Are you interested in supporting people at immediate risk of rough sleeping and making a real difference to the lives of people who are homeless?
This exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.
Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.
The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be expected to work across all 3 services .
- 7 day rolling rota including early, mid and late shifts
- (K&C): medium – high support accommodation.
- (Lambeth): medium support accommodation.
- (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.
Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.
The service will work in partnership with the local authority to secure onward move-on options e.g. access the council PRS scheme, advocating for access to supported housing.
As an Assessment and Reconnection Worker, you will:

Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.

The role:
Taking on a lead role to deliver targeted street work and outreach across the borough of Tower Hamlets. This involves engaging and building relationships of trust with young people at street level and in identified hotspot areas of risk where there is low to high anti-social behaviour, criminality and violence. Working with and engaging young people in strategic interventions across Lifestyle, Enterprise/Employability, Education and Environment that constructively challenge negative patterns of behaviour, create an experience for positive thinking, confidence building, entrepreneurial work skills, and tapping into the young person’s ambition, potential and talent.
You will have senior responsibility with the Frontline Director to plan and execute street work and outreach delivery in identified hotspot areas or risk based on street/community and partner intelligence. Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. Teresa House is a fast stream service where customers are supported up to two years while Hamlets Way is a slow stream service where customers can expect support for a minimum of two to a maximum of 4 years.
The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. About the role

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. We are recruiting Waking Night Support Workers to join our dynamic service and team. In the role of Waking Night Support Worker; individuals will ensure continues support is provided to young people at the standards of day staff with particular focus on safety of vulnerable young people.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Up to NVQ Level 2/3 or equivalent or experience supporting young people or people experiencing mental health challenges.
