The support of people with dementia, learning or physical disabilities, mental health difficulties or substance misuse problems by unpaid carers has been estimated to save the government close to the equivalent expenditure for the whole NHS.

We are looking for a Mental Health Carers Advisor who possesses an understanding of the challenges of unpaid caring and whose experience of working within mental health services includes support, prevention, recovery, and self-management. Applicants should possess effective skills in communication and have experience of completing assessments, managing a diverse caseload and working in a busy work environment. An ability to work independently but collaboratively, to be proactive and to show initiative is essential for this role.

The role will involve:

• Management of a diverse client caseload
• Providing recovery-orientated individual and group-based support to carers of adults with mental health problems
• Collaborative working with all partners involved in Greenwich Mental Health Hub work
• Undertaking holistic needs and risk assessments
• Commitment to regular training and supervision
• Effective contribution to the development and continuous improvement of the service

About the role
 
Location: community-based in Bridgend with some office and home-based working.

We have an exciting opportunity to join a multi-disciplinary team as a Dementia Adviser. You will have the rewarding experience of providing information, guidance and support to people affected by dementia in Bridgend. You will help them in identifying their needs, maintain independence, improve their sense of well-being, and putting them in more control of their lives. This post forms part of the Team working around individuals and their families who are experiencing difficulties with changes to cognitive, emotional, or physical health needs. 

This new post will work as part of the Integrated Community Network teams in Bridgend. The Network Teams consist of staff across several professions. This includes social work, district nursing, community psychiatric nursing, occupational therapy, physiotherapy, speech and language therapy, dietetics and pharmacy. It aims to provide a seamless and holistic approach to an individual’s care or support needs during times of change or challenge.

As a Dementia Adviser, you will:

- Work closely with health, adult social care and third sector colleagues in assessing need and providing information and guidance to support people affected by dementia.
- Help meet the health and wellbeing needs of an individual by looking at “what matters” to them by providing timely and accurate information, advice, and assistance so that they may benefit from what is available to them.
- Work within the Integrated Community Network Teams in supporting the provision of support to people with dementia and their carers and wider support networks. 
- Deliver specific components of the service users care plan as delegated by the Integrated Network Team professional and will be required to implement care plans within the scope of their competence.
- Build robust working relationships and develop networking opportunities with a range of local partners and community organisations. 
- Create pathways with partner organisations who work in the field of dementia, ensuring seamless, holistic consistent support and empowering individuals to make informed choices.  
- Provide dementia support face-to-face, by telephone and online.

About you

We are looking to welcome an approachable, enthusiastic, and knowledgeable Community-based Dementia Adviser who will focus on supporting people affected by Dementia across Bridgend. 

You will have:

- A passion for supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives
- A solid and compassionate understanding of the needs of people affected by dementia
- The ability to work inclusively with people affected by dementia, along with an understanding of the need for client confidentially
- Great communication skills, adapting your approach to ensure understanding at all levels
- Effective networking and influencing skills and the ability to manage several tasks at once and keep yourself motivated to perform under pressure
- Good IT skills and experience working with databases and virtual meeting software (e.g., MS Teams and Zoom)
- The ability to travel across Bridgend independently when required

About the role

We have an amazing opportunity to have a rewarding career providing support and guidance to people with dementia and their carers. This is a home-based role, with local community engagement and home visits to service users.  Our passionate and knowledgeable team is part of a new Multi-agency contract, including Disability Cornwall, Memory Matters, Alzheimer’s society and Cornwall Foundation Trust, working alongside the Primary Care Dementia Practitioners, to provide support to individuals affected by Dementia county wide.

We provide information and support in people’s homes and at locations across Cornwall.

- Offering a vital and compassionate advice service to support and guide those affected by dementia.
- Provide community based, face to face services for an increased level of support for more complex cases across Cornwall.
- Managing referrals, assisting clients in assessing their information and support needs in a person-centred manner.
- Providing knowledgeable and trusted information regarding services and benefits available and, where appropriate, assist clients in accessing them.
- Signposting clients to a choice of suitable other sources of help, where appropriate.
- Developing a proactive approach in reaching people with dementia and carers who may not otherwise access our services.

There is no direct care element to the Dementia Adviser role, however we work passionately to provide advice, support and guidance to people affected by dementia. This role will support people who have received a Dementia Diagnosis and help provide the tools and knowledge to make informed decisions about their future post diagnostically.

About you

We are looking for a highly motivated individual, with an understanding of dementia, and the needs of those affected. The successful candidate will be approachable and knowledgeable to join our passionate team.

- A passion for supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives
- A solid and compassionate understanding of the needs of people affected by dementia

- The ability to work inclusively with people affected by dementia, along with an understanding of the need for client confidentially
Great communication skills, adapting your approach to ensure understanding at all levels - Effective networking and influencing skills and the ability to manage your time effectively.
- Good IT skills and experience working with databases and virtual meeting software (e.g., MS Teams and Zoom)
- The ability to travel across Mid- Cornwall area independently when required
- Excellent partnership working and communication skills

About the role

As a Dementia Adviser you will have the rewarding opportunity to provide support and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs.

- Using a person-centred approach, assess people’s needs so they can access information in their preferred style.
- Work closely with other teams and services within the Alzheimer’s Society to support people affected by dementia.

We have an opportunity to work for our Adult Carers Team as an Adult Carers Support Advisor. You will provide emotional and practical support to Adult Carers in the London Borough of Bromley, aligned and working closely with the integrated care networks.

• To offer emotional and practical support to build resilience for carers on a one-to one basis.
• To provide support face to face, over the phone and in group settings.

Supporter Engagement is one of the most important teams that is central to the success of Alzheimer’s Society. The function of the team is to manage multi-channel communications with our supporters, both inbound and outbound. The team manage emails, telephone calls and social media interactions and is part of the wider Supporter Experience Department. 

The team has a very bespoke tone of voice when managing the high volume of interactions but has the autonomy to manage their own workday within guidelines. Support is second to none and we pride ourselves on having a very open communication style between all members of the team so that help is available for you to be successful in your role.

In this role you will be the first point of contact delivering an excellent supporter experience to members of the public and Alzheimer’s Society’s supporters. You will have enriching one to one conversations across different channels, including phone, email and Social Media. Through answering queries and proactively delivering exceptional levels of stewardship you will build rapport and inspire supporters and potential supporters about our work, whilst maintaining accurate system records that reflect and enhance the supporter experience.

Job Purpose: To develop and manage, to a high standard, Age UK Bromley & Greenwich’s Information and Advice Service. To be responsible for the effective delivery of the I&A Service which will be delivered through a range of channels, including face to face, telephone, email, digital, and other means, across the Bromley and Greenwich boroughs.

Key Responsibilities:

Line management and supervision

• Provide line management and supervision to advisers and support staff.
• Supervising team advice sessions and casework
• Manage the practicalities of the advice session and ensure adequate staffing and resources.
• Undertake monthly supervision with individual team members and annual appraisal.
• Ensure referrals are actioned and clients are supported in a timely manner.

Support learning and development

• To manage networking/ liaison with other local key stakeholders and organisations and feed into wider local engagement and community development work.
• Develop effective partner relationships with referral agencies across  both boroughs, and to refer/ signpost cases to external agencies as appropriate.
• Represent the organisation at outreach and community events as required, continuing to raise the organisational profile and develop wider awareness of our services.
• Work with colleagues to develop marketing materials, give talks and presentations, identify outreach opportunities .

Age UK Sutton are looking to hire an experienced manager who has a background or interest in community delivered health services.

At our independent charity based in London Borough of Sutton, the Head of Health Services will lead on and develop Age UK Sutton's community health commissions including; our Hospital Discharge service which provides practical support to older people in the first days and weeks after discharge from a hospital admittance; Dementia Peer and Practical support service, delivering information, advice, peer support and some practical support to those living with or affected by Dementia; and the innovative Older Person's Mental Health Peer Support service operating in partnership with Sutton's Community Mental Health services and South West London Mental Health Services.

The Head of Health Services will have delivery objectives to achieve with their operational team, with current direct line management of 9 staff over 3 services and have strategic development opportunities to deliver excellent services to the older population in London Borough of Sutton.
