Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. Applicants will need to be pro active and support our customers to enhance their lives.

What you'll do:
- Undertake key-working responsibilities for a caseload assigned by Management staff
- Meet customers regularly for support sessions, providing emotional and practical support

- Develop support plans with customers to identify their goals, ensuring a person centred approach, enabling the customer to establish positive change

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.

About the role:
As a Resettlement Worker and Domestic Abuse Specialist, you will work in person and remotely with clients who have support needs, in particular, those who are survivors of domestic abuse. In conjunction with other members of the Partnership, you will support our clients to engage with services and find accommodation solutions. Main duties:
- To work with clients with support needs, to engage with their prevention, relief or advice goals by; Agreeing with them a support plan to deliver the actions identified in the Personalised Housing Plan.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.
This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.
We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.
Main Responsibilities:
· Undertake 1:1, group, detached and outreach work with young people
· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.
· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.
· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting

Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Look ahead provides the support function and Riverside provides the housing management and night concierge. The Margins Project, Union Chapel’s Frontline charity, work with people who are homeless or who are at risk of homelessness and helps address pertinent social justice issues these communities face.
The charity runs three integrated programmes:
• a community drop-in programme providing access to nutritious hot meals, and limited access to showers & laundry services.
• advice and engagement programme providing one-to-one as well as group advice service on benefits and housing, and onward referrals to pertinent services.
• support employment programme offers a paid catering training to 10 people who have had experience of homelessness, substance misuse, mental ill-health, and or imprisonment.
Our range of services are designed to help guests (service users) at varying stages, from having no roof over their head to getting back into employment.
This role offers an opportunity for an experienced adviser to help shape and evolve Margins services. Experiences on the ground at the drop-in programme inform the charity’s strategic direction. The role will involve expanding our partner network to link into pertinent statutory and other charitable organisations that provide services that will help our guests with their housing, substance misuse, recidivism, and mental health challenges.
We work closely with people on an individual case work basis to get guests off the streets, into hostels or temporary accommodation where possible, and work with private and social landlords on a range of issues. We support and represent the people we work with on the phone and in person, including at the council, the doctors, the job centre, and benefits tribunals.
Our drop-ins run on Mondays and Wednesdays 11:00am to 1:00pm where attendance is currently from 65 to 75 guests. The advice and engagement worker works more intensively with smaller numbers of up to 20. The demand for more follow up work and advocacy support has been increasing over the last two years and this in-house advice worker is critical for this demand.
Working collaboratively with the Drop-in Community worker (currently vacant), the head chef, and a small group of dedicated volunteers, you will be responsible for the support of a caseload of guests, some with complex needs, and will undertake key work responsibilities including registering the guests, identifying their housing, benefit and personal needs and conducting all necessary follow ups including occasional advocacy support, where deemed necessary.
